Early Response in Pediatric Transverse Myelitis: A Case-Control Study from Vietnam.
Acute transverse myelitis (ATM) is a rare inflammatory spinal cord disorder causing significant motor, sensory, and autonomic dysfunction in children. Although advances in diagnosis and therapy have improved outcomes, data on early treatment response and associated factors remain limited, particularly in Southeast Asia. This study aimed to identify clinical, radiological, and immunological factors associated with early in-hospital response in children with ATM in Vietnam. This retrospective case-control study included 56 children under 18 years of age diagnosed with ATM between 2018 and 2023. Participants were divided into a case group (poor early response, n = 29) and a control group (good early response, n = 27) based on their functional status at discharge, as assessed by the Paine and Byers scale. Clinical, cerebrospinal fluid (CSF), and magnetic resonance imaging (MRI) findings, as well as myelin oligodendrocyte glycoprotein (MOG) and aquaporin-4 (AQP4) antibody results, were analyzed. Logistic regression was performed to identify factors associated with poor early response. Severe lower limb motor weakness (Medical Research Council [MRC] muscle strength scale ≤ 1) at admission was independently associated with poor early response (OR 14.7, 95% CI: 3.0-73.2, p < 0.05). Respiratory muscle weakness, the need for mechanical ventilation, and the absence of MOG antibodies were also linked to poor early outcomes. Radiological features, including longitudinally extensive transverse myelitis (LETM), cervical involvement, and gadolinium enhancement, were not significantly associated with early response. Severe motor and respiratory involvement at presentation were key indicators of poor early treatment response in pediatric ATM. Recognizing these factors promptly may guide risk stratification and early management. Longitudinal studies are needed to evaluate their predictive value for long-term recovery.
The Mystery of a Negative Workup: A Diagnostic Dilemma in Seronegative Immune-Mediated Transverse Myelitis.
Transverse myelitis (TM) is an acquired inflammatory disorder of the spinal cord with a heterogeneous aetiology that includes demyelinating, autoimmune, infectious, granulomatous, and paraneoplastic causes. Although advances in antibody testing, particularly aquaporin-4 (AQP4-IgG) and myelin oligodendrocyte glycoprotein immunoglobulin G (MOG-IgG), have refined the classification of many cases, a significant proportion remain seronegative, presenting diagnostic and therapeutic challenges. We report the case of a 44-year-old man who presented with progressive fatigue, significant weight loss, lower abdominal numbness, urinary urgency, and intermittent blurred vision. Magnetic resonance imaging of the thoracic spine demonstrated an intramedullary T2 hyperintense lesion at the T10-T11 level with patchy gadolinium enhancement, consistent with inflammatory myelitis. An extensive diagnostic evaluation excluded infectious, systemic autoimmune, granulomatous, endocrine, demyelinating, and paraneoplastic causes. The patient showed marked clinical improvement following high-dose intravenous methylprednisolone, with resolution of sensory and autonomic symptoms and stabilisation of weight, while mild fluctuating fatigue persisted. This case highlights the diagnostic grey zone of seronegative immune-mediated TM and underscores the importance of systematic exclusion, longitudinal assessment, and therapeutic responsiveness in establishing diagnosis and guiding management.
Successful management of Pott's disease in a young woman with 32 weeks of pregnancy: a rare case report.
Spinal tuberculosis (Pott's disease) during pregnancy is rare and poses diagnostic challenges due to overlapping symptoms with pregnancy-related changes, potentially leading to severe neurological deficits if untreated. We report an 18-year-old primigravida at 32 weeks gestation presenting with progressive lower limb weakness, urinary incontinence, and upper back pain. Neurological examination showed bilateral lower limb paralysis, hypertonia, exaggerated reflexes, and sensory loss up to T10. Contrast-enhanced MRI revealed tubercular spondylo-discitis at T3-T5 with partial T4 collapse, paravertebral phlegmon, and rim-enhancing epidural abscess. Initially managed as acute transverse myelitis with methylprednisolone, the diagnosis shifted to Pott's disease based on MRI. Antituberculosis therapy with initial methylprednisolone led to rapid improvement within 4 weeks, with full neurological recovery, independent mobility, and normal delivery of a healthy infant. This case underscores MRI's pivotal role in timely diagnosis and the efficacy of conservative medical management in late pregnancy to avert irreversible complications.
