A rare case of mosaic partial tetrasomy 18p presenting with oligomenorrhea and intellectual disability: a case report.
Tetrasomy 18p syndrome is an extremely rare chromosomal disorder that is often the result of an additional isochromosome for the short arm of chromosome 18. Most cases result from de novo mutations, although familial cases have also been reported. In this article, we report for the first time a case of a 17-year-old female patient with mosaic tetrasomy 18p. This study aimed to investigate the genetic and clinical characteristics of patients with tetrasomy 18p and to enhance the understanding of tetrasomy 18p. A female patient presenting with oligomenorrhea, distinctive physical features, and intellectual disability underwent chromosomal karyotype analysis via G-, C-, and N-banding techniques. Upon the detection of abnormal results, high-throughput genomic copy number variation sequencing (CNV-seq) was performed to establish a definitive diagnosis. Additionally, chromosomal karyotype analysis was conducted on both parents and the patient's three younger sisters. The G-banding karyotype results indicated 47,XX,+mar[85]/46,XX[15], suggesting a mosaic condition with one additional marker chromosome. C-banding revealed one centromere on the small marker chromosome, whereas N-banding revealed no satellite. CNV-seq analysis revealed a 14.86 Mb mosaic duplication spanning 18p11.32-p11.21, with an estimated copy number of 3.5. On the basis of these findings, the final chromosomal karyotype was designated 47,XX,+i(18)(p10)[85]/46,XX[15]. No chromosomal abnormalities were identified in the karyotypes of the patient's parents or three sisters, indicating that this is a de novo mutation. The patient's oligomenorrhea, distinctive features, and intellectual disability may be associated with a partial duplication of the short arm of chromosome 18. Previous reports on patients with tetrasomy 18p have focused primarily on preschool or prepubescent patients. Here, we present the first reported case of a 17-year-old female with mosaic tetrasomy 18p. The absence of fingerprints on the palms and oligomenorrhea may represent novel characteristics of this condition. For such rare chromosomal abnormalities, the combined use of G-banding, C-banding karyotype analysis, CNV-seq technology, and comprehensive clinical evaluation can facilitate early diagnosis, genetic counseling, and personalized treatment.
Macular and optic nerve hypoplasia in chromosome 2p partial trisomy.
The 2p duplication syndrome is a rare clinically heterogeneous disorder that arises from non-recurrent chromosomal rearrangements involving ~6 Mb up to ~90 Mb. The patients are characterized by a wide range of symptoms, including developmental delay, intellectual disability, distinctive facial features, congenital heart defects, and various ophthalmic manifestations. We report a male newborn with maternally inherited unbalanced translocations resulting in partial trisomy of chromosome 2p (33.9 Mb) - 46,XY,der(10)t(2;10)(p22.3;p1?5)dmat. Fluorescence in situ hybridization and chromosomal microarray analyses confirmed three copies of 2p25.3-p22.3 region, encompassing 51 known disease-causing genes. The patient presented with low birth weight, ventricular septal defect, camptodactyly of the 3rd digit of the left hand and mild early feeding problems. Ophthalmological assessment revealed macular and optic nerve hypoplasia. MRI demonstrated hypoplasia of the optic chiasm and optic nerves, and partial agenesis of falx cerebri. We describe macular and optic nerve hypoplasia in a patient with a novel chromosomal rearrangement resulting in 2p partial trisomy and provide an overview of 17 previously reported cases presenting ocular abnormalities. A broad variability of ophthalmological phenotypes has been observed, further expanded by the current report.
Long-Term Survival Among Children With Trisomy 13 and Trisomy 18 by Cytogenetic Status.
