Randomized Study of Intravitreal Autologous CD34+ Stem Cells in Central Retinal Vein Occlusion (Treatment of Retinal vein occlusion Using STem cells [TRUST] Report 1): Safety and Feasibility.
To assess the safety and feasibility of intravitreal injection of autologous CD34+ bone marrow stem cells (BMSCs) in eyes with vision loss from central retinal vein occlusion (CRVO). Phase I/II single-center, prospective, randomized, sham-controlled, double-masked study. Participants with CRVO of 6 to 42 months duration, best-corrected visual acuity (VA) of 20/40 to 20/400, and no concurrent retinopathy or optic neuropathy contributing to vision loss in the study eye. The exclusion criteria include any concurrent systemic condition that would alter bone marrow components. Participants were randomized to immediate cell injection followed by sham injection at month 6 or immediate sham injection followed by cell injection at month 6. Cell injection consisted of a bone marrow aspiration and intravitreal injection of autologous CD34+ BMSCs. CD34+ BMSCs were isolated from the mononuclear cell fraction of bone marrow using Miltenyi CliniMACS system under current good manufacturing practices. Isolated cells were released for intravitreal injection if they passed the release criteria for quantity, sterility, and viability accepted by the US Food and Drug Administration. Sham injection consisted of a sham bone marrow aspiration and intravitreal injection without entering bone or eye. Eye examination, microperimetry, fundus photography, fluorescein angiography, electroretinography, OCT, and OCT angiography were performed at baseline and during study follow-up of 12 months. Adverse events (AEs) associated with study treatment, number of CD34+ BMSCs injected intravitreally. Sixteen participants (16 eyes) were randomized to 1 of 2 study groups. All received intravitreal injection of autologous CD34+ BMSCs (mean 4.3 million cells) and completed the study follow-up. Rubeosis with vitreous hemorrhage occurred in 1 study eye, <1 month after sham injection and 7 months after cell injection, attributed to normal progression of CRVO. There were no other serious ocular AEs. The most common AE related to the study cell injection was new floaters (15/16, 93%). Other ocular AEs were similarly noted after sham injection. No eye had persistent VA loss of ≥15 letters after cell injection. Intravitreal injection of autologous CD34+ BMSCs appears well-tolerated and feasible in eyes with vision loss from CRVO. Intravitreal Dexamethasone Implant in Retinal Vein Occlusion: A Pilot Study Exploring Baseline Ocular and Circulating Biomarkers.
This pilot study assessed the effectiveness of the intravitreal dexamethasone implant (Ozurdex) in retinal vein occlusion (RVO) patients and explored potential pre-treatment biomarkers to improve management and prognosis. Eighteen patients with branch RVO (BRVO) and twenty-four with central RVO (CRVO) receiving two intravitreal injections of Ozurdex (at baseline and between 4 and 6 months) were included. Best-corrected visual acuity (BCVA) and central retinal thickness (CRT) were recorded at baseline and after 3, 6, and 12 months. Retinal morphology was assessed using optical coherence tomography (OCT), and serum biomarkers were analyzed by ELISAs. No significant BCVA improvement was observed in RVO patients, while CRT significantly decreased from 3 to 12 months. Patients without defects of the retinal inner layers, ellipsoid zone, and external limiting membrane showed significantly higher BCVA at 6 and 12 months. Both BRVO and CRVO groups demonstrated significant BCVA improvement and CRT reduction at 6 and 12 months, with better outcomes in BRVO patients. These patients exhibited lower baseline serum levels of xanthine oxidase (XO) and thrombospondin-1 (TSP-1), which inversely correlated with BCVA at 12 months. Ozurdex was effective in real-life RVO treatment, particularly in BRVO. Serum XO and TSP-1 may serve as prognostic biomarkers for RVO.

Simultaneous Branch Retinal Artery and Central Retinal Vein Occlusions following Blood Transfusion in a Young Man: A Case Report.
Simultaneous branch retinal artery occlusion (BRAO) and central retinal vein occlusion (CRVO) is an exceptionally rare cause of acute vision loss, particularly in young individuals without systemic vascular risk factors. Blood transfusion has been implicated in rare thromboembolic and hyperviscosity-related complications but is infrequently associated with retinal vascular occlusions. A 20-year-old man presented with sudden painless unilateral vision loss shortly after receiving a packed red blood cell transfusion for severe anemia. Ophthalmic examination revealed optic disc edema, venous engorgement, scattered retinal hemorrhages, and superior hemi-macular whitening in the affected eye. Structural optical coherence tomography demonstrated inner retinal hyperreflectivity and subsequent thinning, consistent with ischemic retinal injury. Fluorescein angiography was contraindicated due to an acute hypersensitivity reaction. Extensive systemic, hematologic, inflammatory, and thrombophilia workup was unremarkable. Despite conservative and supportive management, including hyperbaric oxygen therapy and anticoagulation, visual recovery remained limited at follow-up. This case highlights a rare presentation of concurrent BRAO and CRVO occurring shortly after blood transfusion in a young patient without identifiable systemic risk factors. Although causality cannot be definitively established, the close temporal relationship and exclusion of alternative etiologies suggest transfusion-related rheologic changes as a plausible contributing factor. Clinicians should remain vigilant for acute retinal vascular events following transfusion, even in young patients, and prompt ophthalmic evaluation is essential. This report describes an unusual eye problem that developed in a young man shortly after receiving a blood transfusion. The retina is the thin, light-sensitive tissue at the back of the eye that allows us to see. It receives blood through tiny arteries and drains blood through tiny veins. In this case, both an artery and a vein became blocked at the same time in the same eye, which is extremely rare. An artery blockage reduces the supply of oxygen to the retina, while a vein blockage causes pressure to build up and blood to collect inside the retina. When both occur together, vision can suddenly become blurred or dark because the retina is affected in two different ways at once. The patient experienced serious vision loss shortly after his transfusion. When examined, he was found to have signs of both types of blockage. Imaging of the retina confirmed these findings. Although blood transfusion is usually safe, temporary changes in blood thickness can happen, especially in young people. These changes may increase the chance of blood flow becoming sluggish in the eye, which can contribute to blockage in rare situations. This summary explains what happened in this unusual case, why it is medically significant, and how it may help doctors recognize similar problems early. Understanding this event may support quicker diagnosis and appropriate management when sudden vision loss occurs after a transfusion.
[A case of antiphospholipid syndrome complicated with hyperhomocysteinemia presenting initially with central retinal vein occlusion in a young male].
A 28-year-old male presented to the ophthalmology department with decreased visual acuity in the left eye for 4 months. Four months prior, he was diagnosed with central retinal vein occlusion (CRVO) in the left eye at another hospital and received three intravitreal injections of anti-vascular endothelial growth factor (anti-VEGF) agents. Laboratory examinations revealed elevated levels of homocysteine, lupus anticoagulant (LA) 1 screening test result, LA1/LA2 ratio, and anti-β2 glycoprotein 1 antibody. The patient was treated with oral folic acid tablets, systemic anticoagulants, and additional intravitreal anti-VEGF therapy.
