A síndrome auriculotemporal se manifesta com vermelhidão ou suor no rosto, restrito à área de atuação do nervo auriculotemporal. Ela pode surgir após um trauma (lesão) na glândula parótida (uma glândula salivar), estar associada a tumores (neoplasias) nessa glândula, ou aparecer depois da remoção cirúrgica desses tumores.

Clinicopathological characteristics and surgical outcomes of pleomorphic adenoma of salivary glands among Sudanese patients.

Pleomorphic adenoma is the most common salivary gland tumor affecting both major and minor glands. It has different clinical presentations and histological patterns. Outcome of pleomorphic adenoma varies widely with the risk of recurrence, malignant transformation and facial nerve involvement being the most important aspects among other postoperative complications. To study the clinicopathological features of pleomorphic adenoma, its distribution in head and neck region and evaluation of outcomes of surgical treatment in terms of tumor recurrence and other postoperative complications. Retrospective analytical hospital–based study of all records of patients diagnosed with pleomorphic adenoma of salivary glands who attended Khartoum Teaching Dental Hospital (KTDH) during period from 2007 to 2021. Data was analyzed using the Statistical Package for Social Sciences (SPSS) version 27. Chi square test to assess associations. Linear regression, binary logistic regression & cox regression multivariate analysis to estimate the significance and relations. Two hundred fifteen patients fulfilled the inclusion criteria. Pleomorphic adenoma was found to be more common in females, in the fourth decade of life affecting the right side more commonly. Minor glands were affected more commonly with the palate being the most common site followed by the parotid. Definitive histopathological assessment revealed 201 (93.5%) pleomorphic adenoma with classic subtype being the most common, while 14 (6.5%) were carcinoma arising within pleomorphic adenoma. Post operative complications occurred in 72(33%) with facial nerve injury and oroantral fistula being the most commonly encountered. Recurrence was observed in 15 (7%) and malignant transformation in 3 cases (1.4%). Although pleomorphic adenoma is a benign salivary gland neoplasm, it can grow into extensive size if left untreated causing disfigurement and may undergo malignant transformation thus need to be diagnosed early and complete excision of the tumor is the definitive treatment to prevent tumor recurrence. Meticulous dissection of facial nerve during parotid operation and use of SMAS layer flap is also recommended so as to prevent facial nerve damage and occurrence of Frey syndrome. Long-term follow up of pleomorphic adenoma is required to assess the risk of recurrence and malignant transformation as they usually occur over a long duration.

Treatment of Traumatic Frey Syndrome and Facial Contour Deformity Using a De-Epithelialized Free SCIP Flap: A Case Report.

Frey syndrome (FS), or gustatory sweating, is typically characterized by sweating and flushing in the parotid region during eating. While FS most commonly develops after parotidectomy, it has also been reported following traumatic causes such as submandibular gland surgery, radical neck dissection, mandibular condyle fracture, or forceps delivery. The authors present the case of a 58-year-old male who developed FS and a right-sided facial contour deformity 11 months following severe blunt maxillofacial trauma. On the basis of the diagnosis, a de-epithelialized superficial circumflex iliac artery perforator (SCIP) free flap was planned and successfully utilized to simultaneously treat the gustatory sweating and restore facial volume. The patient's complaints of redness and sweating in the preauricular region resolved completely. At the 12-month postoperative follow-up, the patient achieved significant symmetry with the contralateral face. To our knowledge, a free flap has not been previously reported in the English literature for the treatment of established FS. The SCIP flap offers a versatile, reliable, autologous solution that functions as both a barrier to aberrant nerve regeneration and a volume filler for contour correction. The authors believe this case is important as it demonstrates the utility of free flaps in the management of FS.

Frey's syndrome in an infant misdiagnosed as food allergy.

Frey's syndrome (auriculotemporal syndrome) is characterized by gustatory flushing and sweating in the distribution of the auriculotemporal nerve. Although classically described after parotid surgery in adults, congenital and idiopathic forms are increasingly recognized in infants. Because symptoms occur during feeding, the condition can closely mimic IgE-mediated food allergy, leading to unnecessary dietary restrictions and anxiety. We report an 8-month-old boy referred for evaluation of suspected multiple food allergies after several emergency department visits for feeding-associated facial redness, interpreted by caregivers as "hives", and accompanied by significant parental anxiety and progressive dietary avoidance. On detailed reassessment, reactions were found to consist of localized, unilateral flushing and sweating of the right cheek, without urticaria or systemic features. Skin prick testing and serum-specific IgE testing to common food allergens were negative. During a supervised baked milk oral food challenge, a similar unilateral cutaneous reaction was observed, confirming the diagnosis of Frey's syndrome. A home-recorded video demonstrated comparable findings during feeding with formula. Frey's syndrome is an important and under-recognized mimic of food allergy in infancy. Careful attention to laterality, absence of systemic involvement, and reproducibility across foods can prevent misdiagnosis, reduce unnecessary dietary restriction, and alleviate family anxiety. Frey's syndrome should be considered in infants with reproducible unilateral flushing during feeding. Awareness among allergists can prevent misdiagnosis, reduce unnecessary food avoidance, and support safe nutritional and developmental outcomes.

[Update on salivary gland surgery].

