From blink to care: smartphone video-based functional analysis and personalized management in pediatric blepharoptosis.

Early detection of congenital ptosis is critical to prevent visual and psychosocial impairment in children, yet clinical assessment is challenged by limited patient cooperation and specialist availability. In this prospective, multicenter study, we developed and validated a smartphone-based system comprising three modules: morphological assessment, functional analysis, and a domain-adapted dialogue model, using 3164 blink clips and 1,229 facial images. The morphological module showed high measurement accuracy with intraclass correlation coefficients over 0.90 versus manual assessments. The functional module identified levator dysfunction with an area under the curve of 0.993, achieving robust functional stratification accuracy in both internal (0.91) and real-world (0.89) cohorts. The dialogue model demonstrated improved correctness and applicability over its baseline in addressing ptosis-related queries, achieving overall performance comparable to GPT-4o in expert evaluation and a patient satisfaction score of 4.93/5 in real-world deployment. This smartphone platform enables precise ptosis evaluation with patient-centered interaction, facilitating informed decision-making and personalized care in oculoplastic practice. Correction of Congenital Ptosis With Preservation of Single-Eyelid Morphology: A Modified Palpebral Margin Incision Technique.

Conventional anterior ptosis correction typically creates an iatrogenic double eyelid, which may be undesirable for patients who wish to preserve single-eyelid morphology. It may also leave a conspicuous scar. This study evaluates a modified eyelid-margin incision technique to correct ptosis while concealing the scar and maintaining a single-eyelid appearance. The authors retrospectively reviewed 21 patients (33 eyes) with congenital ptosis who underwent ptosis correction by a palpebral margin incision. Preoperative and postoperative margin reflex distance 1 (MRD1) and palpebral fissure height (PFH) were evaluated. Patients were followed up to document postoperative complications. In addition, a satisfaction survey was conducted to assess the surgical outcome. Postoperative follow-up ranged from 6 months to 4 years. The technique demonstrated significant functional improvement: the mean MRD1 increased from 0.30±0.92 mm to 3.48±0.51 mm, and the mean PFH increased from 5.06±1.00 mm to 9.15±0.76 mm (P<0.05). The overall satisfaction rate was 90.5%. Complications were rare, limited to a single case of conjunctival prolapse. The palpebral margin incision technique effectively corrects congenital ptosis and widens the palpebral fissure, while concealing the scar within the lash line. It represents a reliable alternative for patients prioritizing the preservation of natural single-eyelid morphology.

Early Expanded Polytetrafluoroethylene Sheet Removal Due to Postoperative Infection in Frontalis Suspension Surgery Preserves Eyelid Elevation and Curvature.

In frontalis muscle suspension surgery for treating congenital ptosis, the use of expanded polytetrafluoroethylene (ePTFE) sheets often yields stable postoperative outcomes; however, it also carries the risks of foreign body reactions and infection, with few reports describing the detailed course of treatment in such cases. A 15-year-old girl was referred for management of right congenital ptosis. Preoperative margin reflex distance-1 (MRD-1) measurements were 1.5 mm on the right and 4.5 mm on the left, with corresponding levator function of 5 mm and 14 mm on the right and left, respectively. Levator advancement was considered insufficient to correct the asymmetry; therefore, a frontalis muscle suspension using an ePTFE sheet was planned. After positioning the sheet and forming double eyelids, MRD-1 was confirmed to be symmetrical bilaterally (4.5 mm), the curvature was appropriate, and the incision was closed. At the two-week postoperative suture removal, a small portion of the ePTFE sheet was visible through a gap in the wound at the eyelid margin; therefore, it was trimmed, and wound closure was subsequently confirmed. Six weeks after surgery, the patient presented with fever, eyelid redness, and pain. Infection was noted at the eyelid margin wound site; therefore, the ePTFE sheet was removed, followed by wound irrigation. After prompt ePTFE sheet removal, adequate eyelid elevation and curvature were equivalent to those prior to sheet removal, making this protocol a viable treatment option for similar cases.

