Incidental Subependymoma of the Floor of the Fourth Ventricle: A Case Report.
Subependymomas are rare, slow-growing intraventricular tumors that are typically discovered incidentally and seldom present with transient sensory symptoms in young adults. We report the case of a 22-year-old woman who presented with episodic migratory paresthesias and was found on brain MRI to have a small fourth-ventricular lesion radiographically consistent with subependymoma. The patient's sensory symptoms resolved spontaneously, and no further episodes occurred. Conservative management with serial imaging was pursued. At the six-month follow-up, the lesion remained stable, and the patient was asymptomatic. Subependymomas most commonly become symptomatic due to cerebrospinal fluid obstruction, whereas episodic migratory paresthesias are unusual. This case highlights the value of early MRI in young adults with atypical sensory symptoms, which may reveal incidental yet clinically significant findings and help avoid unnecessary invasive testing. Small, non-obstructive fourth-ventricle subependymomas can be safely managed with observation, demonstrating excellent short-term clinical and radiographic outcomes.
Two pediatric supratentorial ependymal tumors with novel PLAG1 fusions.
The 2021 World Health Organization (WHO) Classification of Central Nervous System (CNS) classification formalized routine molecular profiling, and DNA-methylation studies have since delineated PLAG-family-altered CNS entities: PLAGL1 fusion-positive supratentorial neuroepithelial tumors (NET_PLAGL1) and embryonal tumors with PLAGL1/PLAGL2 amplification. PLAG1 fusions with diverse partners have been reported in CNS embryonal tumors, but have not been described in supratentorial neuroepithelial tumors to date. We describe two pediatric supratentorial ependymal tumors with novel PLAG1 fusions that do not match any 2021 WHO-defined entity or any PLAG-family-related entity recently reported in the literature. Case 1 (4-year-old boy) had a 7.6-cm left lateral ventricular mass with edema and heterogeneous enhancement; gross total resection was performed without adjuvant therapy (alive at 8 months). Histology showed diffusely low cellularity with small- to medium-sized round nuclei, minimal atypia, and focal calcifications; no ependymal rosettes, branching vessels, or clear-cell change. Tumor cells were positive for GFAP and H3K27me3, and negative for Desmin, EMA, OLIG2, L1CAM, NF-κB and H3K27M. The MIB-1 labeling index was ~ 5%. RNA-seq identified TNC::PLAG1 fusion; FISH showed PLAG1 rearrangement. DNA methylation clustered with spinal subependymoma, despite supratentorial location. Case 2 (4-year-old girl) had a 1.4 × 1.1 cm left parahippocampal lesion; resected without adjuvant therapy (alive at 4 months). Histology showed low-to-moderate cellularity with diffuse microcystic change, focal clear/vacuolated cells, and delicate branching vessels. Tumor cells were positive for GFAP, EMA, L1CAM, H3K27me3 and ATRX, and negative for EMA, Desmin, NF-κB, OLIG2, H3K27M, and CD34 (MIB-1 ~ 2%). RNA-seq identified TXNIP::PLAG1 fusion. Methylation did not reach a class threshold. Both cases ultimately warrant a final diagnosis of ependymal tumor, not elsewhere classified. To our knowledge, TNC::PLAG1 and TXNIP::PLAG1 are first-ever fusions reported in any tumor type. They also represent the first PLAG1 fusions identified in pediatric supratentorial ependymal tumors. These cases highlight the value of integrating histology, methylation profiling, and fusion detection, and suggest a new candidate supratentorial ependymal subtype with PLAG1 fusions, pending validation in larger series.
Ependymal Tumors.
The 2021 WHO Brain Tumor classification update grouped ependymal tumors into 3 anatomic locations-supratentorial, posterior fossa, and spinal. Supratentorial ependymomas now include ZFTA-fusion tumors which are often cystic, hemorrhagic, and show marked edema and YAP1-fusion tumors which demonstrate garlanded peripheral solid enhancement and diffusion-restriction. Posterior fossa ependymomas are now grouped by methylation profile into category A-typical posterior fossa ependymoma and category B which present in older children with smaller lesions with a better prognosis. Spinal ependymoma is relatively unchanged by molecular characterization apart from the new MYCN-altered tumor which has poor prognosis and is often advanced/disseminated at presentation.

Aggressive biological behavior of benign central nervous system neoplasms: The case of hemorrhagic subependymomas - An illustrated review.
Subependymomas (SEs) are low-frequency, histologically benign tumors. Their association with hemorrhagic phenomena is extremely unusual. This is a literature review of pathologically verified reports of hemorrhagic SE (hSE), analyzing their epidemiological, clinical, diagnostic, therapeutic, and outcome features. This review includes an original illustrating report. A total of 30 cases were analyzed. All but three patients presented unique hSEs growing predominantly within the lateral ventricles (86.6%). The pattern of associated hemorrhage was, in decreasing order of frequency, intratumoral (55.2%), mixed, and extratumoral. Symptoms at presentation, mainly of rapid onset (75%), derived from high intracranial pressure, meningeal irritation, and/or focal neurological impairment. Superficial siderosis was an exceptional complication, presumably produced after repeated bleeding. Most hSEs were misdiagnosed either as a more aggressive neoplasm (84.6%) or as a vascular malformation (7.7%) because of their invariably atypical, heterogeneous radiological appearance. Twenty-nine patients underwent surgical treatment, reaching total resection rates close to 80%. Nevertheless, high figures of both permanent disability (25%) and mortality (10.3%) were recorded at the last follow-up. Bleeding increases both the severity of initial clinical symptoms and the complexity of the radiological differential diagnosis of SEs but does not seem to influence strongly the degree of surgical resection of these otherwise histologically benign neoplasms. Both the exact topography of the hSE within the ventricular system and the presence of intraparenchymal extension of the neoplasm and/or the associated bleeding could negatively affect the final clinical outcome.
