Clinical characteristics and results after conservative treatment or interfascicular neurolysis of 58 limbs with spontaneous posterior interosseous nerve palsy: A prospective Japanese multicenter study.
Spontaneous posterior interosseous nerve (PIN) palsy is a rare condition with controversial clinical features. Although this palsy is conventionally treated conservatively, interfascicular neurolysis to release hourglass-like fascicular constrictions has also been recommended. This study aimed to clarify the clinical characteristics and treatment guidelines for this palsy. Fifty-eight limbs with spontaneous PIN palsy receiving conservative treatment or interfascicular neurolysis. The presence of pain as a premonitory symptom and the degree of muscle weakness were investigated. Patients were followed periodically from 3 months after palsy onset to either recovery or ≥36 months afterwards. Limbs recovering to manual muscle testing (MMT) grade 4 or better for both the extensor pollicis longus and extensor digitorum were rated as Good recovery, with all others judged as Poor. Good recovery was achieved in 31 of 34 limbs treated conservatively and 19 of 24 of limbs by interfascicular neurolysis. Younger age at onset was the factor significantly associated with achieving Good recovery in 58 limbs treated by conservatively or interfascicular neurolysis. In the 24 limbs treated by interfascicular neurolysis, early timing of surgery was the factor significantly associated with attaining Good recovery. All 27 limbs with conservative treatment displaying ≥1 grade of MMT improvement within 6 months after onset achieved Good recovery. In the 23 limbs with Poor recovery at 6 months by conservative treatment, Good recovery was attained in 4 of 7 limbs by continuing conservative treatment and in 12 of 16 limbs by subsequent interfascicular neurolysis. Although the latter treatment tended to produce better results, no significant difference was detected. Diverse clinical characteristics of spontaneous PIN palsy are useful for diagnosis. Conservative treatment is advisable within 6 months after onset of palsy. If no improvement is observed, interfascicular neurolysis without delay is a reasonable option.
Clinical characteristics of hospitalized patients with hepatitis E virus infection in Poland: a multi-centre retrospective study.
The burden of hepatitis E virus (HEV) infections in Poland is highly underestimated and in contrast with high anti-HEV IgG seroprevalence in Polish population. This is the first publication addressing the issues of HEV infections in Poland as well as surveillance and reporting problems referring to this particular disease. In this retrospective study we analysed 61 HEV cases referred to and diagnosed in 10 leading centres in Poland. Males constituted 70.5% of the study group, with equal distribution of cases below and over 50 years of age. Comorbidities were reported in 73.8% of this group. Of major concern was the high frequency of reported complications and clinical outcomes: 3 cases of acute liver failure (ALF) with 1 resulting in orthotopic liver transplantation (OLTx), 1 case of renal insufficiency, 3 cases of neuralgic amyotrophy, 8 cases of thromocytopaenia, and 4 cases of chronic hepatitis E. The HEV infections in Poland are considerably underestimated, which results both from the lack of awareness of general practitioners and limited access to HEV diagnostics, particularly in primary and secondary care medical facilities in Poland.
Isolated phrenic neuropathy in neuralgic amyotrophy.
Pre-Plexal Extension of Parsonage-Turner Syndrome With Nerve Root Involvement on Needle Electromyography.
Parsonage-Turner syndrome (PTS) is an inflammatory, immune-mediated neurogenic condition characterized by acute-onset, severe unilateral shoulder/upper arm pain followed by patchy weakness and atrophy. Often described as an idiopathic brachial plexitis, most cases of PTS involve the brachial plexus or its terminal branches, and root-level or paraspinal muscle involvement is rare. Among the differential diagnoses for proximal neuropathic root involvement, cervical radiculopathy by mechanical compression of the exiting nerve roots and diabetic cervical radiculoplexus neuropathy by way of immune-mediated microvasculitis, ischemic injury, and subsequent ischemic axonal damage should be considered. The latter also shares similar symptomatology to PTS with progression with subacute, painful, asymmetric involvement of the upper limb followed by atrophy and weakness of the limb. Here, we present a case of PTS with notable findings of cervical paraspinal muscle involvement on needle electromyography, highlighting a rare proximal, pre-plexal extension of this idiopathic inflammatory immune process.
Neuralgic amyotrophy: Incidence, specialty of diagnosing clinician, and delays in treatment.
This study sought to determine the incidence of Neuralgic Amyotrophy (NA) in the United States of America's health system, most common presenting symptoms, time from symptom onset and time from initial presentation to diagnosis, number and specialty of clinicians seen prior to diagnosis, and specialty of diagnosing clinician. A retrospective chart review study was conducted to identify all patients diagnosed with ICD-10 G54.5 from September 1, 2016-December 31, 2023. 153 patients with NA diagnoses from 2016 to 2023 were included. The average incidence of NA in the United States of America's healthcare system was 1.77/100,000 per year. The most common chief concern upon presentation was shoulder pain, (n = 57, 37.2%) and the most common secondary or tertiary concern was shoulder weakness (n = 41, 26.7%). Average time from symptom onset to diagnosis was 101.3 days (SD = 155.4; median = 51 days), and from initial presentation to diagnosis was 78.1 days (SD = 145.6; median = 28 days). Sixty-three patients (41.2%) presented to one other clinician with similar symptoms prior to diagnosis, 37 (24.2%) saw two clinicians, 16 (10.5%) saw three, and 7 (4.6%) saw four or more clinicians. Patients most often initially presented to a family medicine, primary care, or urgent care clinician (n = 61, 39.9%). The most common specialties of diagnosing clinicians were orthopaedic surgery (100 diagnoses, 65.4%), neurology (23 diagnoses, 15%) and neurosurgery (15 diagnoses, 10%). Diagnosis of neuralgic amyotrophy is historically rare, but the incidence may be higher than previously thought. Neuralgic amyotrophy is a debilitating disorder that is often initially unrecognized, ultimately leading to a months-long delay in diagnosis. Over 80% of patients were seen by at least one other clinician before diagnosis, and orthopaedic clinicians were most likely to make the diagnosis. Maintaining a high clinical suspicion while recognizing the common presenting symptoms along with onset and evolution of symptoms would help facilitate timely referral to specialists trained in identifying and managing neuralgic amyotrophy, prevent unnecessary and unproductive appointments preceding the diagnosis, and allow earlier initiation of treatment.
