An AI Approach to Differentiating Lung Squamous Cell Carcinoma From Metastases of Other Origins.

Distinguishing primary lung squamous cell carcinoma (SCC) from squamous metastases to the lung is a clinical challenge due to histopathologic similarities. Accurate diagnosis is essential to guide treatment decisions. To assess the utility of an artificial intelligence (AI) approach that includes evaluation of key orthogonal evidence in distinguishing primary lung SCCs from metastatic tumors of other tissue origins. This cross-sectional study used GPSai, a tissue-of-origin AI model run automatically on each sample submitted for molecular profiling, to flag potential misdiagnoses among research-eligible cases submitted as lung SCC. Molecularly profiled cases within the Caris Life Sciences clinicogenomic database from January 1, 2024, to January 31, 2025, were queried. All cases were reviewed by board-certified pathologists. The primary outcome was the rate of misdiagnosis among presumed lung SCCs confirmed by pathologist review and orthogonal evidence, which included clinical history and clinical findings, GATA3 and uroplakin II immunohistochemistry for urothelial carcinoma, UV variant signature for cutaneous SCC, CD5 and CD117 (c-KIT) immunohistochemistry for thymic carcinoma, and human papillomavirus positivity for orogenital SCC (eg, head and neck, cervical). Through a combination of AI and orthogonal evidence, 123 (3.1%) misdiagnoses were confirmed among 3958 cases submitted as presumed lung SCC (patients misdiagnosed: median [range] age, 71 [39 to >89]; 76.4% male). The cohort included 50 cutaneous SCCs (40.7%), 33 orogenital SCCs (26.8%) (including 25 head and neck [75.8%]), 20 urothelial carcinomas (16.3%), 15 thymic carcinomas (12.2%), 4 NUT carcinomas (3.3%), and 1 prostate SCC (0.8%). Ninety-two of the 123 patients (74.8%) had clinical history or findings consistent with the new diagnosis. Eighty-eight cases (71.5%) had differences in guideline-preferred first-line systemic therapies following the diagnosis change. In this cross-sectional study of patients diagnosed with lung SCC, a meaningful number of patients experienced misdiagnosis, which was identified using a multipronged AI-assisted approach. Diagnosis changes prompted by AI and orthogonal evidence may assist clinicians in prognostication and therapy selection.

Neuro-ophthalmic presentation of leptomeningeal metastasis of thymoma: a case report.

Leptomeningeal disease (LMD) of the brain and spinal cord can present with visual loss or diplopia. Although LMD can occur in many forms of neoplasia, thymoma-related LMD is exceedingly rare. A 53-year-old Hispanic male with a history of chest pain, weight loss, and night sweats was diagnosed with stage 4 thymoma with lung and pleural metastasis. He received chemotherapy for metastatic thymoma. Few months later, patient presented with severe right-sided facial pain and lip numbness, ptosis and double vision. The patient was diagnosed with multiple cranial and spinal nerve involvement due to thymomatous LMD, confirmed on magnetic resonance imaging and lumbar puncture. LMD is a rare presentation of a malignant thymoma. Current guidelines for thymoma management emphasize the importance of staging imaging to rule out distant metastasis. Our case highlights the importance of a head-to-mid-thigh positron emission tomography (PET) scan in patients with known metastatic thymomas, with multiple PET scans, if possible, at regular intervals, owing to the aggressive nature of metastatic thymomas. Clinicians should be aware of the neoplastic (e.g., metastatic disease and LMD) and paraneoplastic (e.g., thymoma-related myasthenia gravis) neuro-ophthalmic presentations of thymoma.

Thymic carcinoma presenting with cardiac tamponade and superior vena cava invasion.

We present the case of a woman in her 40s who presented with progressive shortness of breath, facial flushing, tachycardia that worsened when supine and bilateral leg swelling. Initial evaluation included a chest X-ray, which revealed widened mediastinum and a transthoracic echocardiogram with evidence of cardiac tamponade. A pericardiocentesis was performed, draining 900 mL of haemorrhagic fluid. Subsequent CT imaging identified a large anterior mediastinal mass invading into the superior vena cava (SVC) and extending towards the right atrium. A needle biopsy confirmed the diagnosis of thymic carcinoma. The patient remained stable and was started on chemotherapy. This case highlights the importance of considering SVC invasion in patients presenting with cardiac tamponade and mediastinal masses, particularly when positional symptoms such as orthostatic tachycardia and facial flushing are present.

Transforming Growth Factor-β1 as a Biomarker of Malignant Behavior and PD-L1 Expression in Thymic Epithelial Tumors.

The clinical significance and molecular mechanisms underlying tumor progression in thymic epithelial tumors (TETs) remain largely unclear. In this retrospective single-center study, we evaluated the prognostic value of transforming growth factor-beta 1 (TGF-β1) and its relationship with programmed death-ligand 1 (PD-L1) expression. A total of 92 patients with surgically resected TETs, including 79 thymomas and 13 thymic carcinomas, were included. Immunohistochemical analyses were performed to assess the expression of TGF-β1, PD-L1, phosphorylated Smad2 (pSmad2), and pSmad3. Associations between TGF-β1 expression and clinicopathological features were analyzed, and mechanistic interactions were investigated using two thymic carcinoma cell lines exposed to exogenous TGF-β1. High TGF-β1 expression was observed in 28% of patients and was significantly associated with advanced Masaoka stage (III/IV), shorter tumor doubling time (median 328 vs. 713 days, p = 0.042), and lower 5-year freedom from recurrence (FFR) rates (58.1% vs. 95.1%, p < 0.001, log-rank test). Coexpression of high TGF-β1 and PD-L1 was linked to the poorest prognosis (5-year FFR: 46.1%) and was identified as an independent predictor of recurrence (adjusted hazard ratio: 7.15; 95% confidence interval: 1.20-42.8). Immunohistochemically, TGF-β1 expression positively correlated with PD-L1 and pSmad2/3 expression. In vitro, TGF-β1 stimulation upregulated PD-L1 expression in a dose-dependent manner, accompanied by increased pSmad2/3 activation. These findings indicate that high TGF-β1 expression demarcates a biologically aggressive TET phenotype and, together with PD-L1, refines postoperative risk stratification, while its ability to drive PD-L1 via Smad signaling could support the blockade of these pathways as a potential therapeutic strategy.