Comparing Minimally Invasive Modified Levator vs. Conjoint Fascia Sheath-Levator Suspension in Correcting Severe Congenital Ptosis.

To analyze the clinical efficacy and differences between minimally invasive modified levator palpebrae complex and conjoint fascia sheath-levator muscle complex suspension surgeries in correcting severe congenital ptosis. This retrospective analysis included 40 patients with severe congenital ptosis, of whom 22 (25 eyes) and 18 (21 eyes) underwent minimally invasive modified levator palpebrae complex and conjoint fascia sheath-levator muscle complex suspension, respectively. Palpebral fissure height, marginal reflex distance 1, scleral exposure, and eyelid mobility at 1 week and 1 and 3 months postoperatively were compared between the surgical groups. The overall correction rates did not differ significantly between the minimally invasive modified levator palpebrae complex and conjoint fascia sheath-levator muscle complex suspension surgeries (88% vs. 66.67%). At 1 month postoperatively, the average scleral exposure was significantly lower in the modified levator palpebrae complex group than in the conjoint fascia sheath-levator muscle complex group (1.28 ± 1.29 mm vs. 2.17 ± 0.96 mm). Additionally, at both 1 and 3 months postoperatively, eyelid mobility was significantly greater in the modified levator palpebrae complex group than that observed in the conjoint fascia sheath-levator muscle complex group (6.84 ± 1.16 mm and 7.08 ± 1.18 mm vs. 5.54 ± 1.43 mm and 5.91 ± 1.69 mm). Both minimally invasive modified levator palpebrae complex and conjoint fascia sheath-levator muscle complex suspension surgeries effectively correct severe congenital ptosis; however, the former demonstrates superior eyelid mobility and closure function recovery as postoperative outcomes. Monocular elevation deficiency associated ptosis misdiagnosed as simple congenital ptosis: management with Müller Muscle-Conjunctival Resection.

Purpose: Monocular elevation deficiency (MED), formerly termed double elevator palsy, is a rare ocular motility disorder characterized by restriction of elevation in all gaze positions. Ptosis frequently accompanies MED but may be mistaken for simple congenital ptosis, potentially resulting in suboptimal surgical planning. While frontalis sling and levator resection are the most frequently employed techniques for MED-associated ptosis, these procedures carry a considerable risk of lagophthalmos and suboptimal cosmetic outcomes. To date, the use of Müller muscle - conjunctival resection (MMCR) in this context has not been described. Methods: We report the case of a 14-year-old girl initially diagnosed with congenital ptosis who was subsequently found to have MED. Ophthalmic examination revealed marked elevation deficit and mild hypotropia in the right eye, consistent with MED. As strabismus surgery was declined, ptosis correction was prioritized. Preoperative margin-reflex distance (MRD) measured 1.5 mm on the right and 4 mm on the left. A phenylephrine test demonstrated a partial positive response, increasing MRD to 2.5 mm. Considering the poor Bell's phenomenon and risk of exposure keratopathy, a 9-mm MMCR was performed under general anesthesia. Results: The surgery was completed without intraoperative or postoperative complications. At the one-month follow-up, eyelid contour and height were satisfactory, with the right MRD improved and symmetrical to the fellow eye. No lagophthalmos, corneal exposure, or epithelial keratopathy were observed. Functional and cosmetic outcomes were favorable, and the results remained stable at the 6-month and 1-year follow-up visits. Conclusion: This case highlights the diagnostic challenge of distinguishing MED-associated ptosis from isolated congenital ptosis and underscores the importance of comprehensive motility assessment. Traditional procedures for ptosis in MED, such as frontalis sling or levator resection, may predispose to lagophthalmos, particularly in patients with poor Bell's phenomenon. In contrast, MMCR offers a minimally invasive posterior approach that preserves the anterior lamella and reduces the risk of exposure keratopathy. To our knowledge, this is the first reported pediatric MED case successfully managed with MMCR. The procedure provided stable functional and aesthetic results, suggesting that MMCR may represent a safe and effective alternative in selected patients with MED when strabismus surgery is deferred.
