Melkersson-Rosenthal syndrome with facial swelling and palsy and associated diagnostic challenges: A case report.
Melkersson-Rosenthal syndrome (MRS) is a rare neuro-mucocutaneous disorder characterized by recurrent orofacial edema, peripheral facial palsy and a fissured tongue, although the complete triad is rarely observed. The present study reports the case of a 22-year-old male patient who initially presented with facial paralysis, later developing cheek edema, dermatosis and systemic symptoms; the histopathological findings consistent with granulomatous cheilitis. Laboratory analyses revealed a low C1q level and positive antinuclear antibody. Based on clinical and biopsy findings, MRS was diagnosed. The patient responded well to a 6-month regimen of deflazacort, epinastine and methotrexate. On the whole, the present case report illustrates an incomplete, yet clinically significant form of MRS. Diagnosis is primarily clinical, supported by histology and exclusion of similar conditions. Treatment focuses on symptom control with corticosteroids; immunomodulators or biologics may be used in refractory cases. Early recognition is essential, even in the absence of the full triad.
Is Melkersson-Rosenthal syndrome underdiagnosed in oculoplastic clinics?
PurposeMelkersson-Rosenthal Syndrome is a rare neuro-mucocutaneous disease characterised by recurrent orofacial swelling, facial palsy and lingua plicata. Patients often do not present with the complete triad of symptoms which can make this a diagnostic challenge. We present three cases of suspected MRS seen in our tertiary eye unit, highlighting the challenges in diagnosis and management of MRS.Case descriptionsCase 1 is a 54-year-old female with a background of neuro-sarcoidosis who presented with a several year history of bilateral upper lid swelling and recurrent facial nerve palsy. She experienced complete resolution of symptoms after a period of observation.Case 2 is a 49-year-old female who had a one-year history of recurrent facial nerve palsy associated with facial, neck and lip swelling. She was treated with oral steroids but did not experience complete remission.Case 3 is a 74-year-old female who presented with an 18-month history of bilateral peri-orbital swelling. Biopsy of skin and orbicularis showed granulomatous inflammation. She was treated with oral non-steroid anti-inflammatories as well as topical and oral steroids but without complete resolution.ConclusionMRS is a rare but important differential in patients with recurrent peri-orbital swelling. Given the recurrent nature of MRS and its associations with severe systemic diseases, early recognition and a multi-disciplinary approach is essential to diagnosis and management.
Cheilitis: a comprehensive review and a new clinical classification proposal. Part 1: Isolated cheilitis.
Cheilitis refers to an acute or chronic inflammation of the lips. It may occur in isolation, affecting only the vermilion border, or may be associated with oral or perioral involvement. Numerous dermatological and systemic diseases may involve the lips, making the clinical assessment of cheilitis particularly challenging. Various types of cheilitis distinguished by frequency, cause, and duration have been described in the literature. However, there are no univocal recommendations on classification. This article aims to introduce a classification of cheilitis based on etiology, according to the following categories: 1) isolated cheilitis, referring to inflammation confined exclusively to the lips, without systemic manifestations or oral cavity involvement. This group includes cheilitis simplex, contact cheilitis, exfoliative cheilitis, granulomatous cheilitis, glandular cheilitis, plasma cell cheilitis, actinic cheilitis, and infectious cheilitis; 2) cheilitis associated with dermatological conditions that exhibit little to no systemic involvement, such as atopic dermatitis, lichen planus, autoimmune bullous diseases, discoid lupus erythematosus, and psoriasis; 3) cheilitis associated with systemic diseases when lip inflammation occurs in the context of systemic conditions, including systemic lupus erythematosus, orofacial granulomatosis, sarcoidosis, Crohn's disease, Melkersson-Rosenthal syndrome and nutritional deficiencies; and 4) drug-induced cheilitis, when a clear causal relationship is established between drug exposure and lip inflammation, as observed in cases of cheilitis induced by oral retinoids, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, and DRESS (drug reaction with eosinophilia and systemic symptoms) syndrome. This proposed classification may offer clinicians a practical tool for the systematic evaluation and management of patients presenting with cheilitis.
Melkersson-Rosenthal syndrome and upadacitinib: A case report.
Bilateral facial palsy due to Melkersson-Rosenthal syndrome.
A 29-year-old woman developed bilateral facial nerve palsy for which no cause could be identified despite neuroimaging, lumbar puncture, viral serology and blood testing. Neuroinflammatory and dermatology input was sought. The patient later presented with a new manifestation of upper lip swelling, leading to a clinical diagnosis of Melkersson-Rosenthal syndrome. The diagnosis of this neuromucocutaneous disorder is challenging, with the classical triad of signs-facial nerve palsy, orofacial oedema, lingua plicata-often not presenting together. Suspecting the condition in oligosymptomatic presentations could reduce the delay in diagnosis, thereby facilitating earlier consideration for corticosteroid or immunosuppressive treatment and involvement of dermatology as appropriate.
