Familial Cases of Legg-Calvé-Perthes Disease-Hemostatic and Molecular Markers.
Legg-Calvé-Perthes disease (LCPD) is a rare disease caused by avascular necrosis of the femoral head. Although its etiology is still not fully understood, evidence suggests that heritable prothrombotic and inflammatory factors, as well as environmental factors, may be implicated in its onset and progress. The objective of this study is to describe the genetic, biochemical, and environmental factors that may be associated with the etiology of LCPD. This study was conducted in three families and included seven related patients with an LCPD diagnosis. We evaluated the following gene alterations using real-time PCR: MTHFR, CBS, COL1A1, COL2A1, PT, FVL, FVIII, FIX, PAI-1, eNOS, IL-23R, TNF-α, RANNK, RANNK-L, OPG and IL-6. Additionally, we assessed fourteen thrombophilia-associated biochemical markers, as well as environmental factors that may be associated with the etiology of LCPD in family cases. The results show different hemostatic alterations in every individual analyzed, presenting out-of-range values in one or more parameters. Concentrations of hemoglobin and fibrinogen and the FIX activity percentage showed statistically significant differences (p < 0.001) when compared with healthy controls. All patients presented at least one mutated allele for the MTFHR (rs1801133), IL-23R (rs1569922) and OPG (rs2073618) polymorphisms, as well as isolated cases with other genetic variants. Our results show environmental elements from every family, and hemostatic and inflammatory disorders, may be involved in the development of LCPD. Furthermore, genetic variants could contribute to the onset of the disease. This study highlights the multifactorial nature of this pathology, involving various environmental, genetic, inflammatory, and prothrombotic factors in three families that included seven patients diagnosed with LCPD.
From growing pains to growing evidence: a 2025 update on novel insights in lower limb osteochondroses.
Osteochondrosis encompasses a heterogenous group of pathologies affecting endochondral ossification in the growing child and adolescent. The cause of each disease is multifactorial, though many are often related to overuse injury, and may be epiphyseal, physeal, or apophyseal. Identification and treatment of this group of disorders is complex, thus this review aims to briefly describe common pathologies, their management, and highlight novel developments within the field. Machine learning as well as advanced diagnostic tools for more precise evaluation and prognostication of osteochondroses have been studied including perfusion MRI in Legg-Calvé-Perthes disease. Novel treatments include leukocyte-rich platelet-rich plasma (LR-PRP), which offer promising improvements in pain and function in Osgood-Schlatter disease. Surgical technique studies have begun to examine optimal operative management of Freiberg's disease. The osteochondroses are an often-self-limiting spectrum of pathologies affecting the physis in children and adolescents that may be managed with conservative treatment, though some require surgical intervention. Advances in imaging, prognostication tools, and treatment modalities support earlier and accurate diagnoses, as well as better informed treatment decisions.
Etiology and clinical trends in hip osteoarthritis in Japan: Insights from a multicenter cross-sectional study.
Epidemiological characteristics of Hip osteoarthritis (OA) continue to change due to population aging and emerging disease concepts, such as femoroacetabular impingement (FAI) and subchondral insufficiency fracture of the femoral head (SIF). This study aimed to analyze the current etiologies of hip OA in Japan and identify characteristics associated with each etiology. A multicenter cross-sectional study was conducted in 2022 at 12 major Japanese hospitals, including 1197 patients (1515 hips) newly diagnosed with hip OA. Etiologies were classified into nine categories based on patient background and imaging findings: hip dysplasia, primary OA, FAI, SIF, rapidly destructive coxopathy (RDC), trauma, Legg-Calvé-Perthes disease (LCPD), slipped capital femoral epiphysis (SCFE), and skeletal dysplasia. The etiologies included hip dysplasia (74.4 %), primary OA (13.4 %), FAI (4.8 %), SIF (3.4 %), RDC (1.3 %), trauma (1.3 %), LCPD (0.9 %), SCFE (0.1 %), and skeletal dysplasia (0.1 %). Patients in their 70s comprised the largest group, with a predominance of female patients across all ages. The proportion of hip dysplasia decreased in the elderly patients, whereas primary OA and SIF increased. Hip dysplasia was more common in female than male (78.2 % vs. 55.2 %), whereas FAI was more frequent in male (17.9 % vs. 2.2 %). The Japanese Orthopaedic Association hip scores were highest for FAI and lowest for SIF. Kellgren-Lawrence (KL) grade ≥3 was more common in primary OA (82.0 %) and hip dysplasia (76.7 %) than in FAI (57.7 %) and SIF (66.7 %). Hip dysplasia remains the predominant cause of hip OA in Japan. Age-specific analyses revealed that FAI was more common in younger patients, while primary OA and SIF were prevalent in older patients. Patients with hip dysplasia and primary OA often presented with advanced KL grades, while SIF was associated with the poorest clinical scores. These findings may inform diagnostic and treatment approaches based on predominant etiologies.
