Estimating population immunity against serotype-two poliomyelitis from the inactivated polio vaccine in routine immunization across 112 countries: A modelling study.
To mitigate the risk of outbreaks of serotype 2 poliomyelitis after withdrawal of this serotype from oral poliovirus vaccine (OPV) in 2016, inactivated poliovirus vaccine (IPV) was introduced into the routine immunization (RI) programmes of all countries using OPV. Since 2022, WHO has recommended a 2-dose schedule, with a first dose at 14 weeks of age followed by a second dose at least 4 months later (e.g., 14-39 week schedule), although an earlier schedule may be adopted, despite lower immunogenicity, if vaccine coverage is low at older ages. We combined published data on type-2 IPV seroconversion with age, national RI coverage estimates, dose introduction dates, and country-specific schedules using a cohort model of population immunity to estimate IPV-induced immunity from 2024-2031 for 112 countries using either one or two doses of IPV. We projected immunity for current, 6-14, and 14-39 week schedules to find the optimal schedule and estimate the impact of interventions such as schedule changes and catch-ups. Under current schedules, estimated median serotype 2 population immunity in 2025 among children under five years of age is at 61% (IQR: 52%, 72%), rising to 71% (IQR: 57%, 80%) in 2031. The later 14-39 week schedule was optimal in all countries, with potential for the median immunity to rise to 78% (IQR: 66%, 85%) by 2031 if adopted by all countries in 2026. Eight countries would still have <50% immunity, rising to 65%-72% if catch-up campaigns with 80% coverage were implemented in 2030. The work is limited by the fact that IPV provides only a partial picture of total immunity where there has been emergency type-2 OPV use. Furthermore, national estimates may mask subnational coverage differences and pockets of extremely low immunity. Under these estimates, IPV schedules and coverage are suboptimal in many countries. Those with a single dose should introduce a second on the 14-39 week schedule; those on early schedules would benefit from adopting the 14-39 week schedule. IPV catch-up campaigns are recommended where RI coverage is low.
Leveraging community insights and navigating logistical challenges: a case study of the 2024 polio vaccination campaign in Gaza, State of Palestine.
This case study describes how high vaccination coverage was achieved during the 2024 polio vaccination campaign in Gaza amid an ongoing humanitarian crisis marked by damaged critical infrastructure, an obstruction of the entry of health supplies, mass displacement and security concerns for health workers. Despite the immense challenges, 559 161 children were vaccinated in the first round (94% of the revised target of 591 714) and 556 774 in the second round, exceeding expectations. It also highlights how access and availability, not parental reluctance (or vaccine hesitancy), were the primary constraints on coverage. Parents showed strong willingness to vaccinate when services were accessible. Sustained advocacy and diplomacy remain essential to secure access, deliver equitable immunisation and create an enabling environment for humanitarian public health responses in conflict settings. However, the gains achieved through this campaign cannot meaningfully be sustained without a lasting ceasefire, the restoration of essential services and ongoing humanitarian access, particularly for infants born after the vaccination campaign who remain at heightened risk.
Evaluation of hyper-attenuated S19 poliovirus strains for use in poliovirus neutralization assays.
Poliovirus eradication efforts have relied on effective vaccination campaigns and stringent laboratory containment of poliovirus strains. Serological testing is the primary means to address population immunity, vaccine efficacy, and individual poliovirus exposures. However, strict containment requirements for Sabin type 2 and wild poliovirus types 2 & 3 limit access to serological testing. To address these limitations, we evaluated the performance of hyper-attenuated, temperature-sensitive S19 poliovirus strains (S19-PV), which retain the antigenic characteristics of Sabin and Salk strains but can be handled under biosafety level 2 (BSL-2) conditions. Using a modified microneutralization assay, we performed a comparative validation of S19-PVs to their Sabin and wild-type counterparts using WHO international standards, in-house control reagents, and human sera. Neutralizing antibody titers showed strong concordance between S19-PV and reference strains (R² = 0.93-0.99), with high intra- and inter-assay reproducibility (CVs < 10%). Our findings demonstrate that S19-PV strains are antigenically equivalent to Sabin and wild-type viruses and suitable for use in neutralization assays to determine levels of poliovirus neutralizing antibodies outside of strict containment laboratories. Adoption of these strains can significantly expand global capacity for poliovirus serological testing, supporting surveillance and immunization activities in the final stages of poliovirus eradication.
Assessing Seroprevalence and Infection Dynamics of Oncogenic Gammaherpesviruses in South African Paediatric Patients Presenting with Inflammatory Conditions.
Kaposi's Sarcoma-associated herpesvirus (KSHV) and Epstein-Barr virus (EBV) are oncogenic gammaherpesviruses with high prevalence in sub-Saharan Africa. Both viruses are typically acquired during childhood, establishing lifelong latency. While viral reactivation into the lytic cycle has been mainly studied in adult HIV-infected populations-and more recently in the context of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) co-infection-the dynamics of KSHV and EBV infection in children remain poorly understood. Here, we characterize pediatric patients (n = 175; median age 4.6 years; IQR 2.0-8.3) presenting with inflammatory conditions during the COVID-19 pandemic in South Africa (from July 2020 to February 2024). Including a healthy, non-inflammatory control group, we found widespread exposure to SARS-CoV-2 (70.9% seropositivity), with 72.6% of the children being seropositive for EBV and 19.4% for KSHV. There were no significant differences in seroprevalence between children with inflammatory conditions and healthy controls for any of these viruses, although SARS-CoV-2 antibody titers were significantly higher in the inflammatory group, while EBV immune responses were lower in children presenting with inflammation. Among the KSHV-seropositive children, no active viremia was detected (as determined by the absence of viral DNA in the peripheral blood). In contrast, 34.4% of EBV-seropositive children had detectable EBV viral load, with a modestly higher proportion in the inflammatory group. However, EBV viral load levels were comparable between children diagnosed with Multisystem Inflammatory Syndrome in Children (MIS-C), Kawasaki Disease (KD), and other inflammatory conditions. Logistic regression analyses revealed that increasing age was significantly associated with higher risk of SARS-CoV-2 and EBV seropositivity, but not KSHV. Notably, the risk of EBV DNA detection in the peripheral blood decreased with age. In summary, this study suggests effective immunological control of gammaherpesvirus infections in HIV-negative paediatric patients, even in the presence of inflammatory conditions that might otherwise trigger viral reactivation.
Chronobiology of neurotropic viruses: rhythmic viral entry and arrhythmic host clocks.
Neurotropic viruses invade neural tissues, resulting in severe diseases such as poliomyelitis, rabies, herpesviral encephalitis, and viral meningitis. Given this neurotropism, we investigated whether the infection of the host by these viruses is under circadian control. In this study, we found that the expression of most neurotropic virus receptors exhibits rhythmicity across cells, cerebral organoids, and animal models, with host cell susceptibility modulated by the circadian clock. We identified E2F8 as a clock-controlled gene that mediates the indirect regulation of the circadian clock on neurotropic viruses. Notably, E2F8 regulated the expression of core clock components by binding directly to the promoters of REV-ERBα and PER2, suggesting its role as a potential modulator of circadian rhythms. Additionally, we revealed a seldom-recognized viral strategy to accelerate viral replication in the host: rabies virus disrupts the host circadian clock system primarily through its glycoprotein hijacking the E3 ubiquitin ligase HUWE1 to inhibit proteasomal degradation of REV-ERBα. These findings increase our understanding of the interactions between circadian systems and neurotropic viral dynamics and highlight the potential of chronotherapy for improved antiviral treatments.
