Caregiver and healthcare professional perspectives on drivers of routine immunisation uptake in East New Britain, Papua New Guinea: a qualitative study.
Immunisation coverage varies across Papua New Guinea (PNG), with 40% of children in East New Britain Province (ENBP) classified as zero-dose (children missing diphtheria-tetanus-pertussis vaccine first dose (DTP1)) and 55% as under-immunised (missing DTP3) in 2023. This study aimed to understand routine childhood immunisation enablers and barriers in areas of ENBP reported to have highly variable DTP1, DTP3 and measles-rubella first-dose (MR1) coverage. Qualitative in-depth semi-structured interviews were conducted face-to-face with caregivers of children aged 9-23 months and healthcare professionals in Gazelle and Kokopo districts. Local research officers conducted interviews in Tok Pisin and English. Data were thematically analysed and mapped across the six UNICEF Journey to Health and Immunisation framework stages. 33 caregivers and 28 healthcare professionals were interviewed. Despite finding that a high proportion of caregivers' children had received DTP, DTP3 and MR1, participants identified a myriad of contextual caregiver and healthcare professional enablers and barriers to routine immunisation. Caregiver enablers included support provided by families and community-based services; barriers included limited immunisation knowledge, financial and travel challenges, and limited availability of healthcare professionals. Healthcare professional enablers included increased operational funds and working with community leaders, while barriers were focused on service delivery including costs, non-functioning aid posts and vaccine stockouts. Strategies suggested by participants for improving vaccine uptake included developing tailored immunisation communication materials, engaging community members to support routine immunisation promotion activities, and strengthening coordination between communities and healthcare professionals. This study highlights the challenge of identifying and including zero-dose children and their caregivers in research but still provides useful contextual focus points to enhance and inform tailored strategies to strengthen immunisation service delivery. These include strategies that improve bidirectional coordination and communication between healthcare professionals, community and religious leaders about routine immunisation, increase immunisation knowledge and strengthen community-based services for vaccine delivery.
Diphtheria and pertussis outbreaks in the Americas: a scoping review of epidemiology, vaccination trends and control strategies, 2013-2024.
To describe the themes identified in the epidemiology of diphtheria and pertussis outbreaks in the Americas (2013-2024), with a particular focus on vaccination coverage, trends and control strategies. Scoping review. Systematic searches were conducted in PubMed and Embase (2024), supplemented by a search of Latin American and Caribbean Health Sciences Literature (LILACS) in December 2025. Regional case data and vaccination coverage were sourced from the Pan American Health Organization (PAHO)/WHO and UNICEF electronic joint reporting form (eJRF). The study reviewed literature and health reports focusing on diphtheria and pertussis outbreaks and vaccination coverage. Thematic analysis identified themes in the literature on the epidemiology and control strategies of diphtheria and pertussis as well as outbreak reporting and mortality. The retrospective regional data analysis encompassed aggregated eJRF data (2013-2024) on cases and on diphtheria-pertussis-containing vaccination coverage. Of 82 articles identified across 14 countries, 28 met the inclusion criteria. Diphtheria cases peaked in 2017-2018 and have been on an upward trend since. Pertussis cases surged to 66 184 in 2024, with 10 countries reporting high numbers and 17 having their third dose of the vaccine against diphtheria, tetanus and pertussis (DTP3) coverage below 90%. Outbreaks are often linked to low DTP3, inconsistent vaccination coverage, gaps in maternal and booster doses, inadequate surveillance and socioeconomic disparities. However, two countries with high DTP3 coverage (eg, the USA, Canada) still experienced pertussis outbreaks, showing the disease's complexity. Experts recommend scaling up vaccination and strengthening surveillance to address these challenges. Diphtheria and pertussis are resurging in the Americas. The growing number of 'zero-dose' children and socioeconomic disparities in vulnerable communities are exacerbating the problem. A comprehensive approach is recommended, prioritising increased vaccination efforts, strengthened surveillance and ensuring equitable access to healthcare services.
Global patterns and disparities in maternal and child health: a panel analysis of global health observatory and world development indicators data, 2000-2023.
