Tirzepatide counteracts brown adipose tissue whitening, inflammation, and mitochondrial dysfunction in estrogen-deficient obese diabetic mice.
This study investigated the therapeutic potential of tirzepatide, a dual GIP/GLP-1 receptor agonist, in a mouse model combining obesity, type 2 diabetes, and estrogen deficiency. Four groups were initially established and maintained for 12 weeks under ovariectomy and/or a high-fat-high-sucrose diet: control sham (C), control ovariectomy (CO), obese-diabetic (Od), and obese-diabetic ovariectomy (OdO). Mice were then treated with tirzepatide (10 nmol/kg/day) for 4 weeks, generating CT, COT, OdT, and OdOT groups. Od and OdO mice exhibited a 1.3-fold increase in body weight (BW) compared with controls. Tirzepatide normalized BW and reduced relative BAT mass by 25 %. It also corrected the 3.6-fold elevation in leptin and the 1.8-fold increase in insulin observed in Od mice, and reduced Monocyte Chemotactic Protein-2 and resistin levels by approximately 60 % and 42 %, respectively. Histological and molecular analyses showed that tirzepatide reversed BAT whitening, restored multilocular adipocyte morphology, and increased the expression of key thermogenic markers, including uncoupling protein-1 and the β3-adrenergic receptor. Three-way ANOVA revealed tirzepatide as the most potent regulator of the BAT gene landscape. Treatment reversed the suppression of thermogenic and mitochondrial fusion genes in Od and OdO mice and normalized markers of endoplasmic reticulum stress and autophagy dysfunction. Principal component analysis further demonstrated this global rescue, with OdT and OdOT transcriptomic profiles clustering closely with those of healthy controls. In conclusion, tirzepatide acts as a multifaceted therapy that improves obesity, systemic inflammation, and impaired BAT thermogenic function in a model of postmenopausal metabolic dysfunction.
Obesity-induced leptin resistance is directly involved in skin fragility by decreasing type I collagen synthesis.
The relationship between obesity-induced leptin resistance and skin function remains unclear. In this study, we examined the effects of leptin signaling on collagen production in mouse skin. Mice were divided into three groups: control diet, high-fat diet, and switching diet (from the high-fat diet to the control diet). We measured the expression of leptin signaling-related genes and type I tropocollagen levels in the skin. In an additional experiment, leptin was administered to leptin-deficient ob/ob mice to examine the direct effect of leptin signaling on type I collagen synthesis in the skin. Type I tropocollagen levels and Ob-Rb gene expression were decreased in the high-fat diet group. These abnormalities were reversed by the switching diet. We also observed that type I tropocollagen levels were decreased in ob/ob mice, but this abnormality was reversed by leptin administration. It is likely that leptin will help to improve obesity-induced skin fragility.
Inversed impaired osteogenic activity in children with severe obesity due to MC4R deficiency compared to LEP and LEPR deficiency.
Chronic obesity is associated with impaired bone health. However, few investigations have been conducted to assess bone physiology in early-onset obesity. In this study, we measured specific bone turnover and metabolic biomarkers in children with severe obesity with biallelic loss-of-function variants of the leptin (LEP), leptin receptor (LEPR), or melanocortin 4 receptor (MC4R) genes. Thirty-nine children aged 0.3-8.8 years with a BMI SDS ≥ 3, previously identified with pathogenic variants in LEP, LEPR, or MC4R, were recruited for the current study. Additionally, 13 age-matched children with severe obesity who tested negative for variants in known obesity-related genes were included, and another 13 unrelated age-matched children with normal body weight served as the control group. Serum osteocalcin, osteopontin, osteoprotegerin, and sclerostin levels were assessed using multi-analyte profiling. Serum leptin, insulin, and cortisol levels were determined using ELISA. Serum levels of osteocalcin and osteopontin, specific markers of bone formation, were significantly lower in children with LEP and LEPR biallelic variants than in the control group. In contrast, the values of these two biomarkers in children with MC4R deficiency were significantly higher than those in the other groups. No differences were observed in the bone resorption markers osteoprotegerin and sclerostin. Hyperleptinemia was more pronounced in children with LEPR deficiency. Serum insulin concentrations were elevated in individuals with MC4R deficiency, whereas serum cortisol levels were significantly higher in children with LEP deficiency than in all other groups. Our data demonstrate that osteogenic activity (but not resorption activity) is differentially affected in children with complete genetic disruption of the leptin-signaling pathway. Children with MC4R deficiency showed higher osteogenic markers, but children with LEP and LEPR deficiencies showed the opposite. Our results support the usefulness of bone turnover biomarkers for the assessment and management of bone health in different types of obesity.
