A review of research progress of mirror therapy in peripheral nerve injury.
Mirror therapy, as a novel rehabilitation approach, has been widely applied in the treatment of various diseases, including post-stroke limb dysfunction, unilateral neglect, peripheral facial paralysis, complex regional pain syndrome, and childhood cerebral palsy. This study aims to investigate the effectiveness of mirror therapy in treating peripheral nerve injuries, expand the scope of diseases to which mirror therapy can be applied, and provide a novel treatment strategy for peripheral nerve injuries. This study conducted a comprehensive search and analysis of relevant clinical research on mirror therapy for peripheral nerve injuries, providing theoretical and clinical basis for the application of mirror therapy in the treatment of peripheral nerve injuries. Based on a comprehensive analysis of seven clinical studies, it can be concluded that mirror therapy (MT) has a positive effect on improving motor function. Mirror therapy is characterized by its simplicity, economy and non-invasiveness. It is an effective rehabilitation technique for treating peripheral nerve injuries and is suitable for low-cost, long-term home-based rehabilitation. It can be combined with conventional therapies to jointly promote the recovery of motor functions and accelerate the plastic regeneration of nerves.
Long-term relief of CRPS-associated limb dystonia with ketamine infusion.
Complex regional pain syndrome (CRPS) with associated dystonia is a severely debilitating condition often refractory to conventional management. We present a case report of three patients with chronic, treatment-refractory CRPS-related limb dystonia. Patients were treated with multiday intravenous ketamine infusions (target dose 30 mg/hour) every 3-6 months, supplemented with oral ketamine troches. This treatment protocol resulted in the complete resolution of dystonia in two patients and significant functional improvement in the third. The treatment was well-tolerated, with transient and manageable adverse events including nausea and hypertension. While these findings are limited due to the small sample size and lack of randomisation, this case report presents a potentially promising therapeutic approach for the treatment of CRPS-related motor complications in carefully selected patients with appropriate monitoring.
Beyond the Pain: Rethinking Chronic Pain Management Through Integrated Therapeutic Approaches-A Systematic Review.
Chronic pain is inherently multifactorial, with biological, psychological, and social factors contributing to neuropathic pain (NP) and central sensitization (CS) syndromes. Comorbidity between functional disorders and the lack of clinical biomarkers adds to the challenge of diagnosis and treatment, leading to frustration for healthcare professionals and patients. Available treatments are limited, increasing patient suffering with personal and financial costs. This systematic review examined multisensory processing alterations in chronic pain and reviewed current pharmacological and non-pharmacological interventions. A structured search was conducted on the PubMed database using the keywords Central Sensitization, Fibromyalgia, Complex Regional Pain Syndrome, and Neuropathic Pain, combined with the keywords Vision, Audition, Olfaction, Touch, Taste, and Proprioception. Papers were then filtered to discuss current treatment approaches. Articles within the last five years, from 2018 to 2023, have been included. Papers were excluded if they were animal studies; investigated tissue damage, disease processes, or addiction; or were conference proceedings or non-English. Results were summarized in table form to allow synthesis of evidence. As this study is a systematic review of previously published research rather than a clinical trial or experimental investigation, the risk of bias was assessed independently by at least two reviewers. 138 studies were identified and analyzed. Of these, 96 focused primarily on treatment options for chronic pain and were analyzed for this systematic review. There were a few emerging themes. No one therapy is effective, so a multidisciplinary approach to diagnosis, including pharmacological, somatic, and psychological treatment, is generally predicted to achieve the best outcomes. Cranial neurovascular compromise, especially of the trigeminal, glossopharyngeal, and potentially the vestibulocochlear nerve, is being increasingly revealed with the advancement of neuroimaging. Cortical and deep brain stimulation to evoke neuroplasticity is an emerging and promising therapy and warrants further investigation. Finally, including patients in their treatment plan allows them control and offers the ability to self-manage their pain. Risk of bias limits the ability to judge the quality of evidence.
The neuroimmune axis and chronic pain disorders.
Both the nervous system and the immune system alert and protect the body against pathogen invasion and damage, but the extent of their interactions have only truly been elaborated in the past few decades. Lymph nodes (LN) are essential immune command centers that modulate the adaptive immune response through the integration of a multitude of signals and activation of T and B lymphocytes. LN are also known to be innervated by sympathetic and sensory nerves. While sympathetic regulation of the immune system is well-established, the impact of sensory neurons has been overlooked. Recently, the sensory neuronal population that innervates popliteal LN has been described in detail, with most cells characterized as peptidergic nociceptors. Of further interest, patients suffering with pain disorders like fibromyalgia, complex regional pain syndrome (CRPS) and hereditary sensory and autonomic neuropathies (HSAN) display both immune and autonomic dysfunction. The localization of nociceptors on LN indicates that there may be a larger impact of sensory neuronal signaling on adaptive immunity than has been explored currently. Therefore, this paper investigates the impact of neural signaling on immunity, specifically the LN and its main cell populations, and aims to address the gaps in our understanding of immune problems featured in pain disorders with aberrant or absent pain.
Efficacy and Safety of Bisphosphonates for Complex Regional Pain Syndrome : A Systematic Review and Meta-analysis.
Clinical guidelines recommend bisphosphonates for complex regional pain syndrome (CRPS) despite limited evidence of efficacy. To determine the efficacy and safety of bisphosphonates compared with placebo for CRPS. MEDLINE, Embase, Cochrane Central Register of Controlled Trials, and 3 trial registries from inception to 16 September 2025. Randomized controlled trials enrolling adults with CRPS (type I or II) to bisphosphonate treatment or placebo. Primary outcomes were pain intensity and adverse events. Data were synthesized with random-effects meta-analyses. Risk of bias and certainty of evidence were assessed using the Cochrane Risk of Bias 2 Tool and GRADE (Grading of Recommendations Assessment, Development and Evaluation). Eleven trials (754 participants; CRPS type I, 97%), evaluating alendronate (n = 2), clodronate (n = 1), neridronate (n = 5), pamidronate (n = 1), and zoledronate (n = 2), were included. Bisphosphonates may result in little to no difference in pain intensity in the immediate term (≤4 weeks; 0-to-100 scale; mean difference [MD], -9.1 [95% CI, -19.2 to 1.1]; low certainty). In the short term (>4 weeks to 3 months; primary time point), bisphosphonates may reduce pain intensity (MD, -10.0 [CI, -18.9 to -1.1]; low certainty), and in the medium term (>3 to 6 months), they may result in little to no difference in pain intensity (MD, 8.0 [CI, -15.4 to 31.4]; low certainty). The evidence is very uncertain about the effects of bisphosphonates on pain intensity in the long term (>6 months; MD, -2.5 [CI, -19.6 to 14.6]). Bisphosphonates probably increase risk for adverse events (risk ratio, 1.1 [CI, 1.0 to 1.2]; moderate certainty). High heterogeneity and uncertain medium- and long-term effects. Evidence mostly applies to CRPS type I and includes non-U.S.-approved formulations (neridronate, clodronate). Bisphosphonates may reduce CRPS pain intensity in the short term, but treatment is accompanied by adverse events. Future research should resolve uncertainty around which patients with CRPS are most likely to benefit from bisphosphonates. None. (PROSPERO: CRD42024559783).