Recent years have seen publication of studies on salivary gland surgery with a major impact on daily practice. This applies to both the surgical treatment of benign and malignant tumors and to the treatment of obstructive and inflammatory diseases of the salivary glands. In the case of tumors, extracapsular dissection has led to a shift toward de-escalation of facial exposure and resection with low morbidity over the past 20 years. For inflammatory and obstructive diseases, minimally invasive treatment methods have become established over the past 35 years, especially over the past 10. This paper provides an update on salivary gland surgery. Part 1 deals with tumor surgery and part 2 with the surgical therapy of obstructive and inflammatory diseases. This article represents a compilation of current developments based on the latest literature, with a focus on the past decade and the authors' our own experience. Extracapsular dissection is superior to superficial parotidectomy in terms of Frey syndrome, operating time, esthetic appearance, and transient and permanent facial function-provided the indications are appropriate. In malignant tumors without infiltration of the facial nerve, the latter should be preserved in the context of complete parotidectomy. Radical parotidectomy is only indicated in cases of facial nerve infiltration. Elective ipsilateral neck dissection is indicated for T3/4 tumors, high-grade carcinomas, and adenoid cystic carcinomas. Therapeutic and interventional sialendoscopy and sialendoscopy-assisted surgical procedures dominate the treatment of obstructive and inflammatory salivary gland diseases and can also be important in cases of salivary duct injury. The indication for surgery and the choice of surgical technique depend on the location and biological behavior of the tumor. Based on the developments outlined above, resection of the major salivary glands is indicated in less than 5% of cases of obstructive or inflammatory diseases. HINTERGRUND UND ZIEL: In den letzten Jahren sind auf dem Gebiet der Speicheldrüsenchirurgie Studien publiziert worden, die einen großen Einfluss auf die tägliche Praxis haben. Dies gilt einerseits für die chirurgische Therapie von benignen und malignen Tumoren, andererseits auch für die Behandlung von obstruktiven und inflammatorischen Erkrankungen der Speicheldrüsen. Bei den Tumoren gab es mit der extrakapsulären Dissektion in den letzten 20 Jahren einen Shift zu einer Deeskalation der Fazialisdarstellung und einer Resektion mit einer geringen Morbidität. Bei den entzündlichen und obstruktiven Erkrankungen haben sich minimal invasive Therapiemethoden seit den letzten 35 Jahren, insbesondere aber seit den letzten 10 Jahren durchgesetzt. Ziel der Arbeit ist es, ein Update über die Speicheldrüsenchirurgie zu geben. Dabei beschäftigt sich Teil 1 mit der Chirurgie von Tumoren und Teil 2 mit der Chirurgie von obstruktiven und inflammatorischen Erkrankungen. Es handelt sich um eine Zusammenstellung der aktuellen Entwicklungen anhand der neuesten Literatur mit Fokus auf die letzten 10 Jahre und von eigenen Erfahrungen. Die extrakapsuläre Dissektion ist der superfiziellen Parotidektomie bei geeigneter Indikation in den Punkten Frey-Syndrom, Op.-Dauer, ästhetischer Aspekt sowie transiente und permanente Fazialisfunktion überlegen. Bei malignen Tumoren ohne Infiltration des N. facialis sollte der N. facialis im Sinne einer kompletten Parotidektomie erhalten werden. Eine radikale Parotidektomie ist nur bei Fazialisinfiltration indiziert. Eine elektive ipsilaterale Neck-Dissection ist bei T3/4-Tumoren, High-Grade-Karzinomen und adenoidzystischen Karzinomen indiziert. Die therapeutische und interventionelle Sialendoskopie und sialendoskopisch assistierte operative Verfahren dominieren die Therapie bei obstruktiven und inflammatorischen Speicheldrüsenerkrankungen und können auch bei Verletzungen des Speichelgangs von Bedeutung sein. Die Indikationsstellung zur Operation und die Wahl der chirurgischen Technik sind von der Lage und der Dignität des Tumors abhängig. Aufgrund der skizzierten Entwicklungen ist eine Resektion der großen Kopfspeicheldrüsen bei obstruktiven bzw. inflammatorischen Erkrankungen in weniger als 5 % der Fälle indiziert.

Extracapsular dissection versus partial superficial parotidectomy: a systematic review and meta-analysis.

To compare perioperative outcomes and surgical safety between extracapsular dissection (ECD) and partial superficial parotidectomy (PSP) for benign parotid tumors through a systematic review and meta-analysis. PubMed, Embase, and the Cochrane Library were searched for studies published up to March 2025. English-language studies that directly compared ECD and PSP in patients with benign parotid tumors were included. Data were extracted independently by two authors, and meta-analyses were conducted using a random-effects model. Outcomes included operative duration, drainage volume, hospital stay, tumor size, incidences of Frey syndrome, facial nerve (FN) injury (temporary and permanent), recurrence, as well as the rate of complete tumor excision. Fourteen studies met the inclusion criteria. Compared with PSP, ECD was associated with shorter operative duration (mean difference [MD], - 15.42 min; 95% confidence interval [CI], - 23.47 to - 7.37), lower drainage volume (MD, - 31.32 mL; 95% CI, - 32.88 to - 29.76), shorter hospital stay (MD, - 0.54 days; 95% CI, - 0.78 to - 0.29), and reduced rates of both Frey syndrome (risk difference [RD], - 0.03; 95% CI, - 0.05 to - 0.00) and temporary FN injury (RD, - 0.12; 95% CI, - 0.20 to - 0.05). No significant differences were found in tumor size, recurrence, permanent FN injury, complete excision rate, or other complications. ECD and PSP demonstrate comparable surgical safety for benign parotid tumors. ECD offers certain perioperative advantages and may be a suitable option for appropriately selected patients. Further prospective studies are warranted to confirm these findings.
