Células-tronco do limbo, também conhecidas como células-tronco epiteliais da córnea, são células-tronco unipotentes localizadas na camada epitelial basal do limbo corneano. Elas formam o limite entre a córnea e a esclera. As características das células-tronco do limbo incluem uma taxa de renovação lenta, alto potencial proliferativo, clonogenicidade, expressão de marcadores de células-tronco, bem como a capacidade de regenerar todo o epitélio corneano. A proliferação das células-tronco do limbo tem o papel de manter a córnea; por exemplo, substituindo as células que são perdidas através das lágrimas. Além disso, essas células também impedem que as células epiteliais conjuntivais migrem para a superfície da córnea.

Endogenous Therapeutic Agents for Acute Corneal and Ocular Surface Injuries.

Acute corneal injuries vary greatly in morbidity, but can cause severe cases of vision loss. Among these injuries are mechanical corneal traumas and acute chemical burns. The current standard of care in the acute phase of management is aimed at reducing the offending agent, pain management, and infection prevention; however, corneal scarring and limbal stem cell deficiency may result even after the acute management of severe corneal injuries. In the most serious cases, surgical intervention, including cornea or stem cell transplantation, may be required to restore functional vision. As such, there is a great need for additional therapies in the acute phase of management that can help to reduce long-term, vision-reducing outcomes. Certain endogenous agents have emerged as a therapeutic class that may be used for the promotion of corneal wound healing with the goal of reducing the odds of requiring invasive surgery. In this review, we provide an overview of novel endogenous therapeutic agents for the treatment of acute corneal trauma and acute chemical burns. The endogenous therapeutic agents explored include amnion, exosomes, stem cells, platelet-rich plasma, peptides, growth factors, and umbilical cord serum. The results of these studies indicate encouraging potential for endogenous agents to facilitate corneal recovery and vision preservation. However, because many of these agents are in the early stages of development and clinical trials, further research is required to understand their efficacy in the clinical setting.

Repair of Corneal Epithelial Defects.

The corneal epithelium, a stratified squamous non-keratinized layer of 50-60 μm thickness, forms the outermost barrier of the cornea and provides both optical clarity and protection against trauma, infection, and fluid imbalance. It plays a vital role in protecting the eye and maintaining visual clarity. A range of conditions, including trauma, metabolic disorders, microbial infection, and limbal stem cell deficiency, can lead to chronic corneal epithelial defects and subsequent visual impairment. Epithelial renewal is a continuous process, primarily sustained by stem cells located at the limbus. These stem cells give rise to transient amplifying cells, which migrate centripetally and superficially to maintain epithelial integrity. Wound healing follows a highly regulated sequence, superficial cells slide to cover the defect, basal cells proliferate, and corneal nerves realign to support epithelial stratification. This process is orchestrated by cytoskeletal remodeling, integrin-matrix interactions, and growth factor signaling. The epithelium relies on glucose from the corneal stroma, primarily metabolized through glycolysis, while mitochondrial oxidative phosphorylation generates the ATP required for repair. Thus, epithelial regeneration is closely tied to cellular energy availability. Enhancing this process involves supporting mitochondrial function, metabolic signaling pathways, and stem cell activity. Emerging strategies in regenerative ophthalmology include NAD+ replenishment, activation of AMP-activated protein kinase (AMPK), application of growth factors, targeted nanotherapies, and photobiomodulation. This chapter explores these cutting-edge approaches aimed at promoting energy-driven regeneration of the corneal surface.

In situ spatial transcriptomics reveals novel markers of the limbal stem cell niche and ocular surface epithelia.

