Atopic Keratoconjunctivitis: From Molecular Mechanisms to Clinical Implications.
Atopic keratoconjunctivitis (AKC) is a chronic, bilateral inflammatory disease of the ocular surface that primarily affects young adults with a history of atopic dermatitis, asthma, or allergic rhinitis. AKC is considered the most severe form of ocular allergy and can result in serious complications that threaten vision, such as corneal ulceration, neovascularization, and scarring. Genetic predisposition, through regulatory genes that impact skin barrier function, immune system regulation, and allergic responses, plays a crucial role in the development and severity of AKC. Recent research has highlighted the roles of epithelial barrier dysfunction, gastrointestinal and eyelid/conjunctival dysbiosis, Th2-driven cytokines, and eosinophilic inflammation in disease perpetuation. Diagnosing AKC requires a high level of suspicion, careful integration of clinical signs, and a thorough assessment of coexisting atopic conditions. Management is often challenging and involves a stepwise approach, beginning with topical antihistamines and mast cell stabilizers, progressing to corticosteroids, calcineurin inhibitors, and, in severe or refractory cases, systemic immunosuppressive therapy or surgical intervention. Novel therapies, including biologics targeting IL-4 and IL-13, show promise in selected patients. A multidisciplinary approach and long-term follow-up are crucial for minimizing complications and preserving visual function. This review provides a comprehensive update on the epidemiology, clinical manifestations, immunopathogenesis, diagnostic approach, and therapeutic strategies for AKC.
Progressive Keratoconus as a Complication of Atopic Keratoconjunctivitis: A Case Report.
Presentation of a case report demonstrating the complexity of care for a patient with atopic keratoconjunctivitis (AKC), including the management of complications such as corneal ulceration, development of keratoconus, corticosteroid-induced intraocular pressure elevation, and cataract formation. We describe a case of a 52-year-old male with a marginal corneal ulcer within the context of atopic blepharokeratoconjunctivitis, focusing on complications and the need for intensive local and systemic therapy to stabilize the disease. Intensive therapy led to a gradual improvement of the disease. Corneal cross-linking stabilized keratoconus, intraocular pressure elevation was successfully managed with anti-glaucoma medication, and cataract surgery resulted in a significant improvement of visual function. This case report highlights the importance of early diagnosis and the need for long-term, multidisciplinary care in patients with AKC. Regular follow-up examinations, patient education, and a combination of local, systemic, and surgical therapy are essential in order to prevent complications and stabilize the disease.
Cytokine Levels and Clinical Characteristics in the Tear Fluid of Patients with Atopic Dermatitis and Atopic Keratoconjunctivitis.
To investigate the relationship between the cytokine levels in the tear fluid and the severity of ocular surface disease in patients with atopic dermatitis (AD) and atopic keratoconjunctivitis (AKC). Tear fluid was collected from 104 AD patients with stratified AKC severity and the healthy controls, who all underwent ophthalmological examination. Cytokine levels in tear fluid were analyzed with Luminex multiplex assays. Increased levels of eotaxin-3, IFN-β, IFN-γ, IL-1β, IL-13, IL-15, IL-17A, IL-2, IL-4, IL-6, IL-8, MCP-4, MIP-1α, MIP-1β, MMP-9, MPIF-1 and MPO were observed in moderate-severe AKC group and IL-6, IL-8 and IFN-γ was associated with AKC severity. The participants with AKC had higher frequencies of atopic comorbidities and the severity of AD was associated with more severe AKC. AKC severity was associated with AD severity, more atopic comorbidities, and increased levels of tear fluid cytokines associated with Th1, -2 and -3 immune response.
Ocular Allergy Within the Framework of the EAACI Nomenclature of Allergic Diseases and Hypersensitivity Reactions.
Ocular allergy encompasses a heterogeneous group of diseases with overlapping clinical features and complex immunopathological mechanisms. This often creates challenges in classification and, consequently, in optimizing patient management. The nomenclature published in 2023 by the European Academy of Allergy and Clinical Immunology (EAACI) addresses these issues by redefining these conditions and linking clinical phenotypes and environmental modifiers to underlying types of hypersensitivity. This framework enhances diagnostic precision and supports mechanism-guided management. This article applies the EAACI approach based on hypersensitivity types to ocular allergy. The examples addressed include seasonal and perennial allergic conjunctivitis, driven mainly by type I and IV hypersensitivity reactions; vernal and atopic keratoconjunctivitis, involving mixed type I, IVb and IVa pathways; giant papillary conjunctivitis, a tissue-driven type V reaction; and contact blepharoconjunctivitis, a type IVa delayed hypersensitivity reaction with additional components. Distinct endotypes-such as local or acute allergic conjunctivitis, dupilumab-induced ocular disease, and vernal keratoconjunctivitis/atopic keratoconjunctivitis overlap-further illustrate heterogeneity, with the ocular surface microbiome emerging as a modifier. Diagnostics are increasingly aligning with mechanisms, and the EAACI framework translates this complexity into a mechanism-indexed map; this supports the selection of responders for targeted interventions while minimizing overtreatment.
A Case of Distinctive Corneal Opacity Following Ciliary Sulcus Ahmed Tube Insertion in a Patient With Glaucoma Associated With Atopic Dermatitis.
Corneal and conjunctival inflammation associated with atopic dermatitis is classified as atopic keratoconjunctivitis, and eye rubbing related to ocular itching is known to cause various ocular complications. We report a case of a Japanese man in his 40s with open-angle glaucoma associated with atopic dermatitis who developed a characteristic corneal opacity after Ahmed Glaucoma Valve (AGV) implantation with ciliary sulcus tube insertion. Despite the absence of persistent anatomical tube-corneal touch on serial examinations, a stellate corneal opacity developed at a site precisely corresponding to the tube tip during a period of exacerbated atopic dermatitis accompanied by frequent eye rubbing. Anterior-segment optical coherence tomography (OCT) consistently demonstrated a measurable distance between the tube and the corneal endothelium. Intermittent tube-corneal contact induced by corneal deformation during eye rubbing was hypothesized to be a plausible contributing factor. This case provides a clinical lesson that corneal damage related to the tube may occur even after sulcus tube insertion in patients with atopic glaucoma, highlighting the importance of careful postoperative corneal monitoring and appropriate management of atopic dermatitis.
