Comorbidities in Discoid Lupus: A Systematic Review and Meta-analysis of Prevalence and Association.

Discoid lupus erythematosus (DLE) is a chronic inflammatory scarring alopecia that may be accompanied by a broad range of systemic and extracutaneous comorbidities. However, the prevalence and magnitude of these associations have not been comprehensively quantified. Our objective was to systematically evaluate the prevalence and associated risk of comorbidities in patients with discoid lupus erythematosus. We conducted a systematic review and random-effects meta-analysis in accordance with PRISMA 2020 guidelines, prospectively registered in PROSPERO (CRD420250656067). MEDLINE and Embase were searched from inception to June 2025. Observational studies reporting comorbidities in patients with DLE were included. Pooled prevalence estimates and pooled odds ratios (ORs) with 95% confidence intervals (CIs) were calculated using random-effects models. Between-study heterogeneity was assessed using the I2 statistic. Sensitivity analyses excluding small studies (n < 50) were performed for prevalence outcomes, whereas the robustness of association analyses was assessed qualitatively owing to the limited number of contributing studies. Methodological quality was evaluated using the Joanna Briggs Institute (JBI) Critical Appraisal Checklist. In total, 25 studies were included. Pooled prevalence estimates indicated a high frequency of systemic lupus erythematosus (SLE) (22.0%), hypertension (24.3%), depression (38.8%), and hypothyroidism (12.4%) among patients with DLE. Sensitivity analyses showed stable prevalence estimates for SLE and hypertension, with moderate variability for hypothyroidism and depression. Meta-analyses of case-control studies demonstrated significant associations between DLE and cardiovascular disease (OR 3.85, 95% CI 1.67-8.88), hypothyroidism (OR 2.55, 95% CI 1.79-3.61), hypertension (OR 2.22, 95% CI 1.91-2.58), depression (OR 2.21, 95% CI 1.71-2.85), and anxiety (OR 2.40, 95% CI 1.40-4.11). Associations with type 2 diabetes were more modest, while smoking was not significantly associated. Most association analyses were based on a limited number of studies. Patients with discoid lupus erythematosus exhibit a substantial burden of systemic, cardiometabolic, endocrine, and psychiatric comorbidities. While prevalence estimates were generally consistent, associations should be interpreted cautiously owing to limited available evidence. These findings highlight the importance of comprehensive clinical assessment and multidisciplinary management of patients with DLE. The study protocol was prospectively registered in PROSPERO (CRD420250656067).

Implications of Dermatologic Disorders in Facial Cosmetic Surgery: A Systematic Review.

Invasive and minimally invasive cosmetic procedures in patients with autoimmune or inflammatory skin disorders can carry an increased risk of surgical site infection, disease exacerbation, atrophic/hypertrophic scarring, or suboptimal cosmetic results. As facial procedures continue to rise in popularity, increasing complications are being reported in association with underlying dermatologic disease. In this review, we aim to bring attention to these dermatologic conditions and the potential challenges that they pose to the facial plastic surgeon. A comprehensive literature search was conducted across PubMed, Embase, and MEDLINE for articles published before January 2025 describing one or more patients with complications following facial or scalp procedures relating to a pre-existing or underlying dermatologic disorder or systemic condition with dermatologic manifestations. Articles were screened according to PRISMA guidelines, and data extraction included dermatologic disease diagnosis and severity, facial procedure performed, perioperative or postoperative complications, and resolution/treatment. Our search criteria identified 2452 articles; 53 articles met the inclusion criteria. In a total of 469 patients with pre-existing dermatologic conditions, mild-severe complications were reported after blepharoplasty (41/469, 8.7%), rhinoplasty (254/469, 54.2%), rhytidectomy (25/469, 5.3%), hair transplantation (37/469, 7.9%), cosmetic injections (35/469, 7.5%), lasers (15/469, 3.2%), and microneedling (50/469, 10.7%). Dermatologic disorders included acne, atopic dermatitis, frontal fibrosing alopecia, lichen planopilaris, rosacea, and vitiligo; systemic conditions with dermatologic manifestations included Ehlers-Danlos syndrome, sarcoidosis, scleroderma, and systemic or discoid lupus erythematosus. Complications ranged from mild postoperative disease exacerbation to severe hypertrophic scarring, ulcerations, and wound healing defects. The existing literature, while limited to case reports and small case series, demonstrates potential risks following facial procedures in patients with dermatologic disorders. This review highlights the importance of consideration of dermatologic disease in facial surgery planning and decision-making and summarizes effective strategies to minimize any potential risk.

Treatment of discoid lupus erythematosus with crisaborole.

