The Fontan Outcomes Network: Findings After 2 Years and 1121 Participants.
Survival after Fontan palliation for single ventricle heart disease has improved substantially, yet the long-term trajectory remains poorly defined. The Fontan Outcomes Network, a learning health network of 38 congenital heart centers in the United States and Canada, was established to address this gap. We report baseline characteristics and early findings from the first 1121 participants enrolled in this prospective clinical registry. We performed a cross-sectional analysis of individuals who had undergone Fontan palliation enrolled in the Fontan Outcomes Network from August 2022 through August 2024. Demographic, clinical, imaging, procedural, and medication data were analyzed descriptively, overall and by age group (<12, 12 to <18, and ≥18 years). A total of 1121 participants were enrolled (mean age, 16.3±10.2 years; 471 [42%] female). Hypoplastic left heart syndrome (n=431 [38.5%]) and right ventricular-dominant anatomy (n=615 [54.9%]) were the most common primary cardiac diagnoses, especially in younger age groups. Extracardiac conduit was the most frequent Fontan type (n=749 [66.8%]). Typical Fontan-related adverse events were noted in 59% of cases (n=662), including chylous pleural effusions (n=108 [9.6%]), Fontan thrombosis (n=79 [7.1%]), and nonperioperative stroke (n=73 [6.5%]). A history of arrhythmia was present in 41.3% of patients (n=463) overall, increasing with age. Acquired comorbidities were present in 57.5% of patients (n=645), including asthma (n=132 [12%]) and sleep apnea (n=103 [9%]), with burden increasing by age. Antithrombotic medication use was nearly universal. Use of other medications varied widely by age, and included β-blockers, angiotensin-converting enzyme inhibitors, and pulmonary vasodilators. A clinical diagnosis of an anxiety disorder was present in 34.1% of patients (n=382), including 48.1% of adults (n=194 of 403), and antidepressants were prescribed in 11.8% of patients (n=132), including 23.6% of adults (n=95 of 403), highlighting the mental health needs of this population. The Fontan Outcomes Network includes one of the largest prospective, multi-institutional data sets of patients with Fontan circulation. These initial findings demonstrate the potential for a collaborative learning health network to advance research and quality improvement for this rare disease population.
Diagnosis and Management of Overlap Syndrome (COPD-OSA): Evidence, Challenges and Prospects.
The overlap syndrome (OVS), characterized by the co-existence of obstructive sleep apnea (OSA) and chronic obstructive pulmonary disease (COPD), presents a significant clinical challenge due to its association with serious cardiopulmonary consequences and increased mortality. The complex pathophysiology and absence of standardized treatments often lead to underdiagnosis, aggravating its disease burden. Therefore, early and precise diagnosis is crucial. Beyond monitoring carbon dioxide levels as a key indicator of ventilatory limitation, a comprehensive diagnostic approach should integrate subjective assessments and objective examinations. Advances in precision medicine are shifting research focus towards biomarkers, including systemic inflammation ratios and vascular injury markers, to facilitate early detection and risk stratification. In terms of treatment, non-invasive ventilation (NIV) remains the cornerstone for maintaining airway patency, though its effectiveness is constrained by suboptimal adherence and inherent limitations. This has prompted the exploration of pharmacological strategies as alternatives or adjuncts, yet their application requires careful risk-benefit evaluation. For instance, benzodiazepines may worsen upper airway muscle tone and blunt hypercapnic arousal responses. Promising directions include novel pharmacotherapies inspired by recent advances in OSA treatment, alongside weight-loss agents that act through weight management and metabolic regulation. Adjunctive strategies such as mild intermittent hypoxia (IH) conditioning are being investigated to enhance positive airway pressure (PAP) therapy, while targeted agents like thioredoxin (TRX) demonstrate potential by mitigating oxidative stress and neutrophilic inflammation. However, the clinical translation of these innovations faces a major bottleneck the scarcity of large-scale randomized controlled trials (RCTs) to confirm their efficacy and safety. This review synthesizes current evidence to address these gaps and inform the future precision management of OVS.
Lower fat-free mass is independently linked to restless legs syndrome in men: a cross-sectional PSG-BIA study.
Restless legs syndrome (RLS) is a common sensorimotor disorder that disrupts sleep and quality of life. Sarcopenia-reduced skeletal muscle mass and function-has been linked to sleep disturbances, but its relationship with RLS remains unclear. We examined whether sarcopenia is associated with RLS, with a focus on sex-specific effects. We conducted a cross-sectional analysis of 5,752 adults who underwent both type-I polysomnography (PSG) and bioelectrical impedance analysis (BIA) at a tertiary sleep center. RLS was diagnosed by IRLSSG criteria. Sarcopenia was defined using skeletal muscle index (SMI) and fat-free mass index (FFMI) thresholds. Multivariable models adjusted for age, physical activity, caffeine/alcohol intake, and apnea-hypopnea index (AHI). RLS prevalence was 6.6% in females and 2.9% in males. Sarcopenia was more frequent in the RLS group than in non-RLS (10.6 vs. 6.8%), particularly among males (8.7 vs. 3.2%). In males, lower SMI and FFMI were independently associated with higher odds of RLS; sex interaction for FFMI was significant. Reduced muscle mass is independently associated with RLS in men, suggesting a male-specific muscle phenotype relevant to RLS pathophysiology. Incorporating BIA-based screening and muscle-preserving interventions may benefit the management of male patients with RLS.
