To report a rare case of paraneoplastic vitelliform retinopathy, characterized by detachments of the retinal pigment epithelium and neurosensory retina manifesting as vitelliform lesions, successfully managed with a sustained-release intravitreal dexamethasone implant in a patient receiving systemic immunotherapy for metastatic cutaneous melanoma. A 79-year-old man presented with progressive bilateral visual loss and serous retinal detachment. Multimodal retinal imaging revealed bilateral vitelliform lesions, choroidal thickening, and subretinal fluid. Electroretinography demonstrated an electronegative waveform, supporting the diagnosis of a condition in the spectrum of MAR. Systemic evaluation revealed axillary melanoma metastasis confirmed by fine-needle aspiration. Despite treatment with systemic pembrolizumab and a trial of intravitreal anti-VEGF injections, visual symptoms and subretinal fluid persisted. Bilateral intravitreal dexamethasone implants led to marked anatomical and functional improvement within one month, with sustained benefits observed over a two-year follow-up. This case highlights the potential role of local corticosteroid therapy in paraneoplastic vitelliform lesions secondary to metastatic melanoma, particularly when systemic immunosuppression is contraindicated.

Multimodal Imaging Features of Bilateral Diffuse Uveal Melanocytic Proliferation: A Systematic Review of 82 Patients.

Bilateral diffuse uveal melanocytic proliferation (BDUMP) is a rare paraneoplastic syndrome characterized by proliferation of benign melanocytes throughout the uveal tract. Our purpose was to quantify the prevalence of multimodal imaging findings in BDUMP and identify features that most reliably characterize this rare paraneoplastic syndrome. We conducted a systematic review following PRISMA 2020 guidelines, searching PubMed/MEDLINE, Web of Science, and Embase through September 2025. We included case reports and series providing patient-level data on BDUMP with associated imaging. Prevalence rates were calculated using a documented denominator approach with 95% Wilson score confidence intervals. Sixty-seven studies encompassing 82 patients were included. Median age was 67 years (range 35-88); 54.9% were female. Gynecologic (25.9%), gastrointestinal (22.2%), and lung (17.3%) cancers were most common. Optical coherence tomography revealed retinal pigment epithelium irregularities in 91.8% and subretinal fluid in 90.2% of patients, exceeding the prevalence of classical orange-red patches on fundus photography (82.6%). Fundus autofluorescence showed alternating hypo/hyperautofluorescence in 76.6%. Choroidal thickening was present in 88.9% on ultrasonography. Bacillary layer detachment was identified in 16.4% of cases. OCT-based structural abnormalities are more consistently present than Gass's classical funduscopic signs. These quantitative data may assist earlier recognition of BDUMP and suggest that updated diagnostic criteria should incorporate multimodal imaging findings.

Paraneoplastic syndromes and the retina.

Paraneoplastic diseases of the retina are rare. Diagnosis can be challenging, and studies on management strategies are limited. The purpose of this review is to summarize our current understanding of the presentation, diagnosis, and management for cancer associated retinopathy, melanoma associated retinopathy, paraneoplastic vitelliform maculopathy, and bilateral diffuse uveal melanocytic proliferation. Multimodal imaging can aid in the diagnosis of paraneoplastic retinopathies since early fundus findings and symptoms can be nonspecific. No standardized treatment protocol exists with therapies having variable efficacy. Generally, systemic immunosuppression is used with possible concomitant local steroid treatment with improved visual outcomes in some cases. Paraneoplastic syndromes affect the retina as an autoimmune response to tumor antigens or to circulating tumor-derived factors. Diagnosis hinges on maintaining high degree of clinical suspicion, and multimodal imaging can also be helpful. Once diagnosis is made, prompt systemic work-up for malignancy is necessary. Limited literature exists to suggest superiority of a single treatment, but management involves treatment of the underlying malignancy and local or systemic immunosuppression and/or antibody removal.

Nonparaneoplastic Autoimmune Retinopathy: Scoping Review and Suggested Reporting Guidelines.

To investigate trends in the diagnostic approach to nonparaneoplastic autoimmune retinopathy (npAIR). We queried PubMed for clinical reports on npAIR published between January 2016 and September 2025. Articles were assessed to determine criteria used to establish diagnosis of npAIR using a standardized grading system. Articles were categorized as case reports (≤3 patients) or case series (>3 patients). 36 case reports and 41 case series met eligibility criteria (755 total cases). Author subspecialty included 34% uveitis, 20% inherited retinal disease (IRD), 16% general retina, 10% miscellaneous, and 19% unknown specialty. Over 80% of publications reported electroretinography and anti-retinal antibody testing for diagnosis of npAIR. Fundus autofluorescence (FAF) was performed in 67% of case reports and at least one patient in 51% of case series. Widefield FAF was used in 19% of case reports and in at least one patient in 20% of case series. Genetic testing was reported in 22% of case reports and in at least one patient in 27% of case series. Studies with an IRD specialist as first or last author most commonly used genetic testing (35%). Literature on npAIR is hampered by variability in classification schemes and incomplete reporting. Nonspecific electroretinography testing and antiretinal antibody testing are widely employed while widefield autofluorescence testing and genetic testing are not commonly used. Expanded access to these tools provides an opportunity to update diagnostic criteria of npAIR. Improved classification will permit us to better understand the natural history of disease.
