BCG Infection Following Therapy of Infant Acute Lymphoblastic Leukemia.
The Bacillus Calmette-Guérin (BCG) vaccine is widely administered in tuberculosis-endemic regions. BCG infection (BCGitis) is a rare vaccine adverse event seen in immunocompromised patients. While BCGitis is well documented in patients with inborn errors of immunity and acquired immunodeficiency, data on BCGitis in infants with hematological malignancies are scarce. We retrospectively analyzed five cases of BCGitis in infants with acute lymphoblastic leukemia (ALL) and treated at a single tertiary pediatric oncology center. Clinical characteristics, timing of BCGitis in relation to anticancer therapy, immune parameters, management, and outcomes were reviewed. The median age at ALL diagnosis was 1.5 months (range, 0.5-5.3); four patients had KMT2A rearrangements. All received BCG vaccination with the Russian BCG strain vaccine at birth. BCGitis developed during chemotherapy (n = 2), blinatumomab immunotherapy (n = 1), or post-hematopoietic stem cell transplantation (HSCT; n = 2). One patient with BCGitis during chemotherapy had a reactivation of BCGitis after HSCT. All cases presented as localized infection; one had regional lymphadenitis. BCGitis onset correlated with either T-cell reconstitution or therapy-induced cytopenia. Antimycobacterial therapy (levofloxacin, amikacin, isoniazid, and ethambutol) led to resolution in all cases. BCGitis in infants with acute leukemia is a rare complication that is likely associated with chemotherapy-induced immunodeficiency or immune reconstitution following chemotherapy, immunotherapy, or HSCT. While manifestations are typically localized, careful management is required to prevent further complications. This is particularly crucial for patients undergoing HSCT, where prophylactic antimycobacterial therapy may be considered in selected high-risk settings.
Resilient in adversity: Innovative treatment support interventions in facilitating ART adherence among young adults living with HIV in the informal settlements of Kibera, Nairobi.
Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/AIDS) poses significant health challenges. Despite ongoing efforts to scale up Antiretroviral therapy (ART) to save lives from AIDS-related morbidity and mortality,adherence among young adults remains low at 65% in sub-Saharan Africa. Our study aimed to inform the development of youth-friendly, context-sensitive programs that are both scalable and sustainable in urban informal settlements. This paper presents findings on youth facilitators to ART adherence in Kibera informal settlement. The study was conducted in Nairobi urban informal settlement of Kibra, Nairobi county, Kenya. We utilized in-depth interviews (n = 25) and key informant interviews (n = 10) to collect data on faciliators to ART adherence among purposively selected eligible partcipants. Thematic analysis framework was employed to analyze the data. HIV adherence facilitators encompassed patient-related factors like self-acceptance and open disclosure, health system factors such as accessibility, provider relationships, and patient reminders, and socio-economic aspects including support groups, education awareness, family support, and economic empowerment. The findings emphasize the need for tailored, age-appropriate, and context-specific interventions in informal settlements to enhance HIV adherence. Fostering self-acceptance, creating an enabling environment through youth friendly services, improving provider-patient relationships, and leveraging social support systems such as peer groups and educational initiatives are essential strategies. Integrating these elements into programmatic and policy frameworks will ensure that interventions resonate with the unique realities of this populationpromoting sustainable adherence outcomes. The findings will provide a foundation for developing context-sensitive interventions and programs to improve adherence in similar settings.
Delayed viral rebound post-ART interruption in infant macaques given SIV-specific neutralizing antibodies.