Efficacy of GLP-1 analog peptides, semaglutide, tirzepatide, and retatrutide on MC4R deficient obesity and their comparison.
Melanocortin 4 receptor (MC4R) is a G-protein-coupled receptor expressed in the hypothalamus, playing a key role in regulating feeding behavior and energy homeostasis. MC4R is integral to the POMC-MC4R and leptin-MC4R pathways, which control food intake and body weight. Mutations in the POMC gene lead to severe early-onset obesity and increased food consumption. Recently, glucagon-like peptide-1 (GLP-1) analogs, including semaglutide, tirzepatide, and retatrutide, have been explored as potential anti-obesity therapies. This study aimed to assess and compare the efficacy of these GLP-1 analogs in MC4R knockout (KO) mice, which are deficient in the POMC-MC4R pathway. GLP-1 analogs were administered for 21 days to MC4R KO mice and compared their efficacy. The percentage of body weight reduction was 19.7 ± 4.1% for semaglutide, 31.6 ± 7.6% for tirzepatide, and 24.1 ± 5.8% for retatrutide. Body composition analysis, including fat and lean mass, was performed using the Echo-MRI system, revealing significant suppression of both fat and lean mass by all three GLP-1 analogs. Furthermore, GLP-1 analogs improved plasma insulin levels, HOMA-IR, cholesterol levels, and markers of liver damage (AST and ALT), as well as reduced liver hypertrophy. While GLP-1 analogs suppressed genes related to fatty acid synthesis, they had no significant effect on inflammation-related gene expressions. Additionally, GLP-1 analogs reduced energy expenditure, with only tirzepatide showing a significant decrease in the respiratory quotient (RQ) in MC4R KO mice. Our findings demonstrate that all three GLP-1 analogs, semaglutide, tirzepatide, and retatrutide, exhibit significant anti-obesity effects in MC4R KO mice. These results suggest that GLP-1 analogs may provide an effective treatment option for patients with MC4R-POMC pathway deficiencies. Moreover, the efficacy of these drugs in MC4R KO mice aligns with clinical studies, indicating that MC4R KO mice serve as a reliable animal model for obesity research.
Additive effect of leptin and palm-LEAP2(1-14) ameliorates obesity-induced metabolic stress in ob/ob mice.
Liver-enriched antimicrobial peptide 2 (LEAP2) can act as a natural antagonist or inverse agonist of the growth hormone secretagogue receptor, also known as the ghrelin receptor. In our previous studies, palm-LEAP2(1-14), a truncated palmitoylated analogue of LEAP2, proved highly stable and exerted an acute anorexigenic effect. Ghrelin, the endogenous agonist of the ghrelin receptor, and the anorexigenic hormone leptin have opposing effects on appetite. We hypothesized that leptin supplementation combined with long-term antagonism of the ghrelin receptor using palm-LEAP2(1-14) might have a beneficial and potentially additive effect on obesity-related metabolic stress in leptin-deficient ob/ob mice, influencing diabetes, liver steatosis, impaired locomotor activity, and hypothermia. Here, we demonstrated a clear additive effect of combined leptin and palm-LEAP2(1-14) treatment, which increased locomotor activity, reduced plasma cholesterol, liver steatosis, and mRNA expression of the metabolic stress marker Fgf21, and upregulated hypothalamic Pomc gene expression - outcomes not achieved by either treatment alone. Additionally, leptin, palm-LEAP2(1-14), and both treatments combined each reduced cumulative food intake and circulating FGF21 levels. Only leptin reduced body weight, elevated rectal temperature, increased Ucp1 and Pgc1 mRNA expression in brown adipose tissue, and attenuated hyperinsulinemia, indicating that the weight-reducing and antidiabetic effects were attributable to leptin - not palm-LEAP2(1-14). Liver metabolic stress was further alleviated by decreases in plasma FGF21 and hepatic expression of the lipogenic enzymes. Our study demonstrated the beneficial effects of palm-LEAP2(1-14) on obesity-induced metabolic stress, which were further attenuated when palm-LEAP2(1-14) was coadministered with leptin in ob/ob mice.