The mammalian cornea is endowed with stem cells (SCs) that have lifelong regenerative activity. The niche for these cells is the limbus, and damage to it or its SCs results in limbal stem cell deficiency (LSCD). Despite the numerous studies that employ single-cell RNA sequencing, the identity of these cells remains an enigma principally because their spatial positioning is lost upon dissociation. These adversities were avoided via on-tissue spatial transcriptomics where Krt16 and Nkiras1 were differentially expressed. Krt16 was dynamically expressed in the developing limbus, correlated with slow-cycling label-retaining limbal epithelial SCs and was induced during corneal injury, observations consistent with marking functional SCs. Additionally, we established Nkiras1 as a novel maker of limbal neutrophils. Because current gold-standard treatments for LSCD include SC transplantation, our data will inform future studies in delivering a more reliable standard therapy that incorporates an identifiable SC population to improve clinical outcomes.

PAX6 Deficiency Compromises the Ability of Limbal Epithelial Stem Cells to Properly Differentiate Into Mature Corneal Epithelial Cells.

Aniridia, driven by PAX6 mutations, causes aniridia-associated keratopathy (AAK), a progressive condition linked to limbal stem cell deficiency. A major hurdle to developing targeted therapies for AAK is the incomplete understanding of the molecular abnormalities in affected corneas. To address this, we leveraged Pax6± (Pax6 het) mice, a model of AAK, and applied single-cell RNA sequencing (scRNA-seq) to profile the transcriptomic changes at a single-cell resolution. ScRNA-seq of corneal/limbal tissues of wild type (WT) and Pax6 het mice were conducted. Immunostaining was performed to examine the expression of specific markers for stem cells. ScRNA-seq identified a quiescent limbal epithelial stem cell (LESC)-like cell cluster and an early transient amplifying cell (eTAC)-like cluster. An increase in the cell numbers in these two clusters in the Pax6 het mouse corneas was observed. Immunostaining detected a marked increase in markers for these two clusters including Tmem176b, Apoe, and Krt15 in the corneal epithelium of Pax6 het mice, suggesting an increase of these LESC/eTA-like cells into the corneal epithelium. The Pax6 deficiency inhibited the expression of genes involved in cell proliferation in the eTAC-like cluster as well as the expression of genes related to corneal epithelial cell fate and differentiation compared with WT mice. Our single cell transcriptome of the limbus and cornea of Pax6 het mice indicates that AAK may be due to the increase of dysfunctional stem/eTACs with defects in committing to a corneal epithelial cell fate and differentiation.

Losartan Alleviates Chemical Burn-Induced Limbal Stem Cell Deficiency: Repurposing a Venerable Anti-Hypertension Drug.

Currently, treatment for limbal stem cell deficiency (LSCD) involves limbal stem cell transplantation using either autologous or allogeneic tissue to restore the corneal surface. However, limitations such as donor shortages, immune rejection, and variable outcomes highlight a crucial need for more effective and safer therapeutic options. We utilized a nitrogen mustard (NM)-induced corneal injury model to create LSCD, characterized by conjunctivalization. Mice exposed to NM were intraperitoneally injected with losartan. Single cell RNA sequencing (scRNA-seq) was conducted using a 10X Genomics platform. Previously, we demonstrated that corneal inflammation can be reversed by treatment with losartan, a widely used anti-hypertension drug with established long-term safety. Here, we demonstrate that systemic treatment with losartan markedly decreased corneal haze and inflammation during the acute and delayed phases of NM corneal injury. In the delayed phase, losartan treatment reduced NM-induced neovascularization in the cornea. Goblet cells detected within the corneal epithelium (conjunctivalization) during the delayed phase were significantly reduced following losartan treatment, indicating that losartan significantly resolves this classical hallmark of LSCD. The scRNA-seq showed that NM injury induced a dramatic increase in monocytes/macrophages in the cornea and that such an increase was attenuated by losartan treatment. Flow cytometry analysis confirmed that losartan attenuated corneal inflammation via reducing the monocytes/macrophages in the cornea. Our findings strongly support a new therapeutic use for losartan in blunting LSCD, which is a consequence associated with numerous corneal inflammatory conditions (e.g. diabetic keratopathy and tear gland insufficiency).