This study aims to evaluate global trends in maternal and child health-related indicators in the Millennium Development Goals (MDG) and Sustainable Development Goal (SDG) eras, examine whether these associations differ across regions and income groups and assess their impact on life expectancy (LE) at birth. We conducted a country-level panel analysis using data from 2000 to 2023 obtained from the Global Health Observatory and the World Development Indicator database. All WHO Member States with available data were included. Data on under-five mortality rate (U5MR), maternal mortality ratio (MMR), neonatal mortality rate (NMR), skilled birth attendance, ratio of nurses and midwives, total fertility rate (TFR), LE, prevalence of anaemia and hypertension in women aged 15-49 years, diphtheria tetanus toxoid and pertussis (DTP3) vaccine coverage, gross domestic product and government health expenditure as a percentage of GDP were collected, cleaned and prepared for analysis. Missing values (<10%) were imputed using linear interpolation and 5-year mean methods. Descriptive statistics and panel regression analyses were conducted to examine associations between maternal and child health indicators and LE. In addition, Joinpoint regression models were applied to assess temporal trends and estimate annual percent changes (APCs) during the MDG and SDG eras. All statistical analyses were performed using Stata MP version 17.0. Sensitivity analyses were conducted to evaluate the robustness of the findings. Trends in LE and maternal and child health indicators and maternal and child-related factors associated with LE. Maternal and child health improved substantially, with faster progress during the MDG era (2000-2015) compared with the SDG era (2015-2023). The MMR declined by 130.30 (100 in MDG vs 30.30 in SDG) deaths per 100 000 live births overall, from 327.60 (95% UI: 308.80-348.60) in 2000 to 197.30 (95% UI: 174.50-234.00) in 2023. Similarly, U5MR declined by 39.95 (28 in the MDG vs 11.95 in the SDG era) deaths, from 76.67 (95% UI: 75.70-77.86) to 36.72 (95% UI: 34.66-41.09) deaths per 1000 live births. The NMR decreased by 13.38 per 1000 live births, from 30.71 (95% UI: 29.91-31.58) to 17.33 (95% UI: 16.25-19.41) deaths, and LE increased by 6.6 years, from 66.77 (95% UI: 66.23-67.18) to 73.33 (95% UI: 72.73-73.93) globally. Fixed-effects regression results showed that higher U5MR (β=-0.0697, 95% CI -0.080 to -0.060, p<0.001), MMR (β=-0.0118, 95% CI -0.020 to -0.010, p<0.001), TFR (β=-0.0258, 95% CI -0.040 to -0.020, p<0.001) and prevalence of anaemia (β=-0.0223, 95% CI -0.030 to -0.010, p<0.001) were associated with reduced LE, whereas skilled birth attendance (β=0.0417, 95% CI 0.030 to 0.050, p<0.001), DPT3 coverage (β=0.0425, 95% CI 0.030 to 0.050, p<0.001) and GDP (β=0.0041, 95% CI 0.001 to 0.010, p=0.01) were associated with increased LE. Despite progress in maternal and child health, gaps remain, particularly in low-income and lower-middle-income countries. Strengthening and retaining the health workforce, expanding access to immunisation and family planning services and improving early detection and management of hypertension and anaemia among women are critical to sustaining gains and accelerating improvements in LE.
Understanding adverse events following immunisation: evidence of vaccine safety and coincidental events from 3 years of surveillance.
Periodic vaccine safety checks are a crucial component of any country's pharmacovigilance system. Unwanted reactions following childhood vaccination are unacceptable to the public, making rapid safety checks and alerts essential for maintaining public confidence. The study evaluates the safety of childhood vaccines in the paediatric population through active surveillance and explores various predictors for the development of adverse event following immunisation (AEFI) based on population characteristics. A cohort event monitoring study was designed to identify AEFIs in children receiving at least one childhood vaccine under the Universal Immunisation Programme (UIP) at the study site. Both active surveillance (via telephone follow-up calls) and passive surveillance (from healthcare workers) were included. Potential predictors of AEFIs were evaluated using the χ2 test, and relative risk was calculated. The study estimated an AEFI incidence rate of 10.37%, with 3.47% (1.27 events/1000 doses) being serious reactions requiring hospitalisation. The highest incidence was with the Pentavac vaccine (diphtheria, tetanus, pertussis, hepatitis B, Hib) at 19.21% (218 events/1000 doses), while the lowest incidence was observed with the oral polio vaccine at 0.13% (1.32 events/1000 doses). Most reported Adevrse Events were solicited (34.83 events/1000 doses), while unsolicited events accounted for 1.69/1000 doses. The most common reaction was fever (23.75 events/1000 doses), followed by pain at injection site (4.98 events/1000 doses). One death following Pentavac vaccination was reported but later deemed coincidental. Overall, 90.62% of the reactions were vaccine product-related, 4.76% coincidental, 2.70% immunisation error-related and 0.39% were indeterminate. Predictors of AEFIs included paediatric population, delayed immunisation, nutritional deficiencies and comorbid status of the child and children from affluent families. Childhood vaccines included in the UIP have shown a favourable safety profile. Serious reactions are infrequent, emphasising the need for thorough investigation and clear communication to differentiate them from coincidental reactions, which can occur more frequently in this age group.