Immune-Mediated Transverse Myelitis in a Filipino Patient With Prior Hematopoietic Stem Cell Transplantation: A Case Report.
Transverse myelitis is an inflammatory disorder of the spinal cord that can result in significant neurologic morbidity. Diagnostic evaluation is particularly challenging in immunocompromised patients because of a broad differential diagnosis that includes infectious, neoplastic, vascular, and treatment-related etiologies. We report the case of a 64-year-old Filipino male with a history of parotid carcinoma treated with surgery and radiotherapy, acute myeloid leukemia treated with hematopoietic stem cell transplantation, and chronic immunosuppression who presented with acute-onset progressive bilateral lower extremity weakness and sensory loss. Magnetic resonance imaging of the thoracic spine demonstrated focal intramedullary signal abnormalities at the first and second thoracic spinal cord levels. Extensive infectious, neoplastic, and inflammatory investigations were unrevealing. The patient was treated for a case of immune-mediated transverse myelitis with high-dose intravenous corticosteroids, resulting in substantial neurologic improvement. He achieved full motor recovery with residual sensory deficits following rehabilitation. There was no recurrence of myelopathic symptoms during follow-up. The patient later died from pneumonia in the setting of relapse of acute myeloid leukemia, without evidence of recurrent transverse myelitis. This case underscores the importance of systematic diagnostic evaluation and early initiation of immunosuppressive therapy in suspected immune-mediated transverse myelitis, even in patients with complex oncologic and immunologic histories.
Acute transverse myelitis in an immunocompetent patient: syphilis and cytomegalovirus-a case report.
Acute transverse myelitis (ATM) is a pathological heterogenous inflammatory condition which causes acute or subacute spinal cord dysfunction. Diagnosis is challenging as the absence of magnetic resonance imaging (MRI) spinal cord lesions do no exclude this as a differential when patients present with well-defined dermatomal paraesthesia or myotome weakness. In approximately 40% of patient, there is a lack of MRI cord abnormalities. In addition, there are numerous of causes of ATM which contributes to diagnostic difficulty. The aim of this report is to highlight the challenges of diagnosing ATM, especially in the case of syphilis and cytomegalovirus. A 56-year-old man with a background of high-risk promiscuous behaviour, presents with a symmetrical hand and feet desquamating rash with associated lumbago. Subsequently, he develops sensory impairment in his truncal region. Cerebrospinal fluid (CSF) findings revealed lymphocytosis and elevated protein however, MRI head and whole spine were normal. Together, the patient was diagnosed with ATM. Serology was positive for syphilis [Treponema pallidum haemagglutination assay (TPHA) and rapid plasma reagin (RPR)] as well as cytomegalovirus (CMV) immunoglobulin M (IgM) and immunoglobulin G (IgG), while human immunodeficiency virus (HIV) was negative. On first inspection, the most obvious diagnosis was deemed to be syphilis given the history and presentation with concurrent development of neurosyphilis. However, concurrent CMV IgM positivity obscures the underlying aetiology. He was treated with ceftriaxone and corticosteroids for syphilis, and doxycycline for concomitant chlamydia. At 4-month follow-up, CSF protein had improved and lymphocytosis resolved, although CMV IgM remained positive. His sensory deficits gradually improved, with complete recovery after 6 months, without antiviral therapy. This case illustrates the diagnostic challenges of ATM when multiple infections coexist. While neurosyphilis was initially suspected, normal MRI findings and the persistence of CMV IgM suggest CMV myelitis as the more likely aetiology. Clinicians should consider CMV in the differential diagnosis of ATM even in immunocompetent patients, particularly when serology suggests recent infection but CSF polymerase chain reaction (PCR) and MRI findings are negative.
