Determining chronicity and frequency of histologic lung lesions in feedyard cattle mortalities.
Several etiologic factors contribute to bovine respiratory disease, the primary cause of feedyard mortality. We performed a cross-sectional observational study to determine the frequency and chronicity of lesions in fatal feedyard pneumonia cases. Postmortem examinations were performed (n = 443), and 2 lung samples were collected from each animal. Histologic sections were scored using 10 classifications: 5 types of bronchopneumonia (BP; 3 acute, 2 chronic), 4 types of interstitial pneumonia (IP; 2 chronic, 2 acute), and 1 category for other findings. Cases with chronicity in either section were classified as chronic. Results from both lung sections were combined into a case-level diagnosis of BP, IP, or BP with IP (BIP). After exclusions because of autolysis or missing data, our diagnosis frequencies for 352 cases were: BP (150; 42.6%), IP (86; 24.4%), BIP (71; 20.2%), and Other (45; 12.8%). Of cases with an IP component, chronic lymphoplasmacytic IP (CLIP) was identified in 59 of 157 (37.6%) cases. A generalized linear model was used to evaluate the probability of case-level chronicity and potential associations with diagnosis and days on feed. Model-estimated probability of chronic lesions was higher in cases with IP (51% IP; 60% BIP) compared with BP (38%). Of 71 BIP cases, 27 had acute BP and acute IP; 18 had acute IP and chronic BP; 0 had chronic IP and acute BP; 5 had chronic IP and chronic BP; 14 had CLIP and acute BP; and 7 had CLIP and chronic BP. In our final dataset of 352 feedyard pneumonia cases, IP was found histologically in the lungs of 157 cases, either alone or in conjunction with BP; cases with an IP component were more likely to have chronic lesions. Acute interstitial pneumonia (AIP, also known as Hamman–Rich syndrome) is an acute, rapidly progressive idiopathic pulmonary disease that often leads to fulminant respiratory failure and acute respiratory distress syndrome (ARDS). AIP can be distinguished clinically from other types of interstitial pneumonia by the rapid onset of respiratory failure in a patient without preexisting lung disease. Louis Hamman and Arnold Rich first described AIP in 1935 as a fulminating diffuse interstitial fibrosis of the lungs. In 1986, Katzenstein introduced the term AIP, differentiating it from the group of chronic interstitial pneumonias. The American Thoracic Society and European Respiratory Society classify AIP as a major idiopathic interstitial pneumonia, compared with other rare or unclassified idiopathic interstitial pneumonias.
Scoping review of clinical signs, etiologies, pathology findings, risk factors, and treatments of acute interstitial pneumonia in North American feedlot cattle.
Acute interstitial pneumonia (AIP) has been a recognized bovine respiratory disease for many decades, yet the sporadic occurrences of AIP in feedlot cattle are poorly understood. We searched 3 databases (PubMed, CABI Direct Library, AGRICOLA) for primary literature on North American feedlot cattle, with a focus on interstitial pneumonia. We comprehensively reviewed reports of clinical signs, pathology findings, etiologies, risk factors, and treatments associated with AIP in feedlot cattle and identified gaps in knowledge. We included 19 peer-reviewed studies from 1976 to 2023. In 6 articles, the most common clinical signs for diagnosing AIP were severe dyspnea, increased expiratory effort, and grunting while breathing. In the 17 articles that provided detailed descriptions of the pathology and diagnostic features of AIP, consistent postmortem examination findings were interlobular edema and emphysema, a checkerboard appearance, and diffusely overinflated lung lobes. The most frequent histopathology findings were hyaline membranes, type II pneumocyte hyperplasia, and obliterative bronchiolitis. Although some potential etiologies, such as bovine respiratory syncytial virus (BRSV) and 3-methylindole (3MI), have been evaluated, no definitive cause has been identified consistently. Risk factors significantly associated with AIP in multiple studies included heifers, clinical onset at >45 days on feed (DOF), death at >78 DOF, and summer; the exact mechanisms remain unclear. The absence of standardized diagnostic criteria, along with a lack of research into potential effective prevention or treatment, underscore the need for further study to clarify the causes, diagnosis, and treatment of AIP in feedlot cattle.
Drug-induced Lung Injury Caused by Azacitidine That Was Not Treated with Drugs.
An 83-year-old male was hospitalized with pneumonia while receiving azacitidine for myelodysplastic syndrome. Despite treatment with antimicrobial agents, his pneumonia worsened. Bronchoscopy revealed organizing pneumonia (OP). This condition was thought to be a drug-induced lung injury caused by azacitidine. As the patient had no subjective symptoms, antimicrobial therapy was discontinued and the patient was monitored closely. Thereafter, the pneumonia gradually improved. Drug-induced lung injury caused by azacitidine, which has few subjective symptoms, can be treated without steroids. However, it should be noted that the condition may become resistant to steroid therapy if it progresses to acute interstitial pneumonia or if azacitidine treatment is continued.
Economic outcomes associated with acute interstitial pneumonia in Central U.S. High Plains feedyards.
The objective of this study was to evaluate net returns for Central U.S. high plains feedyard cattle identified with acute interstitial pneumonia (AIP) ante-mortem and postmortem (n = 5,339) and to examine economic outcomes across sex, placement weight, and number of AIP treatments. A decision tree framework was implemented to estimate net returns of cattle identified with AIP, where decision nodes represented choices made by the producer, and branches represented potential outcomes following a decision. The initial decision node was whether to treat cattle for AIP after the first identification for illness or to sell (cull) soon after diagnosis at reduced weight and price compared to cattle in the cohort sold at finished weight. Following initial treatment, cattle that remained in the feedyard either finished (with or without further treatment), were culled, or died after additional diagnosis and treatment. Probabilities of incidents at each node were obtained from the data distribution. This research indicates that estimated net returns for feedyard cattle identified with AIP vary by sex, placement weight, and number of AIP treatments. The expected net return to feeding healthy cattle was $193.67/animal, while the expected net return for cattle that finished after AIP treatment was -$639.71/animal for cattle treated once for AIP, -$612.41/animal for those treated twice for AIP, and -$529.57/animal for those treated three or more times for AIP. However, other health indicators and risk factors not included in this analysis should be considered when deciding whether to keep or cull feedyard cattle identified with AIP.
Determining Frequency of Multiple Organ System Involvement and Concurrent Lesions Identified in Feedyard Mortalities and Potential Associations with Cattle Demographics.
Necropsies are commonly used to diagnose the causes of death in feedyard cattle, but the documentation of multiple organ system involvement and concurrent lesions is limited. This observational study aimed to determine the frequency of such findings and their associations with animal demographics. Systemic necropsies were conducted for 889 cattle mortalities with minimal autolysis across six feedyards in the Central High Plains during the summers of 2022 and 2023. Lesions and abnormalities were recorded along with arrival weight, sex, days on feed (DOFs), and number of treatments. The results showed that 72% of mortalities had more than one gross lesion, averaging 2.3 lesions per animal. The most common organ systems affected together were digestive and pulmonary (19%), followed by cardiovascular, digestive, and pulmonary (6%), and cardiovascular and pulmonary (5%). Common concurrent lesions included bronchopneumonia with an interstitial pattern (BIP) and gastrointestinal lesions (GI) (8%), bronchopneumonia and GI (7%), and acute interstitial pneumonia (AIP) and GI (3%). A generalized linear mixed effects model revealed that the likelihood of multiple lesions increased with DOFs (p = 0.02). These findings highlight the value of thorough necropsy documentation to enhance our understanding of disease and guide improved feedyard management and treatment practices.
