The unsung hero: ntnh gene as complementary botulism marker.
Botulism is a rare but severe neurological disease caused by botulinum neurotoxins (BoNTs). Standard diagnostic methods including the mouse bioassay (SMB) and bont gene type-specific PCR, are often limited by the high genetic diversity among bont subtypes, which can lead to false-negative results. The nontoxin-nonhemagglutinin (ntnh) gene is highly conserved and exclusively co-located with the bont gene complex. Thus, this study evaluates the use of ntnh gene as a complementary diagnostic tool for botulism and assesses its association with BoNT types. The ntnh gene was detected in a prospective BoNT-diagnostic group (n=88) and a BoNT-historical group (n=54). Toxin cluster proteins were identified in GenBank and RefSeq Clostridium proteomes using MMSeqs2. ntnh gene detection reinforced positive results from SMB or bont gene tests in 26 cases (35.62% of the total confirmed cases) of foodborne and infant botulism. In two foodborne cases from the BoNT-diagnostic group, the ntnh gene was detected despite negative results from both SMB and bont gene tests, highlighting its potential to identify missed cases. An in silico analysis of 3,250 RefSeq and 2,494 GenBank annotated Clostridium proteomes was conducted. respectively. So, NTNH showed a high co-presence pattern with BoNT. Moreover, NTNH sequences were far more conserved than BoNT sequences in inter-type comparisons (67.2 vs.39.7), which highlights its applicability as a disease biomarker. The ntnh gene analysis is a valuable complementary tool enhancing the diagnosis of botulism. The study highlights the need for clear guidelines to interpret positive ntnh results when direct toxin or bont gene confirmation are negative.
Enteric botulinum neurotoxins facilitate infection by Salmonella and Shigella.
Botulism is a life-threatening disease caused by botulinum neurotoxins (BoNTs) released by Clostridium bacteria. BoNTs induce a flaccid paralysis by blocking neurotransmitter release from peripheral cholinergic neurons via soluble N-ethylmaleimide-sensitive factor attachment protein receptor (SNARE) cleavage. The enteric nervous system (ENS) controls antimicrobial defense reactions within the intestine through neuron-released mediators. Here, we show that subclinical doses of BoNT/A and BoNT/B, which are not sufficient to cause botulism when released in the intestinal lumen, paralyze ENS neurons favoring infection by prototypic bacterial pathogens such as Salmonella enterica and Shigella flexneri. The BoNTs reduce the release of mucin, decreasing the protective barrier against bacterial infection. These findings disclose previously unrecognized functions of BoNTs and roles in intestinal infections. Moreover, they indicate that BoNTs are valuable tools for investigating the ENS neuron involvement in the intestinal defense reactions.
Repetitive nerve stimulation findings in iatrogenic botulism compared with myasthenia gravis.
The widespread use of botulinum toxin has been accompanied by an increase in complications, including iatrogenic botulism (IB). This study investigated repetitive nerve stimulation (RNS) findings in IB and myasthenia gravis (MG) patients. Clinical and neurophysiological data were retrospectively reviewed in IB patients negative for both acetylcholine receptor and muscle-specific kinase antibodies, and in MG patients who were positive for either. RNS was performed on the ulnar nerve at 2, 3, 5, and 50 Hz, and on the facial and spinal accessory nerves at 2, 3, and 5 Hz. Fifteen IB and 23 MG patients were included. Compound muscle action potential (CMAP) amplitudes of the ulnar and facial nerves were lower in IB (p = 0.014, p = 0.006). A progressive decrement at low-frequency RNS occurred in 60% of IB and 17% of MG patients. At 5 Hz facial RNS, 1st-2nd (p = 0.044) and 1st-4th (p = 0.008) decrements were greater in MG, whereas the 4th-10th decrement was higher in IB (p = 0.031). Ulnar CMAP changes at 50 Hz were greater in IB (p < 0.001); nine patients showed increments > 40%, four > 60%, and one > 100%, while none of the MG patients exceeded 40%. Reduced CMAP amplitude, an incremental response at high frequency, and a progressive decrement at low frequency were more commonly encountered in IB. In MG, a U-shaped pattern at low frequency and a more pronounced decrement between the 1st and 2nd responses were noted.
Unusual brain MRI findings in botulism: a case report of brainstem DWI restriction and pathophysiological implications.
Multifocal costal osseous pneumatosis and osteomyelitis secondary to Clostridium botulinum in a dog.
A 3-year-old castrated male German shepherd dog was presented with a 48-hour history of lethargy and anorexia. On presentation, the dog was laterally recumbent, hyperthermic, tachycardic, severely dehydrated, and exhibiting abdominal pain. Abdominal radiographs raised suspicion for splenic torsion. An exploratory laparotomy revealed a suspected splenic vein thrombosis, prompting a splenectomy and an incisional gastropexy. Computed tomography revealed multifocal osseous pneumatosis, rib abscesses, costochondral luxation, renal infarcts, suspected intramuscular thrombosis, aspiration pneumonia, and lymphadenopathy. Despite negative blood culture and peri-costal abscess cultures, Clostridium botulinum was isolated from bone marrow obtained via costochondral rib aspiration. During hospitalization, persistent non-regenerative anemia and thrombocytopenia were observed; the dog was also nonambulatory tetraparetic, worse on the right, indicating a predominantly right-sided myelopathy. After some improvement, the dog was discharged from the hospital; however, his condition progressively worsened despite treatment, leading to humane euthanasia 1 mo later. Necropsy revealed disseminated round cell tumor consistent with lymphoma. Key clinical message: Visceral organ gas accumulation or intraosseous gas in the absence of previous trauma or surgery should prompt suspicion for an anaerobic infection. To the authors' knowledge, this is the first reported case of rib pneumatosis secondary to disseminated C. botulinum in a dog. Les radiographies abdominales ont fait suspecter une torsion de la rate. Une laparotomie exploratrice a révélé une suspicion de thrombose de la veine splénique, conduisant à une splénectomie et une gastropexie incisionnelle. La tomodensitométrie a révélé une pneumatose osseuse multifocale, des abcès costaux, une luxation costochondrale, des infarctus rénaux, une suspicion de thrombose intramusculaire, une pneumonie d’aspiration et une lymphadénopathie. Malgré des hémocultures et des cultures d’abcès péricostaux négatives, Clostridium botulinum a été isolé de la moelle osseuse prélevée par ponction costo-chondrale. Durant son hospitalisation, une anémie non régénérative et une thrombocytopénie persistantes ont été observées; le chien présentait également une tétraparésie non ambulatoire, plus marquée à droite, indiquant une myélopathie prédominante à droite. Après une légère amélioration, le chien a obtenu son congé de l’hôpital; cependant, son état s’est progressivement aggravé malgré le traitement, conduisant à une euthanasie 1 mois plus tard. L’autopsie a révélé une tumeur à cellules rondes disséminée, compatible avec un lymphome.Message clinique clé :L’accumulation de gaz dans les organes viscéraux ou la présence de gaz intra-osseux en l’absence de traumatisme ou d’intervention chirurgicale préalable doivent faire suspecter une infection anaérobie. À la connaissance des auteurs, il s’agit du premier cas rapporté de pneumatose costale secondaire à une infection disséminée à C. botulinum chez un chien.(Traduit par Dr Serge Messier).