Trisomy 13 (T13) and trisomy 18 (T18) are chromosomal abnormalities with high mortality rates in the first year of life. Understanding differences in long-term survival between children with full vs mosaic or partial trisomy is crucial for prognosis and health care planning. To examine the differences in 10-year survival between children with full T13 and T18 vs those with mosaic or partial trisomy. This retrospective, population-based cohort study assessed liveborn infants with T13 and T18 in the Texas Birth Defects Registry (deliveries from January 1, 1999, to December 31, 2008). Follow-up was through December 31, 2018 (the last date available at the time of analyses) to allow for 10 years of follow-up for all infants. All analyses were conducted from January 1, 2022, to December 31, 2024. Cytogenetic status (full trisomy vs mosaic or partial trisomy). The primary outcome was survival to 10 years of age, assessed using Kaplan-Meier survival estimates. The association between cytogenetic status and mortality by 10 years of age was assessed using Cox proportional hazards regression to generate hazard ratios (HRs). Population attributable fraction was calculated to determine the percentage of survival attributable to mosaic or partial trisomy status. The study cohort included 798 infants (463 female infants [58.0%]; mean [SD] maternal age, 30.9 [8.0] years) with T13 (n = 295) or T18 (n = 503). Among all cases with T13, 25 infants (8.5%; 95% CI, 5.5%-12.3%) survived to 10 years of age. Similarly, among all infants with T18, 43 (8.6%; 95% CI, 6.3%-11.3%) survived to 10 years of age. Kaplan-Meier survival estimates to 10 years of age were statistically significantly higher among children with mosaic or partial trisomy (13 [25.0%] and 14 [43.8%], respectively) compared with full trisomy (12 [4.9%] and 29 [6.6%], respectively) (both P < .001). Infants with full trisomy had statistically significantly increased 10-year mortality hazards compared with those with mosaic or partial trisomy for both T13 (HR, 2.00; 95% CI, 1.42-2.82) and T18 (HR, 3.34; 95% CI, 2.08-5.38). The results of the calculated proportion of 10-year survival due to the presence of nonfull trisomy status (population attributable fraction) was 41.7% for children with T13 and 27.9% for children with T18. The findings of this cohort study of infants with T13 and T18 support differences in long-term survival based on cytogenetic status and emphasize the need to potentially reassess the context of these conditions generally being considered incompatible with life, particularly for those with mosaic trisomies. These findings offer context surrounding treatment decisions, such as withholding interventions, for affected infants in the future.
Prenatally Diagnosed De Novo Interstitial Duplication in 2p21p24.3 with Unique Manifestations: Case Report.
Partial duplications involving chromosome 2p are rare genomic events associated with variable phenotypic outcomes, ranging from craniofacial abnormalities, organ malformations to developmental delays. Prenatal identification of such duplications has become increasingly common with advances in chromosomal microarray technology, but predicting postnatal manifestations remains challenging. This report presents a de novo 32 Mb duplication of 2p21p24.3, detected prenatally, in a male patient who demonstrated unique clinical features and multisystem involvement postnatally. A 29-year-old pregnant female was referred for genetic evaluation following prenatal detection of flat nasal bridge, frontal bossing, short extremities, micropenis, and suspected hypospadias. Chromosomal microarray analysis revealed a 32 Mb duplication of 2p21p24.3, confirmed as de novo by parental testing. Postnatally novel features such as subglottic haemangioma, preauricular pit, and behavioural symptoms related to facial contact were noted. Growth delay, craniofacial dysmorphism, congenital heart defects, and genital anomalies were also observed. A multidisciplinary approach involving clinical genetics was critical for addressing the patient's complex medical needs. Despite ongoing supportive care, the patient continues facing significant developmental and medical challenges, underscoring the need for long-term follow-up as well as genotype-phenotype correlations. This case broadens the clinical spectrum of partial trisomy 2p by documenting unique features, including subglottic haemangioma, behavioural symptoms, and preauricular pit. It highlights the value of integrating prenatal diagnostics with detailed postnatal phenotypic assessment to improve understanding of genotype-phenotype correlations in rare chromosomal duplications. Although the PPP1CB gene within the region is considered a plausible candidate for explaining some of the patient's cardinal features, dysmorphism (prominent forehead, preauricular ear pits, low-set, posteriorly rotated ears), cardiac-neurological problems, other genes associated with defined syndromes - whose clinical manifestations overlap to varying degrees - were also considered as potential contributors to the clinical picture, such as MYCN, DNMT3A, DPYSL5, and SOS1. These insights can aid clinicians in genetic counselling, anticipatory guidance, and management of similar cases in the future.
Co-Occurrence of Urogenital Anomalies and Congenital Heart Disease in a Child With Alpha-Thalassemia Mental Retardation Syndrome Associated With Chromosome 16 Abnormalities due to Partial Monosomy 16p13.3 and Partial Trisomy 16q22.1-q24.3.
We report the case of a 3-year-old girl with alpha-thalassemia/mental retardation linked to chromosome 16 (ATR-16) syndrome. The patient presented with hypotonia, developmental delay, and characteristic facial features including hypertelorism and a broad nasal bridge. Blood test results indicated microcytic anemia and normal iron status, suggestive of thalassemia. Genetic analysis revealed that the patient harbored a 465 kb deletion in the 16p13.3 region and a 19.4 Mb duplication in the 16q22.1-q24.3 region. The patient presented with rare complications of ATR-16 syndrome, including anal fistula, vesicoureteral reflux (VUR), and patent ductus arteriosus (PDA). Comparison of this case with previously reported patients with pure partial trisomy 16q suggested that the duplicated distal 16q region may be a critical locus associated with VUR and PDA.