[Legg-Calvé-Perthes disease in early childhood: conservative versus surgical management : A risk-adapted containment approach].
Early-onset Legg-Calvé-Perthes disease (LCPD) carries a favorable prognosis in the majority of cases; key determinants of outcome are adequate containment (acetabular coverage) of the femoral head and preservation of range of motion during the initial Waldenström stages. We present practical classifications and a treatment strategy that prioritizes conservative management and focuses operative interventions on "hips at risk." In children younger than approximately 6 years of age, treatment includes activity modification and dedicated ROM therapy. "Hip-at-risk" signs-particularly progressive decentering of the femoral head-identify the subgroup that benefits from early containment surgery. Proximal femoral varus osteotomy is technically straightforward and widely established; its limitations include limb-length discrepancy and potential worsening of an existing abductor weakness; in childhood, pelvic Tönnis triple osteotomy addresses containment most comprehensively in LCPD and, in experienced hands, can be performed with low morbidity and early mobilization. As a general principle, the procedure should be selected that most reliably restores containment for the individual hip and with which the treating center has the greatest expertise. Centralization of care would be desirable to generate robust long-term data and guidelines for this-perhaps surprisingly-insufficiently standardized condition. HINTERGRUND: Der frühkindliche Morbus Perthes weist in der Mehrzahl der Fälle eine günstige Prognose auf. Entscheidend für den Verlauf sind ein ausreichendes Containment (Überdachung) des Hüftkopfes sowie der Erhalt der Beweglichkeit in den initialen Waldenström-Phasen. Wir stellen praxisrelevante Einteilungen sowie eine Entscheidungshilfe dar, der ein konservatives Vorgehen priorisiert und operative Maßnahmen auf „Hip-at-risk“-Hüften fokussiert. Bei Kindern unter etwa 6 Jahren gelingt die Behandlung häufig konservativ (Aktivitätssteuerung, konsequentes ROM-Training). „Hip-at-risk“-Zeichen, insbesondere die progrediente Dezentrierung, identifizieren die Gruppe, die von einer frühzeitigen Containment-Operation profitiert. Die proximale Femur-Varisationsosteotomie ist technisch einfach und breit etabliert, Limitationen sind die Beinlängenverkürzung und die potenzielle Verstärkung einer Abduktionseinschränkung. Die Tönnis-Tripleosteotomie des Beckens adressiert im Kindesalter das Containment am umfassendsten und kann in erfahrenen Händen komplikationsarm mit rascher Mobilisation erfolgen. Generell sollte das Verfahren gewählt werden, das für die jeweilige Hüfte das Containment am zuverlässigsten wiederherstellt und mit dem die versorgende Klinik am besten vertraut ist. Eine Zentralisierung der Behandlung wäre insgesamt wünschenswert, um langfristig valide Daten und Leitlinien für diese – überraschenderweise wenig randomisiert-kontrolliert untersuchte – Erkrankung zu generieren.
Severe Radiographic Legg-Calvé-Perthes Disease With Favorable Long-Term Functional Outcomes Following Conservative Management: A Case Report.
Legg-Calvé-Perthes disease (LCPD) is a pediatric disorder characterized by avascular necrosis of the femoral head, with prognosis largely determined by the extent of epiphyseal involvement. Severe radiographic classifications are typically associated with poor long-term outcomes and often prompt surgical intervention. We present a case of a male patient diagnosed with unilateral LCPD at age three with extensive femoral head involvement by age six (Catterall Group 4, Salter-Thompson Group B, Herring Group C) who was managed non-operatively over two decades. Treatment consisted of physiotherapy, activity modification, and pain management without surgical intervention. Despite marked femoral head deformity and residual limitations in hip range of motion, the patient achieved high functional capacity, including participation in National Collegiate Athletic Association (NCAA) Division I athletics, and he maintains independence in activities of daily living as a young adult. This case highlights the potential for favorable functional outcomes with conservative management in carefully selected patients with poor-prognosis LCPD and underscores the importance of individualized treatment planning.