Optimising DTwP-containing vaccine infant immunisation schedules in Uganda and Nepal (OptImms): two open-label, non-inferiority, randomised controlled trials.
Few data are available comparing the WHO diphtheria-tetanus-pertussis (DTP)-containing vaccine schedule with reduced-dose or delayed three-dose schedules in infants. We aimed to identify an alternative schedule that is non-inferior or superior to the WHO schedule in maintaining early pertussis antibody levels before booster, while creating space for integration of new vaccines. In two parallel, open-label, randomised, non-inferiority trials in Uganda and Nepal, healthy infants aged 42-50 days were randomised in a 4:4:4:3:3 ratio via an online system to five diphtheria-tetanus-whole-cell pertussis-Haemophilus influenzae type b-hepatitis B (DTwP-Hib-HepB) vaccination schedules (WHO schedule [ages 6, 10, and 14 weeks]; reduced two-dose schedules at ages 6 and 14 weeks or ages 2 and 4 months; or delayed three-dose schedules at ages 2, 3, and 4 months or ages 2, 4, and 6 months), using site-stratified block randomisation with a block size of 18. The primary outcome was the pre-booster IgG antibody response against pertussis antigens (pertussis toxin; filamentous haemagglutinin [FHA]; pertactin; fimbriae 2 and 3 [Fim 2 and 3]). Antibody responses during and 1 month after the primary series were assessed as secondary endpoints. The two-dose schedules were compared with the WHO schedule as primary analyses, and the exploratory analyses were to compare delayed three-dose schedules with the WHO schedule. All these analyses were conducted in the per-protocol population. For the primary endpoint, non-inferiority margins for geometric mean ratios (GMRs) between two-dose and WHO schedules were calculated for each pertussis antigen separately as 35% of the standard deviation of the geometric mean concentrations of the WHO group, and non-inferiority was concluded if the lower bound of the 95% CI of the GMR exceeded the non-inferiority margin. A non-inferiority margin of 0·67 was used for all other non-inferiority comparisons. The trials were registered with ISRCTN (Uganda [ISRCTN60356654] and Nepal [ISRCTN12240140]) and are complete. Between Oct 1, 2021, and July 12, 2022 (Uganda) and Dec 5, 2021, and Feb 26, 2023 (Nepal), 956 infants were recruited per country and randomly allocated across the five study groups; pre-booster analyses were conducted in 876 participants in Uganda and 851 participants in Nepal. Two-dose schedules at ages 6 and 14 weeks and ages 2 and 4 months did not meet non-inferiority criteria for pertussis antibodies pre-booster, except FHA (non-inferiority margin 0·59) in Nepal (6 and 14 weeks GMR 0·83 [95% CI 0·61-1·13]; 2 and 4 months 1·20 [0·88-1·64]). Delayed three-dose schedules at ages 2, 3, and 4 months and ages 2, 4, and 6 months produced similar or higher post-primary series and pre-booster responses compared with the WHO schedule, but the WHO schedule achieved higher antibody responses against all pertussis antigens, except FHA in Nepal, at age 3 months. The WHO DTP schedule is the preferred schedule in high-pertussis-burden settings, as it elicits the greatest antibody responses in the first 3 months of life when infants are particularly vulnerable. Although delayed three-dose schedules may be considered in low-risk contexts and in settings with maternal immunisation programmes, early infant protection should guide policy decisions.