Interventions to prevent HIV rebound in the absence of antiretroviral therapy (ART) are needed. Here, we use a rhesus macaque model of pediatric HIV to assess the impact of simian immunodeficiency virus (SIV) Env-specific rhesus monoclonal antibodies (RhmAbs) on viral rebound. We find that RhmAb-treated infants experience delayed rebound compared to controls (median of 64 versus 7.5 days, p = 0.0007). Time to rebound very strongly correlates with both ITS102.01-LS concentrations (r = 0.9, p = 0.002) and anti-drug antibodies against ITS103.01-LS (r = -0.95, p = 0.008). SIVgag-specific CD8+ T cell levels increase after RhmAb administration and negatively correlate with rebound viremia (r = -0.67, p = 0.0415). Treatment also expands neutralization breadth and potency against tier 2 and tier 3 viruses in 5/8 and 3/8 RhmAb-receiving animals, respectively. Thus, giving SIV Env RhmAbs prior to ART cessation leads to prolonged time to viral rebound, enhanced antiviral T cell responses, and broadening of neutralization capacity, thereby supporting use of neutralizing Abs to promote post-ART viral control.
Env-antibody coevolution identifies B cell priming as the principal bottleneck to HIV V2 apex broadly neutralizing antibody development.
Broadly neutralizing antibodies (bNAbs) are rarely elicited during HIV-1 infection. To identify obstacles to bNAb development, we longitudinally studied 122 rhesus macaques infected by 1 of 16 different simian-human immunodeficiency viruses (SHIVs). We identified the V2 apex region of the envelope (Env) as the most common bNAb target and a subset of Envs that preferentially elicited these antibodies. In 10 macaques, we delineated Env-antibody coevolution from B cell priming to bNAb development. Antibody phylogenies revealed permissive developmental pathways guided by evolving Envs that contained few mutations in or near the V2 apex C-strand, which were a sensitive indicator of apex-targeted responses. The absence of such mutations reflected a failure in bNAb priming. These results indicate that efficiency of B cell priming, and not complexities in Env-guided affinity maturation, is a primary obstacle to V2 apex bNAb elicitation in SHIV-infected macaques and identify specific HIV-1 Envs to advance as vaccine platforms.
Prevalence and predictors of virological failure and quality of life of people with HIV/AIDS at a Municipal Hospital, Volta Region -Ghana: A cross-sectional study.
Despite several interventions to eradicate HIV/AIDS globally, virological failure continues to threaten the goals of antiretroviral therapies (ART) and quality of life (QoL) of people with HIV/AIDS (PWHA). This study aimed to assess the prevalence and predictors of virological failure and determine the QoL of PWHA. A cross-sectional study was conducted at the ART clinic of a Municipal Hospital, from June to August 2023, to assess the socio-demographic, medical data, and QoL of PWHA receiving therapy at the clinic. Participants were randomly selected and interviewed: their weight and height were taken and their clinic folders examined to assess virological failure status. Both self-developed structured questionnaire and the WHOQOL-HIV BREF scale (Cronbach alpha = 0.84) were used to assess participants' data. Bivariate and multiple logistic regression analysis were conducted to determine predictors of virological failure. Also, multiple linear regression was conducted to determine factors influencing QoL of study participants. A total of 398 participants comprising of 328 (82.41%) females, and with a mean age of 48.2 years (SD ± 11.71 years), were recruited into the study. The prevalence of virological failure was 6.03%. Factors such as forgetting to take ART (Adjusted Odds Ratio (AOR) = 2.87, 95% Confidence Interval (CI) = 1.02, 7.51; p = 0.04), being classified as baseline WHO clinical staging II (AOR = 6.20, 95% CI = 1.91, 20.04; p = 0.002), and HIV stigmatization (AOR = 3.97, 95% C.I. = 1.1, 14.25; p = 0.035) were associated with virological failure. The overall QoL was good (75.35%). Having no comorbidities (Coefficient of Determination (β) = -2.7, p < 0.0001), having social support (β = 3.94, p < 0.0001) and receiving an average monthly income (β = 2.03, p = 0.002) contributed to good QoL. Virological failure in the municipality exceeded the 5.0% target set by the Joint United Nations Programme, despite majority of the study participants presenting with good QoL. The National AIDS Control Programme should consider long-acting injectable therapy for PWHA struggling to adhere to medication.
