An immunologic disorder characterized by increased susceptibility to severe viral infections, including herpes simplex virus (HSV), varicella zoster virus (VZV), influenza A virus (IAV), hantavirus, and possibly respiratory syncytial virus (RSV). IMD83 clinical manifestations include acute infection-induced encephalitis and pneumonitis. The susceptibility to encephalitis or pneumonitis appears to result from impaired TLR3-dependent interferon production by nonhematopoietic cells that reside within the central nervous system or lung epithelial cells. IMD83 transmission pattern is consistent with autosomal dominant or autosomal recessive inheritance with incomplete penetrance.

CSF Leukocytes in Intracranial Infections: Diagnostic, Mechanistic, and Therapeutic Insights for Neurocritical Care.
Intracranial infections remain a major neurocritical care challenge, causing high mortality and long-term deficits despite advances in antimicrobials, imaging, and intensive care. Because bacterial, viral, fungal, and parasitic pathogens trigger distinct immune pathways, they produce characteristic CSF leukocyte patterns shaped by pathogen sensing, endothelial adhesion molecule expression, chemokine gradients, and leukocyte migration across CNS barriers. Pleocytosis therefore reflects PRR activation and tends toward neutrophil, lymphocyte, or monocyte-macrophage predominance. Notably, promptly distinguishing infectious encephalitis (IE) from autoimmune encephalitis (AE) is crucial. Specifically, bacterial infections usually create neutrophil-predominant CSF via IL-1β, TNF-α, and G-CSF, whereas viral infections such as HSV and VZV promote lymphocytic profiles through interferons and CXCR3 ligands. Fungal pathogens (Cryptococcus, Candida, Aspergillus) and mycobacteria often produce mixed or mononuclear pleocytosis due to chronic antigen exposure. These patterns guide therapy: bacterial infections require immediate empiric antibiotics with corticosteroids; HSV and VZV encephalitis needs urgent IV acyclovir; enteroviruses and arboviruses rely on supportive care; and fungal infections require staged antifungal therapy with monitoring of CNS penetration, organ function, and intracranial pressure. Parasitic and amoebic infections such as Naegleria fowleri demand individualized antiparasitic treatment, corticosteroids, seizure control, and intensive supportive care. Because CSF leukocytes also influence barrier integrity, neuronal survival, and glial activation, advanced profiling-combining CSF leukocyte phenotyping with cytokine and chemokine analysis-enhances differentiation between IE and AE and supports more targeted treatment. This review elucidates the role of CSF leukocytes in CNS infections, highlighting their diagnostic, mechanistic, and therapeutic significance for guiding precision neurocritical care. Pathogens in CSF create distinct leukocyte patterns via PRR-PAMP signaling and chemokine-driven trafficking. Bacteria cause neutrophil-rich IL-1β/TNF-α responses; viruses (HSV/VZV) produce lymphocyte-dominant interferon profiles; and fungal or mycobacterial infections show mixed or granulomatous patterns. These signatures affect BBB integrity and help distinguish infections from autoimmune encephalitis.
Characteristics of and risk factors for epilepsy after autoimmune and infectious encephalitis.
Encephalitis is associated with significant morbidity including epilepsy. Data on post-encephalitic epilepsy (PEE) incidence and optimal management remain scarce. We prospectively investigated the frequency, characteristics, and risk factors of PEE after autoimmune encephalitis (AE) and infectious encephalitis (IE) in three Austrian tertiary care hospitals over ten years. We included patients ≥ 18 years diagnosed according to internationally recognized encephalitis algorithms or verification of well-characterized antineuronal autoantibodies (AE) or pathogens (IE). Epilepsy diagnosis followed International League Against Epilepsy criteria. All available clinical data were collected from electronic patient files. A prospective follow-up was conducted via structured interviews and clinical visits. Statistical analyses included Kaplan-Meier plots, correlation analyses, and regression models. Of 149 patients, 26 (17%) had AE, 108 (73%) IE, and 15 (10%) encephalitis of unknown etiology. Prospective follow-up was achieved in 74% of cases. Median follow-up was 2304 days (IQR 1433; 3274). Acute symptomatic seizures (ASS), acute symptomatic status epilepticus and PEE are significantly more frequent in AE than in IE (p < 0.001). Antiseizure medication (ASM) use persisted in 85% of AE and 30% of IE patients at last follow-up, despite only 12% meeting PEE criteria. In both types of encephalitis, we see a shift from focal-to-bilateral semiology. Difficult-to-treat seizures during initial hospitalization correlated with PEE risk (rho = 0.317; p < 0.001). Our study provides insights into PEE characteristics, management, and prognosis. It highlights the prevalence of ASM overuse in the wake of encephalitis and provides a prognostic tool assessing the individual PEE risk. Relapsing granulomatous amoebic encephalitis.
Free-living amoebae like Naegleria fowleri, Acanthamoeba or Balamuthia mandrillaris are rare causes of central nervous system infections. Naegleria fowleri causes primary amoebic meningoencephalitis, whereas the latter two cause granulomatous amoebic encephalitis, which usually occurs in immunocompromised people and is almost universal fatal despite treatment. Early diagnosis is challenging due to the non-specific neurological symptoms, and there are no specific treatment guidelines. We report a case of successfully treated relapsing Acanthamoeba encephalitis in an immunocompetent individual.
Comparative Analysis of Clinical and Paraclinical Features in Pediatric Autoimmune Versus Infectious Encephalitis.
Herpes simplex virus type 2 encephalitis in critically ill adults: A retrospective analysis of EURECA and Herpetics cohort studies.
Herpes simplex virus type 1 (HSV-1) is the leading cause of sporadic infectious encephalitis. Although typically associated with benign meningitis, HSV-2 can also cause encephalitis in adults, a condition that remains poorly characterized in intensive care unit (ICU). We aimed to compare the characteristics, management, and outcomes of critically ill adult with HSV-2 and HSV-1 encephalitis. We performed a retrospective analysis of adults with herpes simplex encephalitis (HSE) admitted to the ICU in two large multicenter cohorts. Patients were classified according to cerebrospinal fluid (CSF) HSV genotype. The primary endpoint was unfavourable outcome at 3 months, defined as a modified Rankin Scale score 3–6. Among 285 HSE patients, 17 (6%) had HSV-2 encephalitis (age 60 [53–67] years, female sex 58.8%). Although the proportion of immunocompromised status did not differ significantly between groups (29.4% vs 15.4%, p = 0.13), HSV-2 cases had a significantly higher prevalence of HIV infection (17.6 vs 1.5%, p < 0.01), less frequent brain MRI abnormalities (85.7% vs 97.7%, p = 0.01) and similar CSF findings compared to HSV-1 cases. The duration of intravenous acyclovir treatment was shorter for HSV-2 cases (18 [4–21] days) versus 21[20–21] days, p = 0.04). Unfavourable outcome occurred in 9/16 (56.3%) patients with HSV-2 encephalitis and in 175/236 (74.2%) patients with HSV-1 encephalitis (p = 0.25). Encephalitis caused by HSV-2 represent a consistent proportion of severe HSE. Clinical presentation and outcomes and broadly comparable to those of HSV-1 cases, underscoring the importance of early recognition and standardized management of encephalitis caused by HSV, regardless of genotype.
