A disorder characterized by urinary bladder outflow obstruction, which can represent an anatomical blockage or a functional obstruction. The most common anatomical causes are posterior urethral valves at the level of the prostatic urethra, a lesion unique to males. Less common are anterior urethral valves, also called urethral atresia, that can occur in either sex. LUTO is an autosomal dominant disease with variable expression.

Congenital Anomalies of Kidney and Urinary Tract in Children - Clinical Spectrum and Complications.
To study the spectrum of congenital anomalies of the kidney and urinary tract (CAKUT), and to examine the clinical presentation and potential complications. A total of 304 children were recruited (216 retrospectively and 88 prospectively). Clinical data were obtained from records in the retrospective cases, while clinical assessments, laboratory tests, and imaging were conducted as needed in the prospective cases. Primary vesicoureteric reflux (VUR) was the most common cause of CAKUT (34.2%), followed by posterior urethral valve (PUV) at 24% and pelvi-ureteric junction obstruction (PUJO) at 13.1%. Antenatal detection was the most common mode of identification (47.6%), followed by detection after urinary tract infection (UTI) in 29.5% of cases. Extrarenal manifestations occurred in 6.5% of cases. Recurrent UTI was the most common complication, seen in 42.3%. Additionally, 11.5% had hypertension, and 10% had a glomerular filtration rate (GFR) of less than 60 ml/min/1.73 m2. Primary VUR is the most common CAKUT, and the antenatal detection of anomalies is crucial. The prevalence of chronic kidney disease (CKD) stage 2 and above is quite high among CAKUT, particularly PUV, renal hypodysplasia, and VUR, warranting close nephrology follow-up.
Concomitant Bladder Neck Incision in Patients with Posterior Urethral Valve and Blad-der Neck Hypertrophy: Short-term Outcomes of a Randomized Controlled Trial.
Concomitant bladder neck incision (BNI) with posterior urethral valve ablation (VA) was proposed to mitigate the long-term sequela of posterior urethral valve (PUV) and reduce the reoperation rates. This study aimed to investigate the short-term outcomes of concomitant BNI and VA, particularly short-term reoperation rates. Patients with PUV and bladder neck hypertrophy on preoperative imaging were randomized to undergo VA only or VA with concomitant BNI. Surgical reoperation within one year was the primary endpoint. Renal function, UTI, hydronephrosis and VUR improvement at one year were secondary endpoints. Sixty-three patients were included in the final analysis, 33 in VA group (group A) and 30 in concomitant BNI and VA group (group B). After one year of follow-up, the reoperation rate was similar [5(15.2%) in group A and 3(10%) in group B, p=0.18]. The median (IQR) nadir serum creatinine was lower in group B [0.2 (0.1-0.3) vs. 0.2 (0.2-0.4) mg/dL in group A, p=0.049]. The last follow-up serum creatinine median (IQR) eGFR [107 (89.5-163) in group A vs. 139(102-165) mL/min/1.73 m2 in groups B, p=0.37], and febrile UTI rates were not different between the two groups. Hydronephrosis improved/ resolved in 27 (40.9%) renal units in group A vs. 33 (55%) renal units in group B (p=0.286). Vesicoureteral reflux improved/ resolved in 23(34.8%) and 12 (20%) renal units in group A and B, respectively (p=0.074). Concomitant BNI with VA does not confer a lower short-term reoperation rate or better upper urinary tract outcomes compared to VA only.
Posterior urethral valve presenting as posterior urethral calculus in adult: A rare case scenario.
Posterior urethral valve (PUV) rarely causes adult lower urinary tract obstruction and is rarely linked to posterior urethral calculi. A 21-year-old male with 5-year history of (LUTS) had a 19 × 22 × 39 mm posterior urethral stone removed via open cystolithotomy (35 mm stone). Persistent poor flow led to RUG/MCUG showing urethral dilatation and cystoscopy confirming PUV, ablated with holmium laser. Post-ablation uroflowmetry improved markedly. Adult PUV with impacted calculus is extremely rare, mimicking isolated stone disease. High suspicion for PUV in young males with posterior calculi and post-removal LUTS is vital; cystoscopy and voiding studies ensure accurate diagnosis and management.