Comparison of craniofacial skeletal morphology in pediatric obstructive sleep apnea-hypopnea syndrome patients with class II and class III malocclusions: a retrospective cross-sectional study.
This study aimed to investigate craniofacial skeletal morphology in pediatric obstructive sleep apnea-hypopnea syndrome (OSAHS) patients with different malocclusion types. A retrospective cross-sectional analysis study was conducted at Chengdu Second People's Hospital from 2023 to 2024. A total of 299 children diagnosed with OSAHS (aged 10-12 years) were included. Craniofacial structures were assessed using Jarabak and Ricketts methods. Patients were divided by malocclusion type: Class II (n = 150, 56.7% male, mean age 11.2 ± 0.6) and Class III (n = 149, 50.3% male, mean age 11.4 ± 0.8). Group differences in cephalometric parameters were compared using independent samples t-tests and Mann-Whitney U tests, as appropriate. Significant differences were found between groups. Compared to Class II, Class III patients had lower ANB angle (-2.8 ± 2.6° vs. 5.6 ± 1.9°, Cohen's d = 1.23), Wits appraisal (-2.9 ± 3.7 mm vs. 2.6 ± 0.7 mm, Cohen's d = 2.02), SN:GoMe (103.1 ± 6.0% vs. 106.7 ± 9.0%, Cohen's d = 0.48), MP/FH (32.9 ± 2.99° vs. 37.3 ± 2.65°, Cohen's d = 1.57), Xi-Pm/DC-Xi (29.3 ± 2.1° vs. 24.6 ± 3.1°, Cohen's d = 1.81), and ANS-Xi-Pm (47.8 ± 2.9° vs. 50.7 ± 2.4°, Cohen's d = 1.11) (all P < 0.05). Class III patients showed higher S-Ar:Ar-Go (75.9 ± 7.2% vs. 80.9 ± 13.0%, Cohen's d = 0.48), NP/FH (88.1 ± 2.9° vs. 83.1 ± 1.86°, Cohen's d = 2.14), Pt-Gn/Ba-N (92.2 ± 2.1° vs. 79.6 ± 2.4°, Cohen's d = 5.44), and Hy-C3 distance (5.8 ± 0.9 mm vs. 3.4 ± 0.4 mm, Cohen's d = 3.26) (all P < 0.05). No significant differences were observed in other parameters (N-S-Ar, S-Ar-Go; P > 0.05). Distinct craniofacial skeletal patterns exist in pediatric OSAHS patients with different malocclusions. Class III patients demonstrate mandibular growth restriction with compensatory protrusion, while Class II patients display high-angle, long-face morphology with clockwise growth rotation. These findings have important clinical implications for individualized orthodontic and surgical planning in the management of pediatric OSAHS, highlighting the need for early assessment of craniofacial structure in affected children.
Understanding the impact of growth hormone on ventilatory control stability in children with Prader-Willi syndrome.
A hallmark of Prader-Willi syndrome (PWS) is hypothalamic-pituitary axis dysfunction, which can result in reduced growth hormone (GH) production. While GH replacement therapy is common in children with PWS, it has also been implicated in the development of obstructive sleep apnoea (OSA) in some children. The mechanisms underlying this development are poorly understood but may be related to alterations in ventilatory control. Our study investigated the impact of GH treatment on ventilatory control stability during sleep in children with PWS. Polysomnographic data pre- and post-GH therapy in 25 children (aged 2mo-18y) were used to assess ventilatory control using a validated method that estimates loop gain (dimensionless ratio) from ventilation changes following spontaneous sighs during sleep. Data were analysed using linear mixed-effects modelling with GH as a fixed effect and participant as a random intercept. Covariates that could impact loop gain including age, obstructive-apnoea hypopnoea index (OAHI) and central apnoea-hypopnoea index (CAHI) were each added separately to the base model in a stepwise, manual forward selection approach. Loop gain was not altered by GH treatment (β = 0.003, 95% CI: [-0.042, 0.049], p = 0.878, Cohen's d = 0.031). Age, OAHI and CAHI did not alter the impact of GH on loop gain. No difference in sleep or respiratory characteristics were found, however 20% of children developed OSA post-GH. Initiation of GH therapy was not associated with a change in loop gain, suggesting that changes in ventilatory control are unlikely to contribute to the development of OSA in children with PWS. • Prader-Willi syndrome is associated with abnormal ventilatory control and increased risk of sleep-disordered breathing. • Growth hormone therapy may influence respiratory physiology but its effect on the stability of ventilatory control (loop gain) remains unclear. • In this cohort of children with Prader-Willi syndrome, growth hormone therapy did not alter loop gain despite inter-individual variability. • Our findings suggest that any sleep-disordered breathing that emerges following growth hormone therapy is likely driven by mechanisms other than altered loop gain.
