Localized palmar mycosis fungoides: A case report.

Mycosis fungoides (MF) is the most common primary cutaneous T-cell lymphoma, yet palmar MF is extremely rare and easily mistaken for chronic dermatoses such as psoriasis. Delayed recognition can lead to inappropriate treatment and worse outcomes. This case demonstrates how careful histopathology and immunohistochemistry can provide critical diagnostic clarity in atypical presentations. A 63-year-old woman presented with an 8-year history of erythema, dryness, scaling, and fissuring on both palms. The lesions had been misdiagnosed as psoriasis and showed no response to long-term topical calcipotriol. Histopathology of a palm biopsy revealed epidermotropism of atypical lymphocytes with cerebriform nuclei and Pautrier microabscesses. Immunohistochemistry demonstrated positivity for CD2, CD3, CD4, and CD45RO with immunophenotypic variation, confirming palmar MF. The patient was staged as early MF (IA-IB). Systemic therapy with intramuscular interferon α-2b, oral methylprednisolone, and acitretin was initiated due to chronicity, bilateral palm involvement, and resistance to topical therapy. Significant improvement was noted after 2 months. At the 1-year follow-up, her palms showed complete clearance of lesions with sustained remission. This case highlights the diagnostic pitfalls of palmar MF and underscores the necessity of early biopsy in patients with persistent, treatment-resistant palm dermatoses. Raising clinical awareness of this rare entity may prevent misdiagnosis, facilitate timely therapy, and ultimately improve patient prognosis.

Radiotherapy dose-response of mycosis fungoides with large cell transformation.

Mycosis fungoides (MF) is the most prevalent subtype of primary cutaneous T-cell lymphoma. Large cell transformation of MF (LCTMF) is rare and confers a poor prognosis. Radiotherapy (RT) is an effective local treatment for LCTMF; however, dose-response data are limited. Eligibility for this retrospective study required biopsy-proven LCTMF with clinico-pathological correlation, diagnosed 1/1/1990-1/10/2021, and managed at Peter MacCallum Cancer Centre. 83 patients were eligible. Median age was 68 years, 63 (76 %) patients had cutaneous-only LCTMF at time of diagnosis. Median follow-up was 8.0 (95 % CI: 6-11) years. Details of 155 irradiated LCTMF lesions (from 49 patients) were available: 150 cutaneous, 5 extra-cutaneous. For cutaneous LCTMF, median equivalent dose in 2.0 Gy per fraction (EQD2, α/β of 10) was 30.6 (range, 4.7-46.3) Gy. Dose-response data were available for 141 cutaneous LCTMF lesions. Overall response rate (ORR) was 94 % (60 % complete response (CR), 35 % partial response). For cutaneous lesions treated with >12.0 Gy, ORR was 100 %. Increasing doses of RT were associated with greater CR rates: doses >36.0 Gy achieved 100 % CR rate. 22 (27 %) patients had unifocal cutaneous-only LCTMF and were treated with local RT-alone, median EQD2 36.0 Gy (range, 17.3-46.3 Gy). CR rate was 100 %. 9 (41 %) patients remained relapse-free (median follow-up, 3.2 years). Only 1 patient experienced infield-only recurrence at first relapse. LCTMF is radio-responsive, with EQD2 >12.0 Gy associated with 100 % ORR. Dose-response was observed, with EQD2 >36.0 Gy achieving 100 % CR rate. For unifocal cutaneous LCTMF, RT-alone achieved excellent infield control with possible curative potential in a proportion of patients.

Histopathologic Spectrum of Primary Cutaneous T-Cell Lymphomas: Insights From a Vietnamese Case Series.

Primary cutaneous T-cell lymphomas (PCTCLs) are a rare and heterogeneous group of extranodal non-Hodgkin lymphomas originating from skin-homing T cells. Most current knowledge about PCTCLs is derived from Western populations, with limited data available from Southeast Asia, including Vietnam. To characterize the clinical, histopathologic, and immunohistochemical features of PCTCLs in a cohort of Vietnamese patients. A retrospective case series was conducted involving 39 PCTCL cases. Demographic and clinical data were collected from medical records and archived clinical images. Histopathologic and immunohistochemical evaluations were independently performed by 3 pathologists, following the fifth edition of the WHO Classification of Haematolymphoid Tumours. Subcutaneous panniculitis-like T-cell lymphoma was most frequent (36%), followed by mycosis fungoides (MF, 28%) and primary cutaneous anaplastic large cell lymphoma (pcALCL, 15%). Subcutaneous panniculitis-like T-cell lymphoma predominantly affected young women, whereas MF and pcALCL occurred in older adults (median age, 64 years). Beyond canonical clinicopathologic features, notable findings included high rates of Pautrier microabscesses and eosinophilic infiltration in MF (each 73%), 1 ulcerated MF case, and 1 pcALCL with a CD4-/CD8- phenotype. Although clinicopathologic features largely aligned with established diagnostic criteria, several differences from international series were observed. These disparities likely reflect delayed diagnosis, referral bias toward more severe disease, and broader systemic and cultural factors in Vietnam, and constrained availability of advanced diagnostic assays, underscoring the need to strengthen clinicopathologic integration and expand access to specialized care.

Bacterial skin colonization and systemic antibiotic treatment in patients with cutaneous T-cell lymphoma.

Bacterial colonization, particularly by Staphylococcus aureus (SA), is prevalent on the skin of patients with primary cutaneous T-cell lymphoma (CTCL). This study aimed to investigate risk factors for cutaneous bacterial and Staphylococcus aureus colonization and to evaluate their impact, together with systemic antibiotic use, on the prognosis of patients with CTCL. This retrospective study included 113 CTCL patients who underwent skin swab testing at Peking University First Hospital from 2010 to 2024. Eighty-five patients (75.2%) tested positive for bacterial skin colonization (SA, 60.2%). Ulcerated lesions significantly increased the likelihood of a positive result in bacterial/SA skin cultures (multivariate analysis). Additionally, advanced stages, the presence of tumors, erythroderma, lymphopenia, and eosinophilia were associated with an increased risk of positive skin cultures (Univariate analysis). Cox regression analysis indicated that skin bacterial/SA colonization and antibiotic intervention, were not correlated with overall survival (p > 0.05). This retrospective study presents data on the prevalence of bacterial skin colonization in Asian CTCL patients. Ulcerated lesions emerged as the most significant risk factor associated with cutaneous bacterial/SA colonization. In Asian patients with CTCL, neither bacterial or Staphylococcus aureus skin colonization was associated with a poorer prognosis, nor did short-term systemic antibiotic therapy improve outcomes.