Long-term sexual function, fertility, and quality of life after childhood treatment for posterior urethral valves: a systematic review of clinical outcomes.
Posterior urethral valves are the leading cause of congenital bladder outlet obstruction in boys and can result in long-term lower urinary tract dysfunction and reduced quality of life. Sexual and reproductive consequences in this population are poorly described and understood. The purpose of this systematic review (SR) was to assess the long-term sexual functions and fertility outcomes in patients treated in the pediatric age for posterior urethral valve (PUV). A SR was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. MEDLINE, Embase, and Scopus were searched for studies reporting sexual or reproductive outcomes in individuals with a history of PUV. The primary outcomes were prevalence and nature of sexual dysfunctions and fertility issues. Risk of bias was assessed using ROBINS-I or the Newcastle-Ottawa Scale, as appropriate. Eleven studies (2 non-randomized comparative studies and 9 case series) including 2723 individuals met the inclusion criteria. Most patients exhibited preserved erectile function, ejaculatory function was variably reported with most individuals described normal ejaculation. Semen analysis, reported in 7 studies, revealed normal parameters in approximately half of the cohorts, though abnormalities were noted in patients with renal impairment or complex urological histories. Paternity rates varied widely (6%-58%) and were lower in individuals with renal dysfunction or urinary incontinence. Only one study used validated tools to assess quality of life, showing negative impact in domains related to continence and renal status. Most males treated for PUV during childhood reach adulthood with preserved sexual and reproductive potential. Given the heterogeneity and methodological limitations of the current literature, prospective studies using standardized outcome measures are needed to better define long-term risks and inform clinical follow-up.
Fulguration of Anterior Membrane by Endoscopy (FAME): A modified technique of posterior urethral valve fulguration reduces the incidence of urethral strictures.
The standard technique of endoscopic fulguration of posterior urethral valves (PUV) involves fulguration at 5,7 and 12o'clock, with urethral stricture being a major complication. Since the anterior portion of the valve is the point of maximal obstruction, hence we modified the technique by fulgurating only the anterior part of the valve between 10 and 2 o'clock positions. We present the results of the standard and modified techniques of PUV fulguration. We hypothesised that the modified technique might reduce the risk of urethral strictures, while achieving adequate valve ablation. All PUV undergoing cystoscopic fulguration in our unit over a 6-year period were divided into 2 groups (group 1: standard 3-point fulguration; group 2: modified anterior fulguration). For valve fulguration, a 3F bugbee or hook electrode was used with appropriate sized compact cystoscope in all cases (6F in infants and 8/9.8F in older children). The two groups were compared for the adequacy of fulguration and urethral stricture formation during follow-up. Fifty-nine cases were included (22 group 1, 37 group 2). The mean age at fulguration was comparable between both groups. Follow-up voiding cystourethrogram and check cystoscopy revealed urethral stricture in 3 and residual valves in 4 children. The incidence of urethral stricture was significantly higher (p=0.047) in group 1 (3/22, 13.6 %) compared to group 2 (0/37, 0 %); there was no difference in the incidence of residual valves between both the groups (0/19, 0 % group 1 vs 4/37, 10.8 % group 2, p=0.288). We adopted a modified technique of PUV fulguration limiting diathermy application to the anterior part of PUV. There is no consensus on the best method of PUV ablation; commonly used techniques are cold knife and diathermy ablation. Incision with cold knife may cause bleeding, and may have a higher incidence of residual valves. Diathermy may predispose to more urethral strictures, especially when applied at 3 points circumferentially inside the small urethra, as in the standard technique of fulguration. Several autopsy and videocystoscopic studies, including our own observations, have documented that the valve is, in reality, a membrane with a posterior opening, the anterior part of the membrane being the major obstructing component. The results of the present study seem to be in agreement with this morphology of PUV. The modified technique of only anterior PUV fulguration resulted in significantly reduced incidence of urethral strictures with no increase in residual valves compared to the standard technique.
