Digital twin frameworks for polio and post-polio neurodegeneration: Toward predictive, personalised lifelong neuro-rehabilitation.
Poliomyelitis is a neuroinvasive viral disease that, despite near-global eradication, has left millions of survivors worldwide with lifelong motor-system injury. Many develop delayed neurological decline decades later in the form of Post-polio syndrome, characterised by progressive weakness, fatigue, pain, and respiratory compromise. Disease trajectories are highly heterogeneous, unpredictable, and shaped by complex interactions between residual motor-neuron pools, muscle adaptation, ageing, metabolism, and immune tone. Conventional care relies on episodic clinical review and reactive rehabilitation, with limited capacity for long-term personalised forecasting. Digital-twin technology, defined as an adaptive computational model that continuously mirrors an individual's biological and functional state, offers a transformative framework for predictive and precision-guided lifelong polio care. By integrating neuromuscular physiology, biomechanics, metabolic status, wearable sensor data, and rehabilitation responses, digital twins could enable real-time modelling of motor-unit decline, functional reserve, respiratory vulnerability, and fatigue dynamics. This article outlines a focused conceptual framework for polio digital twins, emphasising model architecture, feedback loops, and translational applications, rather than virological biology. The proposed framework positions digital twins as a missing precision-medicine layer for a neglected global survivor population.
Determining the presence of restless legs syndrome in patients with poliomyelitis sequelae and its effect on pain, fatigue, sleep and quality of life.
Investigation of Restless Legs Syndrome (RLS) in patients with poliomyelitis sequelae (PS) and the effect of RLS on pain, fatigue, sleep and quality of life (QoL). We investigated the presence of RLS in patients with PS who consecutively applied to the neurology outpatient clinic and did not have secondary causes of RLS. Gender, age, side affected by PS, leg muscle strength and length, International RLS Study Group criteria, Pittsburgh Sleep Quality Index questionnaire, Fatigue Severity Scale, SF-36 Form scores were recorded. After receiving a diagnosis of RLS, patients completed the RLS Severity Rating Scale (RLS-SWS) questionnaire. We identified RLS in 12 (37.5%) of the 32 patients included in the study. There were no differences in demographic characteristics. The leg lengths in the patients with RLS (PwRLS) were significantly shorter (p < 0.001). PwRLS had shorter sleep duration, longer latency, and worse sleep quality and fatigue severity scores (p < 0.001). The pain score was found to be statistically significantly higher in the PwRLS group (p = 0.007). There were no differences in other QoL areas. In the PwRLS study, the RLS-SWS score was positively correlated with sleep latency, sleep quality, and pain and fatigue scores. It was also negatively correlated with sleep duration (p < 0.001). These findings demonstrate that RLS, which has an effective treatment, is a prevalent condition among PS patients that worsens sleep quality and increases pain. Clinicians should screen PS patients who complain of leg pain and/or sleep disturbances for RLS and provide appropriate treatment.
[Late-onset sequel states of poliomyelitis (postpolyomelitic syndrome)].
Poliomyelitis remains a Public Health challenge in countries with low vaccination coverage and outbreaks of vaccine-derived polioviruses. The aim of this paper was to review the current evidence on post-polio syndrome (PPS), including its pathophysiological basis, clinical manifestations, diagnostic criteria, and current therapeutic approaches. A narrative review of PubMed, Scopus, Web of Science, and World Health Organization documents was conducted, prioritizing systematic reviews, cohort studies, and clinical trials published between 1980 and 2024. PPS affects 20-50% of polio survivors. It is characterized by progressive muscle weakness, fatigue, and pain decades after the initial infection. Its etiology remains unclear; proposed mechanisms include degeneration of reinnervated motor units, neuromuscular dysfunction, viral persistence, and autoimmunity. No pathogenetic treatments are available; management is symptomatic and multidisciplinary. PPS is a chronic condition with high prevalence among poliomyelitis survivors. Current treatment focuses on rehabilitation, respiratory care, fatigue management, and psychological support. Further research is required to develop specific therapies. La poliomielitis sigue siendo un desafío de Salud Pública en países con bajas coberturas vacunales y con brotes de poliovirus derivados de la vacuna. El objetivo de este artículo de revisión fue revisar la evidencia disponible sobre el síndrome postpoliomielítico (SPP), sus bases fisiopatológicas, manifestaciones clínicas, criterios diagnósticos y abordajes terapéuticos actuales. Se realizó una revisión bibliográfica narrativa de artículos publicados en PubMed, Scopus, Web of Science y documentos de la Organización Mundial de la Salud, priorizando revisiones sistemáticas, estudios de cohortes y ensayos clínicos entre 1980 y 2024. El SPP afecta a entre el 20%-50% de los supervivientes de polio. Se caracteriza por debilidad muscular progresiva, fatiga y dolor, tras décadas de estabilidad clínica. Su etiología es desconocida; las teorías incluyen degeneración de unidades motoras reinervadas, disfunción neuromuscular, persistencia viral y autoinmunidad. No existen terapias específicas; el manejo es sintomático y multidisciplinar. El SPP constituye una entidad crónica de alta prevalencia en sobrevivientes de la poliomelitis. El tratamiento actual se centra en rehabilitación, cuidados respiratorios, manejo de la fatiga y apoyo psicológico. Se requieren más investigaciones para desarrollar terapias específicas.
