Acute fibrinous and organizing pneumonia after bone marrow transplantation, an underrecognized, severe condition.
Allogeneic bone marrow transplantation can lead to various pulmonary complications, including acute fibrinous organizing pneumonia. This condition is rare and presents with aggressive clinical features, distinct from other forms of organizing pneumonia, such as cryptogenic organizing pneumonia. A literature review using the PubMed, Embase, Lilacs, and Cochrane databases was conducted to analyze cases of acute fibrinous organizing pneumonia following transplantation focusing on clinical features, therapeutic approaches, and outcomes. The case of a 62-year-old female who developed acute fibrinous organizing pneumonia after transplantation for acute myeloid leukemia is presented. Despite an initial absence of infectious agents, parainfluenza virus was later identified in a bronchoalveolar lavage. The patient progressed to severe hypoxemic respiratory failure and was unresponsive to corticosteroids and rituximab, ultimately dying seven months post-transplant. This is a rare and severe complication following allogeneic bone marrow transplantation. Early diagnosis, histopathological confirmation, and prompt initiation of corticosteroid therapy are critical for improving outcomes. Patients diagnosed before Day +100 generally have a better response to treatment and more favorable clinical outcomes. The need for a more effective and targeted treatment strategy remains an unmet challenge in managing this condition.
Pulmonary Complications Following Hematopoietic Cell Transplantation in Patients with Myelofibrosis: A Single-Center Retrospective Cohort Analysis.
Myelofibrosis (MF) is a clonal hematopoietic stem cell disorder that frequently results in extramedullary hematopoiesis. Hematopoietic cell transplantation (HCT) is the only cure. While a 2-fold increase in pneumonia and respiratory-related mortality was recently reported in MF patients compared to the general population, the incidence and etiology of HCT-related pulmonary complications have not been well studied. We performed a retrospective chart review of consecutive MF patients who underwent HCT between 2002 and 2022. Patients with evidence of pulmonary impairment on pulmonary function tests, defined as a ≥10-point decline in forced expiratory volume in 1 second (FEV1) compared to pre-HCT and/or a decline in the FEV1/forced vital capacity (FVC) ratio to <0.7, received further chart review to determine the etiology of this decline. Of 228 patients evaluated, 126 (55%) met either the FEV1 or FEV1/FVC criteria for pulmonary impairment. After further chart review, 35 patients (15.4%) were diagnosed with broncholitis obliterans (BOS) by a pulmonologist or graft-versus-host disease specialist. Other common causes of impairment included respiratory infections (n =10), cryptogenic organizing pneumonia (n = 8), and deconditioning (n = 8). MF patients have a >50% likelihood of developing pulmonary impairment following HCT and a higher-than-expected incidence of BOS. This study supports increased pulmonary monitoring post-HCT in patients with MF.
When Positron Emission Tomography (PET) Is Misleading: Ion™ Robotic Bronchoscopy Identifies Organizing Pneumonia and Unmasks a Rare Pulmonary Granular Cell Tumor.
Pulmonary nodules with increased metabolic activity on positron emission tomography (PET) are frequently presumed malignant; however, inflammatory and rare benign neoplastic processes may produce false-positive findings. A 61-year-old African American male with a history of calcified and non-calcified pulmonary nodules and severe emphysema was referred to our pulmonary nodule clinic from the emergency department after a newly discovered lung nodule was identified on computed tomography (CT) of the chest. Subsequent evaluation demonstrated metabolic activity on PET, raising concern for malignancy; however, tissue diagnosis revealed organizing pneumonia. This case highlights diagnostic pitfalls associated with commonly used imaging and biomarker modalities in the evaluation of pulmonary nodules and emphasizes the importance of clinical awareness of a rare tumor, granular cell tumor (GCT), among physicians who may be unfamiliar with or have never encountered this condition.
Organizing Pneumonia Phenotype.
Organizing pneumonia (OP) is a nonspecific lung parenchymal response to any form of injury, with its idiopathic variant coined as cryptogenic organizing pneumonia (COP). The hallmark pathological features are presence of polypoid granulation tissue, or Masson bodies, in the alveolar spaces, with lung architecture preserved in most patients with good clinical outcomes poststeroid treatment. Clinically, patients exhibit signs and symptoms of subclinical or subacute infection that do not respond to antibiotics. Computed Tomography (CT) plays a vital role in establishing the diagnosis, monitoring of OP, and for better understanding of its imaging phenotypes which are crucial for clinical decision making. This article highlights the 3 main imaging patterns of OP on CT, the classic, nodular and fibrotic forms. Identifying these diverse imaging features holds the key for precise diagnosis of OP.
Incidence of malignancy in lung lesions initially classified as organizing pneumonia on CT-guided biopsies.
Organizing pneumonia is an inflammatory disorder that may co-exist with malignancy in the lung or elsewhere in the body. We aimed to assess patients with a lung biopsy diagnosis of organizing pneumonia for subsequent pathology confirmation of co-existing malignancy. In this retrospective IRB-approved, HIPAA-compliant study, 1314 consecutive patients who underwent CT-guided lung biopsy for suspected lung cancer or metastatic disease from 02/2014 to 04/2022 at a single tertiary referral hospital were identified. In 98/1314 (7.5%) patients, biopsies showed organizing pneumonia, which represented our study cohort. Clinical outcomes, including follow-up imaging and repeat tissue sampling if performed, were evaluated through chart review. Descriptive statistics were calculated. There were 43/98 (44%) females, mean age was 66 ± 14 years, mean lesion size 2.9 ± 2.1 cm, and 11/98 (11.2%) had prior history of malignancy. Of 98 patients initially diagnosed with organizing pneumonia on lung biopsy, 11 (11.2%) were subsequently found to have malignancy. Among these, 6 (54.5%) had pulmonary metastases and 5 (45.5%) had primary lung cancer. Malignancies were confirmed through percutaneous re-biopsy in 3/11 (27%) and bronchoscopic, endoscopic, or surgical procedures in 8/11 (73%). Malignancy can co-exist with organizing pneumonia in a substantial percentage of initial lung biopsies. Therefore, repeat tissue sampling should be considered when there is high clinical suspicion of malignancy despite an initial histopathological diagnosis of organizing pneumonia. This is especially relevant in lesions that demonstrate FDG avidity on PET/CT or an increase in size on interval imaging, or in instances where the biopsy core sizes are small or where the biopsies have intraprocedural complications.
