Can topical coconut oil reduce late-onset sepsis in extremely preterm infants? A pragmatic cluster-randomised controlled trial protocol.
Infants born before 28 weeks' gestation account for approximately 75% of neonatal morbidity and mortality. Late-onset sepsis (LOS) affects around 25% of these infants and is associated with an increased risk of adverse long-term outcomes. The topical application of coconut oil has been used for centuries in newborn care. Coconut oil is rich in saturated fatty acids, several of which have demonstrated antimicrobial properties. It is considered safe for extremely preterm infants, improves skin condition and may reduce the incidence of LOS. This is a pragmatic, cluster-randomised, two-arm, parallel-group, multicentre, phase III clinical trial evaluating the effect of topical coconut oil versus routine skin care on the incidence of LOS in extremely preterm infants. Participating neonatal units will be cluster-randomised, and all infants born at <28 weeks gestational age will receive either routine skin care or routine care with the addition of topical coconut oil. A minimum of 616 infants will be recruited into each treatment arm. Intention-to-treat and per-protocol analyses will be performed. Following ethical approval, patients will be recruited at participating sites under a waiver of consent with opt-out framework. Drying Very Preterm Infants Before Plastic Wrapping at Birth: A Randomized Clinical Trial.
Despite continuous improvements in neonatal resuscitation and stabilization in the last decades, thermal management immediately after birth remains an unresolved issue in preterm infants. To compare 2 strategies of thermal management (plastic wrapping with or without drying) for preventing heat loss at birth in very preterm infants. This multicenter, unblinded, randomized clinical trial was conducted among very preterm infants (birth weight <1500 g and/or gestational age ≤30 weeks 6 days) at 21 tertiary care hospitals in Italy from February 21, 2023, to July 18, 2024. Eligible neonates were randomly allocated to either drying before plastic wrapping in the delivery room (intervention arm) or plastic wrapping without drying (control arm). The primary outcome was the proportion of participants with normothermia (36.5-37.5 °C) at admission to the neonatal intensive care unit (NICU). The secondary outcomes included hypothermia (<36.5 °C), moderate to severe hypothermia (<36.0 °C), and hyperthermia (>37.5 °C) at NICU admission, temperature at 1 hour after NICU admission, intraventricular hemorrhage, respiratory distress syndrome, late-onset sepsis, bronchopulmonary dysplasia, and mortality before hospital discharge. Overall, 354 very preterm infants were randomized (180 [50.8%] female; mean [SD] gestational age, 28.6 [2.5] weeks); all received the allocated intervention and were included in the analysis. Normothermia at NICU admission was achieved in 81 of 177 dried infants (45.8%) and 82 of 177 undried infants (46.3%; risk ratio, 0.99; 95% CI, 0.79-1.24). The mean (SD) neonatal temperature at NICU admission was 36.4 °C (0.8 °C) in dried neonates and 36.5 °C (0.7 °C) in undried neonates (mean difference, -0.1 °C; 95% CI, -0.2 °C to 0.1 °C). In-hospital mortality included 26 of 177 dried neonates (14.7%) and 10 of 177 undried neonates (5.6%) (unadjusted risk ratio, 2.60; 95% CI, 1.29-5.23). The other secondary outcome measures were not different between the 2 arms. In this multicenter randomized clinical trial, drying before plastic wrapping provided no benefit to very preterm infants in maintaining normothermia at NICU admission. Most deaths could be expected due to the compromised profile of the neonates, with no pathophysiological explanation related to the trial interventions. Approximately half of the infants were outside the normal thermal range at NICU admission; hence, thermal management remains a challenge requiring further investigations. Hospital-Acquired Citrobacter Meningitis Complicated by Pneumocephalus in a Neonate.
Citrobacter koseri, a facultative gram-negative bacillus, colonizes the intestine and environmental reservoirs. In neonates, especially preterm infants, it can cause severe central nervous system infections (eg, meningitis, encephalitis, abscesses), often with fatal outcomes. Pneumocephalus, an accumulation of intracranial gas, is a rare but deadly complication. We report a preterm female infant admitted to the neonatal intensive care unit (NICU) for respiratory distress who developed high fever and sepsis; blood culture results were positive for C. koseri. Rapid neurological deterioration with seizures occurred, and a cranial computed tomography scan showed extensive pneumocephalus. Despite intensive care, the infant died at 17 days. Autopsy and histology revealed widespread purulent meningitis and meningoencephalitis, pneumocephalus, cerebral hemorrhages, acute edema, and ventricular dilation. An environmental investigation traced the origin of the infection to the bathroom sinks, confirming the nosocomial nature of the pathogen. This case highlights the high virulence of C. koseri in neonates, the catastrophic potential of pneumocephalus, and the crucial importance of strict infection control in NICUs.
Immune profiles and growth outcomes in very and extremely preterm infants with bronchopulmonary dysplasia: A prospective cohort study.
Preterm birth is associated with immune dysregulation, and bronchopulmonary dysplasia (BPD) is the most prevalent complication affecting preterm infants. However, studies addressing the correlation between immune profiles and BPD severity in preterm infants remain limited. A total of 78 preterm infants born at less than 32 weeks of gestation and 46 full-term controls were enrolled. Basic characteristics, including birth data, medical history, and growth, and immune profiles including CD3+ T cells and subsets of CD4+ and CD8+ T cells, along with CD19+ B cells, were analyzed and compared across various gestational age (GA) and BPD grading groups. Extremely preterm infants and those with severe BPD had lower birth weights, lower body weight percentiles, and higher rates of sepsis at 6 months corrected age compared with full-term infants and those with no or mild BPD (P < 0.01). Among preterm infants, extremely preterm infants exhibited higher CD8+ T cell percentages with lower CD4/CD8 ratios than very preterm infants (P < 0.05). Infants with severe BPD had the highest percentage of CD4/CD8 ratios below the 25th percentile among BPD grading groups (P < 0.05). Furthermore, males had lower CD4+ T cell percentages than females (P < 0.05), but no differences in immune profiles were found between those with or without sepsis. Extremely preterm infants with severe BPD showed underdeveloped growth, higher sepsis rates, and altered high CD8+ T cells with lower CD4/CD8 ratios, which is potentially related to premature pulmonary processes in early infancy.
S100-alarmins, antenatal corticosteroids and the risk of late-onset sepsis in preterm infants: A prospective cohort study.
Antenatal corticosteroids (aCS) are an important measure improving the outcome of preterm infants. Their influence on late-onset sepsis (LOS) risk remains inconclusive. The alarmin S100A8/A9 protects from LOS by regulating innate immune responses. We examined whether aCS impact on postnatal S100A8/A9 serum-levels and consequently on LOS risk. In a prospective birth-cohort study of 162 preterm infants born before 32 gestational weeks, we determined postnatal S100A8/A9 serum-levels in relation to the timing of aCS and LOS incidence. aCS administration within 7 days before birth decreased LOS incidence in infants born via primary C-section compared to infants not exposed to aCS (5/69 (7.2%) vs. 4/27 (14.8%)). This effect was linked to increased S100A8/A9 levels, with nocturnal aCS administration being most effective. Opposite, S100A8/A9 levels were lower and the LOS incidence higher compared to unexposed infants (7/23 (30.4%) vs. 4/27 (14.8%)) when aCS were administered more than 14 days before delivery. Our data suggest that aCS administration affects the risk of LOS in preterm infants in dependence of the timing of administration by influencing the infant's S100A8/A9 levels. This underlines the importance of optimal timing of aCS facing imminent preterm birth.