Metabolic Syndrome in Ambulatory People With Post-Polio Syndrome, Charcot-Marie-Tooth Disease, and Other Neuromuscular Diseases.
The metabolic syndrome is a growing health issue. This study evaluated the prevalence of metabolic syndrome and individual metabolic syndrome risk factors, in people with different neuromuscular diseases. We used baseline data of a randomized controlled trial on the efficacy of a physical activity program in neuromuscular diseases. Metabolic syndrome was defined as the presence of at least three out of five risk factors of the revised National Cholesterol Education Program Adult Treatment Panel III. The 84 participants (50 female, median age = 63 yrs, interquartile range = 48, 68 yrs) were diagnosed with post-polio syndrome ( n = 22), Charcot-Marie-Tooth disease ( n = 37) or other neuromuscular diseases ( n = 25). Metabolic syndrome was present in 18 participants (21%). The most common risk factors were hypertension (56%), central obesity (49%), and increased fasting blood glucose (33%). Logistic regression results showed that adjusted for age and muscle strength as confounders, participants with Charcot-Marie-Tooth disease (Exp(B) = 0.107, 95% confidence interval: 0.019-0.609) and other neuromuscular diseases (Exp(B) = 0.039, 95% confidence interval: 0.004-0.390) had significantly lower odds of metabolic syndrome compared with post-polio syndrome. The metabolic syndrome prevalence that we found is comparable to the general Dutch population. However, a focus on the prevention of metabolic syndrome in neuromuscular rehabilitation is warranted, as certain neuromuscular diseases subgroups may be at increased risk of developing metabolic syndrome.
Low-grade persistent poliovirus infection in long-term polio survivors diagnosed with post-polio syndrome: diagnostic and clinical implications.
Despite extensive research, the pathogenesis of Post-Polio Syndrome (PPS) remains unclear. We investigated 251 participants from Northern Italy: long-term polio survivors with PPS, long-term polio survivors with stable polio, family members of both groups, subjects with neurological disorders other than poliomyelitis, and healthy controls. This study investigated whether persistent viral activity or the existence of viral reservoirs contributes to causing PPS. Poliovirus (PV) genomes and proteins were detected in 87.2% of PPS cases versus 12.0% of stable polio cases and 3.5% of control family members, but not in pathologic and healthy controls. Among PPS patients, the highly concordant detection of PV strains in both peripheral blood leukocytes and cerebrospinal fluid (CSF) suggests the presence of an ongoing low-grade infection. Conversely, the very low detection rate in family members indicates the minimal transmissibility of these PV variants. Molecular analysis of the detected PV strains revealed mutations across most genome regions, likely leading to defects in virus replication. Furthermore, in cell cultures, PPS-derived PV strains induced the release of inflammatory mediators (IL6, IL8, MCP1) that may play a pathogenic role. These findings have several clinical implications. First, the presence of mutated PV forms in blood leukocytes and CSF could serve as a diagnostic marker for PPS. Second, the persistent virus infection suggests that antiviral treatments might help reduce PPS progression. Furthermore, advanced genome sequencing techniques hold potential for distinguishing vaccine-derived from wild-type PV strains, thereby refining our understanding of PPS and the full spectrum of polio disorders.
