Periodontitis Induces B Cell-Macrophage Crosstalk to Exacerbate Glucose Dysregulation in Obesity.
A growing consensus indicates that periodontitis (PD) adversely affects glycemic control in type 2 diabetes, though its underlying mechanisms remain unclear. Here, we report that PD, induced by long-term oral ligatures, significantly aggravated hyperglycemia and hepatic gluconeogenesis in high-fat diet-induced obese mice, with its impact on insulin resistance being more pronounced in the liver than in adipose or muscle tissue. Immunologically, PD increased systemic B cell abundance, promoted hepatic B2 cell mobilization, facilitated the expansion of Kupffer cells and adipose tissue macrophages, and activated the NLRP3 inflammasome. Depleting B cells significantly alleviated PD-induced glucose dysregulation, whereas adoptive transfer of B cells from PD mice exacerbated glucose dysregulation and Kupffer cell expansion in Rag1-/- mice. Mechanistically, upregulation of the interleukin (IL)-18 receptor was observed in PD-exposed B cells. IL-18 directly enhanced B cell proliferation, and hepatic macrophages from PD mice secreted elevated levels of IL-18, further driving B cell expansion. Neutralizing IL-18 or depleting macrophages significantly mitigated PD-associated metabolic and B cell abnormalities. Therefore, PD exacerbates hyperglycemia by promoting the pathogenic expansion of B cells and their crosstalk with macrophages via the IL-18 signaling axis. Targeting the NLRP3/IL-18 axis holds promise for preventing glucose dysregulation and aberrant immune cell interactions primed by PD.
African immigrants with type 2 diabetes present with three physiologic subtypes: implications for screening, diagnosis and treatment.
As type 2 diabetes (T2D) prevalence increases in the USA and Africa, factors from both regions affect African immigrants. T2D in African immigrants was characterized by examining: (a) insulin deficiency and insulin resistance; (b) phenotypic presentation; (c) sociodemographic factors. In 633 African immigrants (male: 62%, age 39±11, (mean±SD), range 20-70 years), body mass index (BMI): 27.8±4.6, range 18.8-46.2 kg/m2), weight gain information was collected, BMI and waist circumference (WC) measured and OGTT performed. Insulin resistance was defined by the lowest quartile of the Matsuda Index (≤2.80); insulin secretion by the Insulin Secretion Index (ISI). Insulin deficiency was defined as less than the maximum ISI in participants with T2D without IR (0.430). WC thresholds defined central obesity (men: WC ≥94 cm; women ≥80 cm). Normal glucose tolerance, pre-diabetes and T2D occurred in 61%, 32% and 7%, respectively. Three subtypes of T2D were identified: insulin-deficient-T2D (ID-T2D) in 45%, insulin-resistant-T2D (IR-T2D) in 30%, insulin-deficient+insulin-resistant (ID+IR-T2D) in 25%. ID+IR-T2D had the highest glucose concentrations (all p<0.05), whereas insulins were highest in IR-T2D (all p<0.01). Phenotypic differences by T2D subtype were identified. In the ID-T2D group, 20% of participants had a healthy weight and central obesity occurred in 55%. In the IR-T2D and ID+IR-T2D groups, 100% had central obesity and a BMI in either the overweight or obese categories. Sociodemographic factors specifically, weight gain, sedentary lifestyle and percent married, increased across glucose tolerance category (p values <0.01) but did not differ by T2D subtype (p≥0.3). Spanning the BMI spectrum from normal to obese, African immigrants have three subtypes of T2D. Weight gain was greatest in immigrants who developed T2D but did not differ by subtype. As life in America promotes weight gain, sharing information about the consequences of weight gain with all Americans, both native and foreign-born, is key to T2D prevention and treatment.
A Combined Probiotic-Morus alba Strategy Enhances Glucose Homeostasis in an In Vitro Gut-Pancreas-Liver Axis Model: A Preliminary Mechanistic Screening Study.
Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disorder linked to gut microbiota dysbiosis and impaired inter-organ metabolic signalling. This study investigated the combined effects of the probiotic Lactiplantibacillus plantarum TJA7 and Mulberry Leaf extract (Morus alba) on cellular processes relevant to T2DM-related metabolic dysfunction. An advanced in vitro gut-pancreas-liver axis model, using Caco-2, EndoC-βH5, and HepG2 cells, was employed under hyperglycemic and oxidative stress conditions. The combined treatment consistently outperformed the individual components by improving intestinal barrier integrity, as indicated by increased transepithelial electrical resistance (TEER), and by enhancing butyrate translocation across the intestinal layer. Metabolites derived from the combination attenuated pancreatic β-cell dysfunction, reducing reactive oxygen species (ROS) levels and increased insulin secretion (1.7-fold compared with Mulberry Leaf extract alone). At the hepatic level, co-administration modulated key glucose metabolism pathways, including Insulin Receptor Substrate 1 (IRS1), Protein Kinase B (AKT), AMP-Activated Protein Kinase (AMPK), and Peroxisome Proliferator-Activated Receptor Gamma Coactivator 1 Alpha (PGC-1α), suggesting improved cellular glucose handling. Collectively, these findings support a positive dose-specific interaction under the tested conditions and provide a biologically plausible, hypothesis-generating framework for probiotic-phytochemical cooperation along the gut-pancreas-liver axis. Further in vivo and clinical studies are required to establish causality and translational relevance.
Immune cells regulate circulating adipocyte extracellular vesicle levels in response to metabolic shifts.
Extracellular vesicles (EVs) are now recognized as potent mediators of inter-organ signaling and are implicated in the pathogenesis of obesity and associated comorbidities. Despite a recent surge in functional information about EVs, we still lack a basic understanding of how endogenous EV levels are controlled to regulate inter-organ signaling. New flow cytometry technologies have allowed us to study the regulation of circulating endogenous EVs from metabolically relevant cell types such as adipocytes (adipocyte-derived EVs [adipoEVs]). We provide evidence for a paradigm of EV regulation in which tissue-resident immune cells, predominantly macrophages, clear EVs released by local tissue cells or those entering the tissue from circulation, an activity that determines circulating EV levels. In obesity, EV uptake by adipose tissue immune cells is reduced, leading to increased circulating adipoEVs and reduced adipoEV clearance rates. This work shows that tissue immune cells gate tissue EV entry into the circulation, making them key regulators of inter-organ EV signaling.
Delayed acute respiratory distress syndrome and sepsis-associated disseminated intravascular coagulation following aluminium phosphide poisoning.
Aluminium phosphide is a highly lethal pesticide that liberates phosphine gas, leading to mitochondrial dysfunction, distributive shock and cardiotoxicity, but guidance on late pulmonary complications is limited. We report a previously healthy adolescent who ingested ~1.5 g of aluminium phosphide in a suicide attempt and initially presented haemodynamically stable with metabolic acidosis and elevated lactate. By 72-96 hours, she developed progressive hypoxemic respiratory failure with bilateral infiltrates fulfilling Berlin criteria for moderate acute respiratory distress syndrome (ARDS) (PaO₂/FiO₂ nadir ≈115 on FiO₂ 1.0), preceded by fever and culture-proven lower respiratory and urinary tract infections, resulting in sepsis and disseminated intravascular coagulation (DIC). She required escalation from non-invasive to invasive mechanical ventilation, with lung-protective strategies, targeted antibiotics, component therapy for DIC and early magnesium and calcium supplementation. Despite markedly elevated N-terminal pro-B-type natriuretic peptide with preserved biventricular function on echocardiography and transient acute kidney injury, she made a full clinical recovery and was discharged with normal oxygenation and functional status. This case underscores that delayed moderate ARDS with superimposed sepsis and DIC can complicate initially stable aluminium phosphide poisoning and highlights that meticulous, protocol-based supportive care can still result in survival in this rarely reported scenario.
