Maxillary mesenchymal chondrosarcoma harboring HEY1::NCOA2 fusion in a 13-year-old girl: a rare case report and literature review.

Mesenchymal chondrosarcoma (MCS) is a rare and highly aggressive subtype of chondrosarcoma, accounting for less than 1% of all chondrosarcomas. It predominantly affects adolescents and young adults and frequently arises in craniofacial bones and soft tissues. Diagnosis is challenging because of significant histological overlap with other high-grade spindle cell sarcomas, particularly when the cartilaginous component is minimal or absent. The identification of the HEY1::NCOA2 gene fusion has emerged as a highly specific molecular marker for MCS, substantially improving diagnostic accuracy and providing potential therapeutic implications. We report the case of a 13-year-old girl who presented with a 3-month history of progressive right cheek swelling. Imaging revealed a destructive mass in the right maxillary sinus. Histopathological evaluation demonstrated a high-grade spindle cell tumor, initially interpreted as fibrosarcoma, showing diffuse vimentin positivity, a high Ki-67 proliferation index (35%-40%), and CD34 negativity. Comprehensive molecular analysis confirmed a pathogenic HEY1::NCOA2 gene fusion, while ETV6::NTRK3 fusion was excluded. The patient was treated with VAC chemotherapy (vincristine, actinomycin D, cyclophosphamide), local radiotherapy (60 Gy), cranial prophylactic radiotherapy (12 Gy), and subsequent debulking surgery. Follow-up 18F-FDG PET/CT demonstrated a partial metabolic response. Given persistent disease and molecular evidence suggesting activation of the PI3K/AKT/mTOR pathway in MCS, maintenance therapy with the mTOR inhibitor sirolimus was initiated. This case highlights the pivotal role of molecular diagnostics-particularly RNA sequencing-in establishing the diagnosis of mesenchymal chondrosarcoma and differentiating it from other high-grade pediatric sarcomas with overlapping morphology. Identification of the HEY1::NCOA2 fusion not only confirms the diagnosis but may also support biologically targeted therapeutic strategies. Multimodal treatment incorporating chemotherapy, radiotherapy, surgery, and targeted maintenance therapy can achieve meaningful disease control in aggressive craniofacial MCS. To our knowledge, this represents one of the very few reported pediatric cases of maxillary MCS with confirmed HEY1::NCOA2 fusion managed with sirolimus-based maintenance therapy.

Vasculogenic Mimicry: A Potential Therapeutic Target for Chondrosarcoma Therapy.

Chondrosarcomas (ChSs) are mesenchymal chemo- and radiation-resistant tumors, representing the second most frequently diagnosed bone sarcoma after osteosarcoma and 20% of all bone sarcomas. Most of ChS patients have a good prognosis after complete surgical resection. Conversely, patients with inoperable disease, due to the tumor location or metastatic dissemination, represent a great clinical challenge due to the lack of effective therapeutic options. In this study, to the best of our knowledge, we document, for the first time in human ChS tissues, the existence of CD-31- and Podoplanin-negative vascular-like channels containing red blood cells, allowing us to hypothesize the occurrence of vasculogenic mimicry (VM) in ChSs. By using patient-derived ChS cells and a stabilized ChS cell line, we demonstrate that ChS cells are able to form in vitro tubules apparently similar to those formed by endothelial cells. Further characterization of these vessels revealed the pivotal role of the Urokinase Plasminogen Activator Receptor (uPAR) in mediating the capability of ChS cells to form VM. Finally, we provide evidence that, unlike bevacizumab, which did not exert any effect, the uPAR-derived antiangiogenic peptide RI-3 behaves as a potent inhibitor of VM.

Endoscope-assisted Transmastoid Infralabyrinthine approach to the petrous apex: A new corridor for multicompartmental jugular foramen tumors.