Discoid lupus erythematosus (DLE) is a chronic autoimmune skin disorder that primarily affects sun-exposed areas and may lead to permanent scarring and dyspigmentation if inadequately treated. Therapeutic options for DLE remain limited, and long-term use of topical corticosteroids or calcineurin inhibitors is often constrained by adverse effects, particularly on facial skin. This report aims to describe the clinical response of facial DLE to topical crisaborole treatment. We report the case of a 20-year-old woman with a 7-year history of a persistent erythematous plaque on the nose, previously misdiagnosed and refractory to multiple treatments, including systemic antimalarials and topical agents. Histopathological examination and direct immunofluorescence findings supported a diagnosis of DLE without systemic involvement. The patient was treated with topical crisaborole 2% ointment once daily in combination with hydroxychloroquine. Marked clinical improvement was observed after two months of treatment. Sustained remission was maintained during a seven-month follow-up period with continued crisaborole use and dose reduction of hydroxychloroquine, without recurrence or adverse effects. Crisaborole, a topical phosphodiesterase-4 inhibitor approved for atopic dermatitis, may represent a well-tolerated nonsteroidal therapeutic option for facial DLE, particularly in patients who are unresponsive to or intolerant of conventional treatments. Further studies are needed to clarify its efficacy and long-term safety in DLE.

Association of Autoimmune Diseases With Pancreatic Cancer: A Nationwide Follow-Up Study From Sweden.

The incidence of pancreatic cancer (PC) and autoimmune diseases (ADs) has been increasing worldwide. While certain associations between specific ADs, such as pancreatitis, and PC have long been well confirmed, population-focused research investigating the broader spectrum of ADs and their relationship with PC risk remains limited. We implemented a cohort study based on population data to analyze the relationships between ADs and PC susceptibility. Diagnostic information on 43 ADs was obtained from the Swedish Inpatient Register (1964-2018), while cancer incidence and mortality data were sourced from the National Cancer Registry and Cause of Death Register starting in 1997. Relative cancer risks were quantified using standardized incidence ratios (SIRs) and standardized mortality ratios (SMRs). Within the study's total population of 16.4 million, 1.1 million cases of ADs were identified, and 3257 patients were later diagnosed with PC accounting for 5.42% of all cancer cases in the cohort. The SIRs for PC in patients with ADs were 1.24 (men) and 1.19 (women); 12 ADs were positively correlated with the incidence of PC. The SMRs for PC in patients with ADs were 1.28 (men) and 1.22 (women); 15 ADs were positively correlated with PC mortality. When the follow-up time was less than 1 year, the overall risk of PC in patients with ADs was 3.88; over 10 years, the risk reached 1.12. We have newly discovered the relationship between several ADs and the risk of PC incidence and mortality, including discoid lupus erythematosus, lupoid hepatitis, giant-cell arteritis, and rheumatic fever. The results of this study back the notion that ADs may have a role in promoting the onset of PC.

Pigmented Allergic Salute Sign in an Adult With Dark Skin: Clinical and Dermoscopic Features.

Post-inflammatory hyperpigmentation due to chronic mechanical friction is well recognized in individuals with darker skin types, yet the pigmented variant of the allergic salute sign remains rarely documented. We report a 40-year-old woman with a persistent transverse hyperpigmented band over the nasal dorsum, associated with long-standing allergic rhinitis and recurrent conjunctivitis. She described habitual upward rubbing of the nose due to intense pruritus. Clinical examination revealed a sharply demarcated dark linear band above the nasal tip. Dermoscopy demonstrated a homogeneous dark pseudo-network with prominent follicular openings, consistent with post-inflammatory hyperpigmentation in Fitzpatrick phototype VI skin. Awareness of this presentation is essential to differentiate it from lichen planus pigmentosus or discoid lupus erythematosus. Management focuses on controlling allergic rhinitis, minimizing mechanical friction, and cautious use of depigmenting therapies. This case highlights the pigmented allergic salute sign as a distinctive marker of chronic allergic disease in patients with dark skin. Lupus erythematosus is a multisystem disorder that predominantly affects the skin. There are many types of lupus, including systemic lupus erythematosus (SLE), drug-induced lupus erythematosus, neonatal lupus erythematosus, and cutaneous lupus erythematosus. The most common types of cutaneous lupus erythematosus include acute cutaneous lupus erythematosus, subacute cutaneous lupus erythematosus, and chronic cutaneous lupus erythematosus; discoid lupus erythematosus is a subtype of chronic cutaneous lupus erythematosus. Dr James Gilliam described the most commonly used classification of cutaneous lesions in lupus erythematosus. He distinguished lesions as either specific or nonspecific, based on the presence of interface dermatitis on histopathologic examination. Within the category of specific cutaneous lesions, he subdivided these into acute cutaneous lupus erythematosus, subacute cutaneous lupus erythematosus, and chronic cutaneous erythematosus. Discoid lupus erythematosus is the most common subset of chronic cutaneous lupus erythematosus. Patients may or may not report photosensitivity, but lesions are frequently photodistributed and often exhibit secondary atrophy or scarring. Most patients with discoid lupus erythematosus do not have significant systemic disease. Discoid lupus erythematosus can also occur as a manifestation of SLE in approximately 20% of patients. Other less common variants of chronic cutaneous lupus erythematosus include hypertrophic lupus erythematosus, tumid lupus erythematosus, lupus erythematosus panniculitis (also known as lupus profundus), chilblain lupus erythematosus, oral discoid lupus erythematosus, and discoid lupus erythematosus lesions on the palms and soles.