Multicompartmental jugular foramen tumors may extend anteriorly and superiorly toward the petrous apex, posing significant challenges for traditional surgical corridors. The authors study the feasibility of endoscope-assisted transmastoid infralabyrinthine approach (e-TIA) to access the superior aspect of petrous apex. Five cadaveric specimens were studied. A reference object was placed transnasally at the petrous apex to assess surgical exposure. To assess the working space of e-TIA, cross-sectional area was measured using three-dimensional reconstruction. Furthermore, we reviewed three patients with multicompartmental jugular foramen tumor in which this corridor was used. The e-TIA, initiated with a 30° endoscope using the chopstick technique, accessed the superior petrous apex, Meckel's cave, and the internal carotid artery (ICA) from its cervical to lacerum segments. Radiological analysis demonstrated that the approach trajectory shifts at the cochlea level. Cross-sectional area analysis revealed that the e-TIA corridor provides sufficient space for manipulation of the endoscope, suction, and drill. Clinically, near total resection was achieved in all cases without new neurological complications. The e-TIA offers a systematic extension to the superior petrous apex and the ICA from its cervical to the lacerum segments. utilizing an angled endoscope to enhance visualization. When combined with an anterolateral approach, this dual-corridor strategy provides a unique "looking-up" line of sight to the petrous bone. Overall, the e-TIA represents a promising option in selected multicompartmental jugular foramen tumors with petrous apex extension.

Integrated Immune and Molecular Profiling Identifies Prognostic Subgroups and Therapeutic Targets in Chondrosarcoma.

Chondrosarcoma (ChS) is a rare bone malignancy with heterogeneous behavior, the molecular and immunological background of which remains unknown. No effective systemic treatment for advanced ChS patients is available. The aim of this study was to develop an immune-mutational classification of ChS and to search for novel prognostic factors and molecular targets. We performed an immunological-molecular profiling of 99 patients diagnosed with primary ChS G1-G3 and dedifferentiated ChS. An expression of 20 immune response markers was assessed by IHC and targeted the next-generation sequencing of 409 genes was performed. Immunological and mutational profiles were correlated with overall survival using a multivariate LASSO-penalized Cox model. Three immunophenotypes were described-"cold" (IMP1), "hot" (IMP2), and "intermediate" (IMP3). IMP1 was the most prevalent in G1 cases, while IMP2 was the most prevalent in dedifferentiated cases. IDH1/2 or TP53 mutations were associated with high-grade ChS (FDR < 0.05). IMP2 was characterized by a higher number of immune infiltrates in the central region of the tumor (HR: 3.3; CI: 1.13-9.8; p < 0.05). IDH1 mutations were present most often in IMP2 cases (HR: 3.8; CI: 1.75-8.1; p < 0.001). Tumor size, dedifferentiated subtype, IDH1 mutation and the presence of IMP2 were identified as independent negative prognostic survival factors in ChS. An immune-mutational classification system for ChS patients was proposed, which may be used to identify those potentially suited for immunotherapy combined with IDH-mutant inhibitors in future research.

Olutasidenib in recurrent/relapsed locally advanced or metastatic IDH1-mutated chondrosarcoma: phase 1b/2 trial.

Chondrosarcomas are rare cartilaginous neoplasms with limited treatment options. Isocitrate dehydrogenase 1/2 (mIDH1/2) mutations occur in 65% of chondrosarcomas. Here we report safety and efficacy of olutasidenib, an mIDH1 inhibitor, evaluated in patients with locally advanced or metastatic mIDH1 chondrosarcoma (Clinicaltrials.gov identifier: NCT03684811). The primary endpoint was objective response rate by tumor evaluation; secondary endpoints included adverse events, progression-free and overall survival. Patients received olutasidenib 150 mg twice daily. Twenty-three patients were enrolled; 16 were diagnosed with conventional chondrosarcoma (cCS). Median age was 57 (range, 30-71) years. In 21 response-evaluable patients, 11 (52%) had stable disease, 8 (38%) had progressive disease, and 2 (10%) were not evaluable. Median progression-free survival (mPFS) was 2.0 months (95% confidence interval [95%CI]: 1.7, 4.7); median overall survival was 16.0 months (95%CI: 7.7, not reached). Among patients with cCS, 10 (63%) had stable disease; 6 (38%) had progressive disease; mPFS was 3.5 months (95%CI: 1.7, 5.1). Median overall survival in cCS patients was 19.0 months (95% CI: 7.7, not reached). No dose-limiting toxicities were reported during the study. Olutasidenib was well tolerated and conferred disease control in cCS. Study limitations include open-label design and low patient sample due to rarity of cCS.
